(Also known as Baihu Lijie Feng [White Tiger Joint Bi Syndrome])
Lijie pain (pain in the joints) may be caused by irregular diet and lifestyle, or by improper regulation of the Seven Emotions (Qing) and Six Exogenous Pathogens (Yin), leading to Spleen-Stomach deficiency, loose Cou Li (pores and interstitial spaces), and invasion of external pathogens; or due to internal stirring of Liver Fire and consumption of Liver Blood; or manifesting as limb pain; or difficulty in extending the joints; or sudden severe pain; or wandering pain without fixed location; or internal heat and afternoon tidal fever, spontaneous sweating and night sweats; or irregular menstruation and poor appetite. Its treatment methods are as follows: for those belonging to Wind pathogen, use Xiao Xuming Tang (Minor Continuation Life Decoction). For wandering and migrating pain, use Loulu San (Radix Euphorbiae Humifusae Powder). For bone and joint pain, use Sisheng Wan (Four Living Pill). For Damp-Heat pain, use Qingzao Tang (Clear Dryness Decoction); if accompanied by Phlegm, assist with Erchen Tang (Two Aged Substances Decoction). For Liver Fire, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) plus Qianghuo (Notopterygii Rhizoma et Radix) and Chuanxiong (Chuanxiong Rhizoma). For Spleen Qi Stagnation, use Jiawei Guipi Tang (Modified Restore the Spleen Decoction) plus Qianghuo and Chuanxiong. For Blood deficiency, use Siwu Tang (Four Substances Decoction) plus Qianghuo and Chuanxiong. For Qi deficiency, use Sijunzi Tang (Four Gentlemen Decoction) plus Qianghuo and Chuanxiong. For combined Qi and Blood deficiency, use Bazhen Tang (Eight Treasures Decoction) plus Qianghuo and Chuanxiong. For menstrual period preceding schedule with pain, take Jiawei Xiaoyao San as the main formula. For menstrual period delayed with pain, take Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) as the main formula. Generally speaking, pain that cannot bear pressure belongs to excess of both Pathogenic Qi and Healthy Qi; pain that relieves upon manual pressure belongs to deficiency of both Pathogenic Qi and Healthy Qi. If pain occurs after overwork, it is Healthy Qi deficiency; if pain occurs due to improper diet, it is Spleen-Stomach deficiency; if pain occurs due to anger and irritation, it is Liver Fire; if pain occurs during menstruation, it is Blood deficiency. All these cases prioritize consolidating Healthy Qi, assisted by herbs treating the disease.
Clinical Cases
A woman suffered from spontaneous sweating and night sweats, fever and afternoon tidal fever, fatigue, reduced appetite, irregular menstruation, and excessive expectoration for two years. Her whole body ached, worsening on cloudy and rainy days. Pain ceased after using Xiao Xuming Tang. She was cured after taking thirty-plus doses of Buzhong Yiqi Tang and Jiawei Guipi Tang. The symptoms of spontaneous sweating etc. were all caused by emotional constraint damaging Spleen Qi, preventing it from nourishing the Zang-Fu organs. Therefore, the aforementioned two formulas were used to specifically target the Spleen and Stomach. If cold and bitter herbs were used to clear fire, or herbs regulating Qi and resolving Phlegm were employed, they would further damage the generating Qi, often leading to untreatable conditions.
A woman, due to anger, experienced heavy menstruation, fever, thirst, contraction of the left eye, trembling of the head and neck, convulsions of the four limbs, and whole-body pain. This was anger stirring Liver Fire, with Liver Blood deficiency generating internal Wind. After several doses of Jiawei Xiaoyao San plus Gouteng (Uncariae Ramulus cum Uncis), her symptoms gradually improved. She recovered after conditioning with Bazhen Tang.
A woman had early menstruation and chronic pain that always occurred after labor, relieved slightly only by heavy pressing with multiple hands. This indicated Qi and Blood deficiency with Fire. The pain was cured using Shiquan Dabu Tang (Ten Thousand Complete Great Tonifying Decoction) plus Duhuo (Angelicae Pubescentis Radix), and menstruation was regulated using Liuwei Wan (Six-Ingredient Pill) and Xiaoyao San.
A woman suffered from Lijie pain, fever, thirst, poor appetite, and delayed menstruation. Various medicines failed. Her pulse was large and forceful but fine and weak upon deep pressure. Pain ceased after four doses of Fuzi Bawu Tang (Aconite Eight Ingredients Decoction). Healthy Qi recovered using Jiawei Xiaoyao San, and menstruation was regulated using Liuwei Wan.
A woman was obese, had inherent internal heat, early menstruation, and suffered from gouty arthritis. Her lower body was mildly swollen and painful, with frequent urination, heavy body, and slow pulse. The syndrome belonged to Wind-Dampness, with Blood deficiency and Heat. First, she took four doses of Qianghuo Shengshi Tang (Notopterygium Overcoming Dampness Decoction), and swelling and pain gradually subsided. After several doses of Qingzao Tang, urination became clear. After more than ten doses of Jiawei Xiaoyao San, internal heat gradually resolved. Due to food stagnation, fever and pain persisted, with facial edema. She was cured using Liujun Zi Tang (Six Gentlemen Decoction) plus Chaihu (Bupleuri Radix) and Shengma (Cimicifugae Rhizoma). Later, due to anger, she experienced abdominal distension, aversion to cold, fever, and vomiting, indicating Wood insulting Earth (Spleen). She stabilized using the previous prescription plus Zhizi (Gardeniae Fructus) and Muxiang (Aucklandiae Radix). Only the sensation of lower abdominal heaviness and urgency to defecate remained, indicating Spleen Qi sinking. She was cured using Buzhong Yiqi Tang. Later, due to overwork and anger, she experienced vomiting, expectoration, whole-body swelling and pain, and sudden onset of menstruation with chills and fever. She used Liujun Zi Tang plus Chaihu and Zhizi to support Healthy Qi and clear Liver Fire, eliminating swelling, pain, and vomiting. She then used Buzhong Yiqi Tang to raise Yang Qi and strengthen Ying Qi, curing the menstrual chills and fever.
A woman had poor appetite, fear of wind and cold, suffered from gouty arthritis, vomiting, chills and fever, with a wiry and tight pulse. Limb pain healed using Fuzi Bawu Tang. Lower back pain healed using Duhuo Jisheng Tang (Duhuo and Sangjisheng Decoction). Only knee swelling and pain remained, which subsided using Dafangfeng Tang (Great Wind-Expelling Decoction). Healthy Qi recovered using Jiawei Guipi Tang and Xiaoyao San.
