[Shu He's Song on Sudden Abdominal and Epigastric Pain] Sudden severe pain in the heart (epigastrium) and abdomen, with a bluish complexion, cold sweat, and shortness of breath indicates imminent peril to life: If bleeding from the lower orifice continues unceasingly, with fetal Qi rushing upward and cold oppression in the heart and abdomen, the body will surely be damaged.
When abdominal pain during pregnancy is accompanied by bleeding, it is called Tai Lou (Fetal Leakage). It resolves upon delivery, but the born child will be physically weak and deficient. Zi Su Yin (Perilla Decoction) added with Chen Pi (Tangerine Peel) and Sha Ren (Amomum Fruit) may be used. If stomach pain is severe, Zhi Mi Tang (Finger-Enchanting Decoction) or Yang Wei Tang (Stomach-Nourishing Decoction) should be used. There are also cases of Pregnancy Carbuncle; how can they be distinguished? If An Tai Yin (Fetus-Securing Decoction) and herbs for digesting food and regulating Qi are ineffective, but there is swelling, pain, and shining skin near the lower umbilicus, it is an abdominal carbuncle. Because it occurs in pregnant women, it is named Pregnancy Carbuncle. Shi Bu Tuo Li San (Ten-Ingredient Supporting Interior Powder) is appropriate; this formula tonifies without hindering the fetus. Secondly, Qian Jin Tuo Li San (Thousand-Gold Supporting Interior Powder) is also acceptable. Cases of cold pain in the lower umbilical region, abdominal distension, frequent urination, and loose stools are caused by deficiency of Zheng Qi (Upright Qi) and excessive consumption of raw and cold foods. An Tai Yin (Fetus-Securing Decoction) added with Zhi Ke (Aurantium Fruit) and Qi-regulating herbs is suitable. For lower abdominal pain, the pulse must be examined. If the condition allows for securing the fetus, Zi Su Yin (Perilla Decoction) should be used to secure it; if not, blood-invigorating and Qi-moving herbs should be employed. Severe internal injuries may require purgation. If blood stasis accumulates and does not descend, it inevitably causes abdominal pain; Jin Xie San (Gold-Powder Powder) should be taken. Or, if retained food forms masses causing Qi and Blood stasis and pain, Cong Bai San (Scallion White Powder) is the primary treatment. For all types of blood-stasis pain, Rou Zhi San (Meat-Roasted Powder) and Bai Zhu Jian (Atractylodes Decoction) are used. Severe lower back pain indicates Kidney Deficiency, where the fetus is prone to miscarriage; An Tai Yin (Fetus-Securing Decoction) should be taken urgently to stabilize the fetus. If there is no aversion to cold, Huang Qin (Scutellaria Root) can be added to Zi Su Yin (Perilla Decoction). If fetal movement is frequent, accompanied by waist and abdominal pain, An Tai Yin (Fetus-Securing Decoction) is also appropriate.
For pregnant women with severe hypochondriac pain, there are three explanations: either due to weeping, anger/pain, or internal injury. In cases of internal injury with an existing fetus, blood-breaking and damaging herbs must not be used. Only Tong Bian Jiu (Child Urine Wine), or Zi Su Yin (Perilla Decoction) added with Bai Shao (White Peony Root), Dang Gui (Angelica Sinensis), and Sha Ren (Amomum Fruit), decocted with Ren Shen (Ginseng) and Tong Bian (Child Urine), is suitable, which is sufficient to secure the fetus. Some say that for hypochondriac pain, only Jie Zi (Mustard Seed) can penetrate, while Chai Hu (Bupleurum Root) and Zhi Ke (Aurantium Fruit) can also be used. For back pain, it is due to Qi stagnation and failure to circulate; Zi Su Yin (Perilla Decoction) is suitable. Dry mouth, irritability, and inability to lie down during pregnancy are named Zi Fan (Fetal Irritability); Zi Su Yin (Perilla Decoction) is the main treatment. For sores on the mouth and tongue, and swelling and pain in the throat, Fang Feng Tong Sheng San (Saposhnikovia Unblocking Hero Powder) must absolutely not be used. Milk secretion before delivery in pregnant women is named Ru Qi (Breast Weeping); children born under these conditions are often infertile. If a fetus cries in the womb, medication is unnecessary. Throw soybeans on the ground and ask the pregnant woman to pick them up carefully; the crying will stop naturally.
