After childbirth, seeing black spots before the eyes, dizziness, fainting, and loss of consciousness is called "Blood Fainting" (Xue Yun). There are three causes: first, exhaustion from overwork leading to Qi depletion and spirit confusion; second, massive blood loss leading to Qi collapse and syncope; third, Phlegm-Fire taking advantage of deficiency to rise upward, causing unclear spirit. These three conditions all represent a state where the Hun (Ethereal Soul) does not follow the Shen (Spirit), and vital functions are nearly ceased.
If physicians blindly trust ancient formulas, mistaking this fainting for "Evil Blood Snatching the Heart," and lightly administer blood-dispersing agents; or if they片面ly attribute it to Phlegm-Fire, neglecting tonification, and exclusively using methods to eliminate and descend phlegm-fire, these are errors that cost lives.
One should urgently administer a large dose of Shenghua Tang (Transform the Stasis Decoction) to first resolve stasis and relieve pain. If it is truly a case of physical collapse manifested by pallor, or fainting due to excessive sweating, one should urgently administer one dose of Shenghua Tang, followed immediately by adding 2-3 qian of Renshen (Ginseng Radix et Rhizoma). If it is fainting due to severe overwork, or fainting due to metrorrhagia and Qi collapse, continue administering two doses consecutively, then add 2-3 qian of Renshen. One must absolutely not hesitate to use Renshen because it is considered a tonic.
If it is truly fainting caused by Phlegm-Fire rising due to deficiency, add Ju Hong (Citri Reticulatae Pericarpium Rubrum) 4 fen to Shenghua Tang. If the deficiency is severe, also add Renshen. If the patient is obese and prone to phlegm, additionally add Zhu Li (Bambusae Succus) 7 fen and a small amount of Sheng Jiang Zhi (Zingiberis Rhizoma Recens Expressum). For the above three types of fainting, blood-breaking formulas must not be used. If the pain from stasis is severe, administer Yi Mu Wan (Motherwort Pill), Lu Jiao Hui (Cornu Cervi Degelatinatum Ash), Yuan Hu San (Corydalis Powder), or Du [San] (Single Herb Powder) with the decoction.
For postpartum fainting due to physical collapse or excessive sweating:
Chuan Xiong (Ligustici Chuanxiong Rhizoma) and Dang Gui (Angelicae Sinensis Radix) (each 4 qian), Jiang Tan (Zingiberis Rhizoma Carbonisatum) and Zhi Gan Cao (Glycyrrhizae Radix et Rhizoma Praeparata cum Melle) (each 4 fen), Tao Ren (Persicae Semen) (10 grains), Renshen (Ginseng Radix et Rhizoma) (3 qian), Jing Jie (Schizonepetae Spica) (4 fen), and Zao (Jujubae Fructus) (1 date). Decoct and take orally. In cases of pulse collapse and physical collapse nearing extinction, add Renshen (3-4 qian) and administer frequently for rescue. The same applies to excessive sweating. If there is no sweating and no signs of collapse, only administer a large dose of Shenghua Tang without adding Renshen. If the pulse on the left hand and foot disappears, this is also a critical condition requiring the addition of Renshen. If stasis pain is severe, add Rou Gui (Cinnamomi Cortex) (7 fen); if thirsty, add Mai Dong (Ophiopogonis Radix) (1 qian) and Wu Wei Zi (Schisandrae Chinensis Fructus) (10 grains); if sweating is excessive, add Ma Huang Gen (Ephedrae Radix et Rhizoma) (1 qian). If there is no stasis pain, Renshen can be used in conjunction.
In treating any critical postpartum conditions, medicines must be administered frequently, 3-4 doses within a day and night, to turn danger into safety. Otherwise, if treated normally, it will be too late.
