Postpartum heat and oppression, with qi rushing upward and counterflow, transforming into Hongkejiao (a condition characterized by swelling and heaviness of the lower legs and weakness), should be treated with Xiaoxuming Tang (Minor Continuation Life Decoction), removing Ephedrae Herba (Mahuang, 麻黄), Gypsum Fibrosum (Shigao, 石膏), and Aconiti Lateralis Radix Praeparata (Fuzi, 附子). This occurs because the patient was previously affected by the pathogenic factors of wind, cold, summer-heat, dampness, dryness, and fire. Due to the deficiency of blood and qi after childbirth, these pathogens invade the foot meridians, taking advantage of the deficiency to manifest. In such cases, Duhuo Jisheng Tang (Angelicae Pubescentis and Taxillus Decoction) is indicated.
Duhuo Jisheng Tang (Angelicae Pubescentis and Taxillus Decoction)
Du Huo (1 qian) Sang Ji Sheng (8 fen) Du Zhong (1 qian) Niu Xi (1 qian) Xi Xin (7 fen) Qin Jiao (1 qian) Bai Fu Ling (1 qian) Bai Shao (1 qian, stir-fried with wine) Rou Gui (6 fen) Chuan Xiong (8 fen) Fang Feng (7 fen) Ren Shen (8 fen) Zhi Gan Cao (7 fen) Shu Di Huang (1 qian) Dang Gui (1 qian). Grind all ingredients into a coarse powder, decoct in water, and take orally.
Generalized body pain after childbirth, caused by early labor and walking, leads to abnormal ascending and descending of qi and blood, which stagnate between the joints, pulling on the sinews and vessels. This may result in spasms or contractures of the hands and feet, preventing extension and flexion, thus causing pain throughout the body and joints. The appropriate formula is Chengtong San. If lochia (postpartum uterine discharge) is not cleared completely and flows into the generalized body, limbs, joints, waist, and feet, causing pain, Rushen Tang is indicated.
Chengtong San (Pain-Relieving Powder) Treats敗血 (stagnant blood) flowing through the meridians.
