In cases of postpartum vulvar swelling, prolapse of the uterus or vaginal walls, and inability to close the Yumen (vaginal opening), use one sheng of calcined litharge (dust stone), stir-fried until extremely hot. Add two sheng of boiling water, pour in the ash, and wait for it to cool and clarify. Sit in the solution to soak the Yumen; after a short while, it will recover as before.
For postpartum swelling and pain of the Yumen, remove the hard shell of cuttlefish bone (Hai Piao Xiao), grind into a fine powder, mix with eggshell paste, and apply topically. The swelling will subside immediately.
Postpartum failure of the Yumen to contract is due to deficiency of Qi and Blood. It is appropriate to administer Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction), doubling the dosage of Shengma (Cimicifugae Rhizoma).
Postpartum hypochondriac pain is still caused by stubborn phlegm and blood stasis. Pain on the left side indicates phlegm, while pain on the right side indicates blood stasis. Buzhong Yiqi Tang is appropriate. If stubborn phallm is on the left, add Baijiezi (Sinapis Semen). If blood stasis is in the right hypochondrium, add Zhike (Aurantii Fructus) and Chaihu (Bupleuri Radix).
Postpartum vulvar pruritus, or accompanied by swelling and pain, is due to damp-heat. It is appropriate to fumigate and wash with herbal decoction.
Herbs: Jingjie (Schizonepetae Herba), Baizhi (Angelicae Dahuricae Radix), Baifan (Alumen), Chuanjiao (Zanthoxyli Pericarpium), Xingren (Armeniacae Semen Amarum), Jiegeng (Platycodonis Radix), Xixin (Asari Radix et Rhizoma).
Decoct and use for fumigation and washing.
In cases of postpartum vulvar pruritus, use 1 liang of Chuangzi (Cnidii Fructus) and 2 qian of Baifan (Alumen), decoct, and wash frequently; the pruritus will cease naturally.
Sudden onset of massive postpartum hemorrhage, falling in clots like a uterine bleeding disorder (Beng Lou), is due to extreme deficiency of Qi and Blood, combined with weak Spleen and Stomach function. This causes Qi and Blood to attack the Spleen and Stomach, leading to disharmony of Stomach Qi, resulting in this syndrome, which represents the collapse of Ying (Nutritive) and Wei (Defensive) Qi. One should harmonize the Blood and regulate Qi, administering Siwu Zhijing Tang (Four Substances Hemostatic Decoction).
Herbs: Shudi (Rehmanniae Radix Preparata), Baishao (Paeoniae Radix Alba), Danggui (Angelicae Sinensis Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Baiye (Plagiopteridis Folium), Fuling (Poria), Xiangfu (Cyperi Rhizoma), Ejiao (Colla Corii Asini), Puhuang (Pollinis Typhae), Baizhu (Atractylodis Macrocephalae Rhizoma), Zaoren (Ziziphi Spinosomi Semen), Chenpi (Citri Reticulatae Pericarpium), Renshen (Ginseng Radix et Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma).
Occasionally lifting heavy objects postpartum may cause the bladder (ancient term: Niaobao) to prolapse externally and fail to retract. This is due to Qi deficiency and Cold in the Blood; lifting heavy objects and straining injures the tendons/vessels, causing prolapse and inability to retract. In cases where it does not return to its original position after three to four months, or even half a year to a year, Shouyin San (Retract Yin Powder) is appropriate.
Herbs: Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma), Rougui (Cinnamomi Cortex), Zhike (Aurantii Fructus), Shengma (Cimicifugae Rhizoma), Wuyu (Evodiae Fructus), Chenxiang (Aquilariae Lignum Resinatum).
Combine with Siwu Tang (Four Substances Decoction).
Difficult childbirth and the desire for sterilization are not trivial matters. Some women suffer repeated trauma from hooks and cuts due to difficult deliveries. Others, such as those of irregular status or nuns/prostitutes, who do not wish to conceive and seek to terminate fertility, often use toxic substances to abort. They fail to realize that although fertility is stopped, the underlying damage is deeper. Wise individuals should take this as a warning.
Sanbai Wan (Three Whites Pill) treats female infertility.
Herbs: Baiji (Bletillae Rhizoma), Bailian (Tupistrae Chinensis Bulbus), Baifuling (Poria cum Agarwood - note: text says Baifuling, usually just Poria, but here likely distinct or typo for standard Poria, sticking to text entity), Qinjiao (Gentianae Macrophyllae Radix), Houpo (Magnoliae Officinalis Cortex), Danggui (Angelicae Sinensis Radix), Wuyu (Evodiae Fructus), Renshen (Ginseng Radix et Rhizoma), Rougui (Cinnamomi Cortex), Rushen (Olibanum). (Each 4 qian).
Grind into powder, form pills with honey, size of paulownia seeds. Take 30 pills per dose, swallowed with wine on an empty stomach.
