[Xue Ji] Postpartum lower abdominal pain, commonly known as "child pillow mass" (Erzhen Kuai), should be treated with Shixiao San (Pain-Relieving Powder) to promote blood circulation and dissipate stasis. If lochia has been discharged but pain persists, use Sishen San (Four-Gentlemen Powder variant) to regulate and tonify. If ineffective, use Bazhen Tang (Eight-Treasure Decoction). If pain is accompanied by nausea or a desire to vomit, use Liujunzi Tang (Six-Gentlemen Decoction). If pain is accompanied by diarrhea, administer Liujunzi Tang with Sishen Wan (Four-Essence Pill). If diarrhea, pain, and tenesmus occur, administer Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with Sishen Wan. If there is fullness in the chest and diaphragm, aversion to food, acid regurgitation, or abdominal pain that cannot be pressed, this is caused by food stagnation; use Erchen Tang (Two-Aged Decoction) added with Hawthorn Fruit (Shanzha) and Atractylodes Macrocephala Rhizome (Baizhu) to digest and guide out stagnation. If food stagnation has cleared but pain remains, or if pressing does not cause pain, or if headache, vexation, heat, thirst, aversion to cold, and vomiting appear, this indicates damage to Middle Qi; the treatment should primarily focus on tonifying the Spleen and Stomach. If fever and abdominal pain occur, with exacerbation upon pressure, no aversion to food, and no acid regurgitation, this is due to blood stasis retention; use Shixiao San to dissipate it. If there is only fever and headache, or accompanied by abdominal pain but no pain upon pressure, this is Blood deficiency; use Siwu Tang (Four-Material Decoction) added with Fried Ginger (Paojiang), Ginseng (Renshen), and Atractylodes Macrocephala Rhizome (Baizhu) to tonify. *Bingji Yao* states: Diseases related to pregnancy and childbirth should be discussed from the perspective of the Jueyin Meridian. One must not violate the Stomach Qi and the Upper and Middle Jiaos; this constitutes the "Three Prohibitions": no sweating, no purgation, and no promoting urination. Sweating is similar to the syndrome of premature purgation in Cold Damage; purging the bowels causes rapid pulse and moves the Spleen; promoting urination leads to loss of body fluids and dryness of the Stomach. The principle of preparing medicines is that if these three prohibitions are not violated, Ying and Wei will naturally harmonize, and chills and fever will cease. If thirst occurs, use Baihu Tang (White Tiger Decoction); for weak Qi, use Astragalus Root (Huangqi); for stabbing blood pain, use Angelica Sinensis Radix (Danggui); for abdominal pain, add White Peony Root (Shaoyao). One should carefully observe the pulse and symptoms before using them. Mr. Danxi said: Postpartum, one should prioritize greatly tonifying Qi and Blood. Even if there are other miscellaneous syndromes, they should be treated as secondary. Most diseases are due to Blood deficiency, and releasing the exterior (sweating methods) should never be used. A postpartum woman had abdominal pain and fever, with a large pulse at the Qi Kou position. I believed it was food stagnation, but she did not believe me. Instead, she used blood-breaking and tonifying drugs, which resulted in chills, fever, headache, and vomiting of saliva. She then used drugs to descend fire, resolve phlegm, and regulate Qi, leading to cold extremities, diarrhea, and a sensation of bearing down. Only then did she regret it. She asked me why. I said: This is a transformational syndrome of Spleen-Stomach deficiency. The method should be warming and tonifying. Thus, I used Liujunzi Tang added with two qian of Fried Ginger, one qian of Cinnamon Twig (Guixin), and one qian of Aucklandia Root (Muxiang). After four doses, all symptoms subsided. Then, using Buzhong Yiqi Tang, her Yuan Qi was completely restored. Another woman had postpartum abdominal pain with tenesmus, profuse dysentery, physical fatigue, poor appetite, long-term emotional depression, cornified lip (Jianchun), heavy night sweats during sleep, insomnia, and mental exhaustion. I said this is Qi and Blood deficiency with Heat. I used Danggui Liu Huang Tang, frying Huangqin, Huanglian, and Huangbai black. After one dose, sweating stopped immediately; after two doses, it stopped completely. Then, Guipi Tang and Bazhen San were taken together, and her Yuan Qi gradually recovered, leading to cure. A postpartum woman had lower abdominal pain. Medicines for moving Qi and breaking blood were ineffective. Her pulse was surging and rapid, indicating that blood stasis had internally ulcerated into pus. She took Guazi Ren Tang (Melon Seed Decoction) for two doses, and the pain stopped. Then, Taiyi Gao (Great One Ointment) was used to discharge pus, leading to recovery. This condition is common postpartum; even if it is not an abscess, this formula is more effective. A postpartum woman had lower abdominal pain and difficulty urinating. She took Yiyi Ren Tang (Coix Seed Decoction) for two doses, and the pain stopped. Then, Siwu Tang added with Safflower (Honghua) and Peach Kernel (Taoren) was used to discharge blood stasis, leading to recovery. Generally, this syndrome is caused by disharmony of Ying and Wei, or retention of blood stasis. If the pulse is surging and rapid, pus has formed; if the pulse is merely rapid, there is slight pus; if the pulse is slow and tight, it is blood stasis, which can be cured by purgation. If the abdomen is distended and large, with water sounds when turning, or if pus discharges from the umbilicus or bowels, use Lafan Wan (Beeswax and Alum Pill), Taiyi Gao, and interior-supporting medicines. A postpartum woman had lower abdominal pain with a mass, and her pulse was hollow and choppy. She was treated with Siwu Tang added with Corydalis Rhizome (Yanhusuo), Safflower, Peach Kernel, Achyranthes Root (Niuxi), and Aucklandia Root, leading to recovery. A postpartum woman had lower abdominal pain and was cured by taking Guazi Ren Tang to discharge blood stasis. Whenever blood stasis is retained, it should be treated urgently; if delayed, it rots and turns into pus, which is difficult to treat. If it flows into the joints, it causes bone abscesses (Guju); failure to treat properly often leads to fatal conditions. A woman gave birth in the cold winter month, suffering severe abdominal pain and feeling a mass. Hundreds of remedies failed. Someone taught her to decoct four liang of mutton, two liang of Prepared Rehmannia Root (Shudihuang), and one liang of fresh ginger in water. After two or three doses, she recovered.
[Da Lun] "Child pillow" (Erzhen) occurs because the mother had stagnant blood clumps in the womb prior to conception. During delivery, if this blood breaks apart and is expelled with the child, there is no harm. However, if the产妇's Zang-Fu organs are affected by wind-cold, causing blood to coagulate and stagnate in the lower abdomen without flowing freely, it forms a lump and causes pain, named "Child Pillow."

