Postpartum edema due to Water-Qi, swelling of hands and feet, skin appearing shiny and lustrous. This is caused by Spleen deficiency failing to control Water, and Kidney deficiency failing to transport Water. Treatment must primarily greatly tonify Qi and Blood, assisted by Cangzhu Rhizoma Atractylodis Lanceae, Baizhu Atractylodis Macrocephalae Rhizoma, and Fuling Poria to tonify the Spleen. If there is fullness/stagnation, use Banxia Rhizoma Pinelliae Preparatum, Chenpi Pericarpium Citri Reticulatae, and Xiangfu Rhizoma Cyperi Rotundi to restrain it. For deficiency, add Renshen Radix Ginseng and Mutong Caulis Akebiae. For heat, add Maidong Radix Ophiopogonis and Huangqin Radix Scutellariae to clear Lung Metal.
([Wu's Note] Pressing an area that leaves a depression that does not rise indicates Qi stagnation; pressing an area that leaves no depression and rises immediately indicates Water retention. Distinguishing by this method is a key technique.)
Edema occurring seven days or more postpartum.
Renshen Radix Ginseng (2 qian), Baizhu Atractylodis Macrocephalae Rhizoma (3 qian), Fuling Poria, Baishao Radix Paeoniae Alba (1 qian each), Chenpi Pericarpium Citri Reticulatae (0.5 qian), Mugua Fructus Chaenomelis (0.8 qian), Zisu Folium Perillae, Mutong Caulis Akebiae, Dafupi Pericarpium Arecae Catechu, Cangzhu Rhizoma Atractylodis Lanceae, Houpo Cortex Magnoliae Officinalis (0.4 qian each). If stool is obstructed, add Yuliren Semen Pruni Armeniacae and Maren Semen Persicae (1 qian each). If Cold-Evil, Damp-Heat, or pathogenic factors injure the exterior resulting in anhidrosis and swelling, add Jiangpi Cortex Zingiberis Recens, Banxia Rhizoma Pinelliae Preparatum, and Zisu Folium Perillae within the Qi and Blood tonifying formula to induce sweating and release the exterior. Postpartum Wind-Dampness invading the Spleen Meridian, causing Qi and Blood stagnation, leading to puffy face and eyes, swelling of limbs, and wheezing/dyspnea, is suitable for treatment with Wupi Yin (Five-Peel Decoction).
