The *Daquan* (Compendium) states: Postpartum lochia that does not cease is caused by damage to the menstrual blood after childbirth, resulting in deficiency and insufficiency. Or, during delivery, if the evil blood (lochia) is not completely expelled and remains in the abdomen, with the zang-fu organs harboring old cold pathogens, this leads to disharmony of qi and blood, causing the lochia to drip continuously without stopping.
Dai Fu'an states: If women take too many medicines for stabilizing the fetus, whether in full-term delivery or miscarriage, although the fetus has been expelled, the evil blood may not be removed. Even after two or three months, the lochia may still drip incompletely. This is not comparable to retained dead blood; it is precisely because the blood that should have been discharged was obstructed by astringent medicines for stopping bleeding, leading to blood stasis and failure to transform. The treatment should facilitate blood flow and promote qi circulation, avoiding the accumulation of blood and closure of qi.
Xue Lizhai states: For the aforementioned conditions, if liver qi is hot and unable to generate blood, use Liuwei Wan (Six-Ingredient Rehmannia Pill). If liver qi is deficient and unable to store blood, use Xiaoyao San (Free and Easy Wanderer Powder). If spleen qi is deficient and unable to control blood, use Liujunzi Tang (Six-Gentlemen Decoction). If stomach qi sinks and cannot command blood, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). If there is stagnant heat in the spleen meridian causing blood to fail to return to its source, use Jiawei Guipi Tang (Modified Restore the Spleen Decoction). If fire from anger in the spleen meridian causes reckless bleeding, use Jiawei Siwu Tang (Modified Four-Substance Decoction). If both qi and blood are deficient, use Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction). If wind pathogen in the liver meridian causes blood to boil, use Yiwei Fangfeng Wan (Single-Ingredient Saposhnikovia Pill).
Shenzhen's Commentary: The above three passages describe the syndrome of postpartum lochia that does not cease. After childbirth, if bleeding continues, it generally lasts about one month. If it stops before one month, it indicates deficiency of qi and blood. If it persists dripping for more than one or two months, it is either due to qi deficiency failing to control blood, or as Xue Lizhai discussed, deficiency in the liver and spleen meridians, which aligns with the *Daquan*'s statement on deficiency and insufficiency of menstrual blood. It is also attributed to the zang-fu harboring old cold. However, blood flows when heated and coagulates when cooled; how can lochia that does not cease be caused by cold? Xue Lizhai attributes it to stagnant heat and fire in the liver and spleen, which indeed demonstrates a keen understanding of the disease mechanism. Nevertheless, following postpartum blood loss, methods of tonifying qi and lifting yang should be employed. For example, the *Qianjin Fang* treats lochia that does not cease, lasting half a year, using only Yiwei Shengma (Single-Ingredient Cimicifuga), decocted in wine, which embodies this principle. In cases where excessive downward discharge leads to loss of yin, presenting with cold rather than heat, Jiang (Zingiberis Rhizoma) and Gui (Cinnamomi Cortex) are also appropriate. Clinical judgment must be exercised carefully.