[Lochia Not Exhausted]
[Jinkui] Seven to eight days postpartum, without Taiyang syndrome, hardness and pain in the lower abdomen indicate lochia is not exhausted. Constipation, vexation, fever, faintly solid pulse, doubled fever, evening vexation, anorexia, delirium after eating, improvement at night. Da Chengqi Tang (Major Order the Qi Decoction) is indicated. Heat is in the interior, knotting in the Bladder.
[Daqquan] Although lochia usually flows normally, external invasion of the Five Evils or internal injury of the Seven Emotions can cause sudden cessation. Residual blood stagnates, and incomplete discharge causes abdominal pain. One must investigate the cause and treat accordingly. A postpartum woman had severe lower abdominal pain and locked jaw; this was internal blood stasis. She was灌ed with Shixiao San, discharged blood, and regained consciousness. Then treated with Siwu Tang added with Fried Ginger, Atractylodes Macrocephala Rhizome, and Tangerine Peel for recovery. A woman had excessive menstrual bleeding that did not stop. She used astringents to stop it, causing abdominal pain due to Qi and Blood stagnation. Two doses of Shixiao San cured her.

*Daquan* divides lochia not exhausted abdominal pain, child pillow heart/abdominal stabbing pain, lower abdominal pain, and cold hernia into four categories. "Child pillow" refers to blood clumps existing in the womb before birth that do not exit with the child and remain in the abdomen causing pain. Pain from sluggish lochia discharge, where there was no original accumulation in the womb, is not called child pillow. If lochia is exhausted, and pain is due to other causes, such as Zhongjing's Zhishi Shaoyao San syndrome, or Blood deficiency abdominal pain like Zhongjing's Danggui Shengyang Roushou Tang syndrome, these should be discussed separately. Therefore, famous formulas are listed below. If Zhishi Shaoyao San fails, one must still seek blood stasis. Thus, Xiayuxue Tang formulas are attached, followed by tonification formulas. Cold hernia section is omitted.
*Jinkui* says: Postpartum abdominal pain, vexation, fullness, inability to lie down. Zhishi Shaoyao San is indicated.

If due to anger, use Siwu Tang added with Aucklandia Root (Muxiang) and Bupleurum Root (Chaihu). If due to Blood deficiency, use Siwu Tang, Ginseng (Renshen), Atractylodes Macrocephala Rhizome (Baizhu), and Fried Ginger (Paojiang). If due to Yang Qi weakness, use Sijunzi Tang, Angelica Sinensis Radix (Danggui), and Fried Ginger (Paojiang). If due to Spleen deficiency and weak Blood, use Liujunzi Tang, Angelica Sinensis Radix (Danggui), and Fried Ginger (Paojiang). A postpartum woman suffered the aforementioned syndrome, sometimes vomiting, sometimes unconscious. This was Spleen Qi deficiency and cold. She gradually improved with Renshen Lizhong Tang, and fully recovered with Buzhong Yiqi Tang added with Poria (Fuling) and Pinellia Tuber (Banxia). Later, pain recurred with wheezing. She used Buzhong Yiqi Tang again to tonify Spleen and Lung, leading to recovery.
