Miao Zhongchun stated: In cases of postpartum dysentery, even if there is abundant accumulation and stagnation and extreme abdominal pain, one must not use purgative herbs such as Rheum palmatum (Dahuang, 大黄) to attack the pathogen, as this would damage Stomach Qi, rendering the condition incurable. It suffices to use Ginseng Radix et Rhizoma (Renshen, 人参), Angelicae Sinensis Radix (Danggui, 当归), Paeoniae Radix Alba (Baishao, 白芍), Red Yeast Rice (Hongqu, 红曲), and Cimicifugae Rhizoma (Shengma, 升麻) processed with vinegar, along with a doubled dose of Glycyrrhizae Radix et Rhizoma (Gancao, 甘草), supplemented with Motherwort Herba (Yimucao, 益母草) and Talcum (Huashi, 滑石). If lochia has not yet cleared, adding Frankincense (Ruxiang, 乳香), Myrrh (Moyao, 没药), Amomi Fructus (Sharen, 砂仁), and Donkey-hide Gelatin (Ejiao, 阿胶) will lead to spontaneous recovery.
Shen Zhai commented: The above five sections describe the syndrome of dysentery occurring after childbirth. Dysentery originates from external contraction of the Six Excesses (Liu Yin, 六淫) and internal injury from diet. In the postpartum period, treatment must account for the deficiency of Qi and Blood; therefore, the conventional methods for treating dysentery should not be used, but rather the regulation and tonification of the Spleen and Stomach should be prioritized. Furthermore, postpartum dysentery belongs to severe deficiency of Qi and Blood; it should not be treated by targeting the dysentery itself, but solely by tonifying Qi and Blood, primarily using large doses of Ginseng Radix et Rhizoma (Renshen, 人参) and Angelicae Sinensis Radix (Danggui, 当归).
The *Quan Shu* (Complete Works) states: Postpartum Ru Lao (Childbed Exhaustion) occurs when, shortly after delivery, due to weak Qi and Blood and unsettled diet, within less than a hundred days, improper nursing allows Wind-Cold pathogens to invade. These pathogens knot in the Qi and Blood, failing to warm the skin and muscles, resulting in weakness, fatigue, alternating lying down and rising, haggard appearance, indigestion, cough, dry mouth, dizziness, and pain in all joints. When Ying and Wei are affected by Wind pathogens, they flow into the Zang-Fu organs, causing frequent attacks in short intervals, occasional night sweats, chills and fever resembling malaria, oppression in the back and shoulders, inability to lift the limbs, heaviness in bed—these are the signs of Ru Lao.
Chen Liangfu stated: Women may suffer from difficult labor processes, exhausting their sinews and strength, combined with worry, fatigue, and overthinking, leading to emaciation, panting, and weakness, chills and fever resembling malaria, headache, spontaneous sweating, limb fatigue, cough with counterflow of phlegm, and cramping or stabbing pain in the abdomen. This is named Ru Lao.
The *Chan Bao Bai Wen* (Hundred Questions on Childbirth Treasures) states: Postpartum emaciation gradually developing into Ru Lao is caused by the depletion of Qi and Blood following childbirth. One must be cautious with daily activities and regulate diet, nursing for a hundred days to ensure freedom from disease. For women of middle age or those who experienced difficult labor, regardless of the time elapsed, one must wait until the body recovers balance before engaging in physical activity; otherwise, Qi and Blood will be injured again, and the syndrome of emaciation will manifest.
