The *Daquan* (Great Compendium) states: Postpartum incoherent speech, or delirious and rambling talk, as if seeing ghosts and spirits, has various origins. It must be differentiated and treated accordingly.
First, due to postpartum heart deficiency, with stagnant blood accumulating and interfering upward with the heart, causing delirious and solitary speech, one should refer to the section on "Suddenly Seeing Ghosts and Spirits" for treatment.
Second, due to postpartum organ deficiency, with the heart-mind startled and palpitations, unstable will and intention, chaotic speech without self-awareness, and restless spirit, one should refer to the section on "Palpitations and Fright" for treatment.
Third, there is residual wind-toxin; due to postpartum heart deficiency and weak qi, leading to stiffness and rigidity of the waist and back, or singing, crying, anger, and laughter, with chaotic speech, it should be treated as Wind Spasm (Feng Jing), referring to the section on "Heart Palpitations and Stroke" for treatment.
Fourth, postpartum cases are often caused by stagnant blood confusing the Heart Meridian, leading to manic speech or fainting and dizziness; one should seek treatment within the context of Blood Syncope (Xue Yun).
Fifth, postpartum contraction of wind-cold, with lochia abruptly ceasing, aversion to cold and fever resembling malaria, clarity during the day but delirium at night as if seeing ghosts, should be treated as Heat Entering the Blood Chamber (Re Ru Xue Shi), suitable for Hupo Dihuang Wan (Amber and Rehmannia Pill) and Siwu Tang (Four Substances Decoction).
Regarding the above syndromes, generally, postpartum treatment should first expel stagnant blood and generate new blood, then carefully examine the disease. One must not arbitrarily create names or novel ideas. Adjusting formulas and herbs requires precise syndrome differentiation; following ancient methods ensures safety.
Xue Lizhai's commentary: For the aforementioned syndromes, the primary approach should be to consolidate Stomach Qi, assisted by herbs addressing specific symptoms. If one solely attacks phlegm, it would be a mistake.
The *Daquan* states: Postpartum residual blood rushing to the heart causes empty irritability. This is likely due to not drinking child's urine after delivery, failing to treat the area below the heart, lying down too early, and consuming incompatible foods.
Chen Wuyue states: Normally, various types of empty irritability and heat are treated with Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction) and Wendan Tang (Warm Gallbladder Decoction). However, postpartum conditions differ from ordinary ones; herbs like Shigao (Gypsum) should not be used lightly, as their use can lead to death.
The *Jiyan Fang* (Collected Proven Prescriptions) states: Postpartum blood-thirst, which refers to thirst caused by blood deficiency. The formula uses Renshen (Ginseng), Maidong (Ophiopogon Root), Shengdi (Rehmannia Root), Gancao (Licorice Root), and Gualou (Trichosanthes Fruit). Additionally, for postpartum mental restlessness and thirst, Qingxin Lianzi Yin (Clear Heart Lotus Seed Drink) is suitable.
