Within seven days postpartum, if Shenghua Decoction (Invigorating Blood Decoction) has not been taken and pain from retained blood clots persists, Shenghua Decoction may be used to clear the clot-related pain. Even if two or three doses of Shenghua Decoction have already been taken within three days, after taking a few more doses, the pain from retained blood clots will naturally subside.
Within seven days postpartum, tonifying herbs such as Ginseng Radix et Rhizoma (Renshen), Atractylodis Macrocephalae Rhizoma (Baizhu), and Astragali Radix (Huangqi) should not be used lightly, lest it cause the pain from retained blood clots to persist indefinitely. If the patient suffers from Qi and Blood collapse, syncope, excessive sweating leading to cold limbs, unceasing irritability and thirst, or rapid panting with a faint pulse, these are all critical conditions. In such cases, one must act flexibly and use large amounts of Ginseng combined with Shenghua Decoction to rescue the patient. If signs of life return after administration, the dosage of Ginseng should be reduced, and only Shenghua Decoction should be continued to eliminate the pain from retained blood clots. Once the pain from retained blood clots has ceased, tonics like Ginseng can then be reintroduced. In summer, parturient women take Shenghua Decoction primarily to eliminate pain from retained blood clots; if the abdomen is exposed to cold without proper covering, even if Shenghua Decoction is taken, the pain from retained blood clots will ultimately not subside.
If a parturient woman does not pass stool for eight or nine days postpartum, this is due to insufficient Blood causing dryness in the intestines. It is appropriate to use Shenghua Decoction supplemented with Semen Cannabisae (Huo Ma Ren) to moisten and unblock the bowels. Following the dosage ratio of Angelicae Sinensis Radix (Danggui) and Chuanxiong Rhizoma (Chuanxiong), add 1-2 qian of Ginseng Radix et Rhizoma (Renshen). Do not use Cathartics such as Rhei Radix et Rhizoma (Dahuang) to purge the bowels.
Within one or two days postpartum, if abdominal pain persists despite having taken two or three doses of Shenghua Decoction, but can be relieved by rubbing and pressing, this indicates a Deficiency pattern. It is appropriate to use Shenghua Decoction with an addition of 2 qian of Ginseng Radix et Rhizoma (Renshen). (Wu Qian notes: If the pain is aggravated by pressure, it indicates an Excess pattern, and Modified Shenghua Decoction should be used.)
Furthermore, within seven days postpartum, if pain arises from blood coagulation due to exposure to cold, it is appropriate to use Shenghua Decoction with an addition of Cinnamomi Cortex (Rougui) 4-5 fen. By the time half a month or more than a month has passed... [Text fragment implies adjustment for later stages].
For critical postpartum conditions, Ginseng is used flexibly for emergency rescue. For obese patients with abundant Phlegm, or those who suffer sudden stroke due to violent anger, only Shenghua Decoction supplemented with Bamboo Sap (Zhuli) and Ginger Juice (Jiangzhi) is suitable.
For postpartum blood syncope, rapidly administering a large dose of Shenghua Decoction yields miraculous results. If the condition is indeed caused by constitutional weakness or pre-existing deficiency during pregnancy, resulting in postpartum fatigue and syncope, administering 1 qian of Shenghua Decoction and continuing for two doses can save the patient. For symptoms such as fever with sweating, shortness of breath with throat obstruction, mental confusion and discomfort, as well as diarrhea present before delivery that continues postpartum, along with coma and fatigue, add 1 qian of Ginseng Radix et Rhizoma (Renshen) to two doses of Shenghua Decoction for emergency rescue.
In cases of postpartum metrorrhagia (profuse bleeding) accompanied by syncope, fatigue, and cold limbs, if the body and mind feel warm, open the patient's mouth and pour in Ginseng-supplemented Shenghua Decoction. If the medicine cannot be swallowed, insert a goose feather into the throat to induce swallowing or stimulate the gag reflex. Once the abdomen gradually becomes warm, the number of doses is not fixed until the patient survives.
For postpartum metrorrhagia with Qi collapse, restlessness, eyes closed as if possessed, and incoherent speech, rapidly administer Shenghua Decoction. After the first decoction, immediately follow with Dingzhi Yangrong Decoction (Calming Spirit and Nourishing Blood Decoction). This condition involves blood stasis manifesting as hallucinations of seeing ghosts.
Cold limbs and syncope of the hands and feet postpartum, or extreme dryness of the mouth with great thirst, indicate a critical state of severe Deficiency. Profound tonification is required to restore life. If thirst persists after taking Ginseng-supplemented Shenghua Decoction, further treatment is necessary... [Text fragment]. If long-term lack of food intake leads to vomiting upon taking medicine, use 1-2 qian of Ginseng Radix et Rhizoma (Renshen), 2-3 slices of Zingiberis Rhizoma Recens (Shengjiang), and a pinch of aged rice (Chenmi). Decoct separately in a new pot to calm the Stomach Qi. For any critical postpartum condition, administering three to four doses continuously within twenty-four hours is necessary. If treated according to standard methods, how could one possibly reconnect the dying Qi and rescue the acute crisis?
(Wu Qian notes: Decocting Sprouted Grain (Sheng Guya) and Caulis Bambusae in Medulla (Zao Xin Tu) to dissolve tea is also effective.)
