Postpartum "Chui Nai" (Blown Milk) occurs when the infant falls asleep suddenly while breastfeeding, causing obstruction to milk flow. The milk accumulates internally, leading to swelling and induration. The blockage of the lactiferous ducts prevents the flow of body fluids, resulting in putrefaction, condensation, and pain. In some cases, there is no itching or pain, but the breast becomes swollen and hard as stone, a condition named "Chui Nai". If not treated urgently, severe swelling may develop into an abscess. Postpartum Chui Nai requires immediate treatment; otherwise, the formation of an abscess can lead to death. One should promptly administer Zao Jiao San (Gleditsia Sinensis Powder) or Gua Lou San (Trichosanthes Root Powder), apply Tian Nan Xing San (Arisaema Cum Bile Powder) externally, and massage the area with hands to dissipate the accumulation. (From *Zhi Mi Fang*)
*Prescription:*
Ru Xiang (Frankincense, 1 qian, crushed), Gua Lou Gen Mo (Trichosanthes Radix powder, 1 liang).
Grind the ingredients uniformly, mix with warm wine, and take 2 qian orally.
*Tian Nan Xing San Application:*
Tian Nan Xing (Arisaema Cum Bile) ground into powder, mixed with warm water, and applied to the affected area using a goose feather.
*Verse:*
"What is the matter with postpartum blown milk?
Burn Gleditsia sinensis to ash and mix with Halitum (Ga Fen);
Mix eight characters' worth (approx. 2 qian) with a cup of hot wine,
Massage until dissipated shortly, and laugh with relief."
*To treat milk stasis, hardness, and pain (From *Jing Yan Fang*):*
Bai Yao Jian (Pyrus Sinensis Fermented Cake) ground into fine powder. Take 3 qian per dose. Boil one zhan of wine for several boils, and take it hot.
*To treat breast hardness and pain:*
Freshly picked tender mulberry leaves, crushed, mixed with rice water, spread on paper flowers, and applied to the lesion.
*(Appendix: Prescription for small superficial heat sores on the nipples of women and girls, which discharge yellow fluid upon scratching)*
"Du Ru" (Jealous Milk) is caused by the newborn's inability to suckle immediately after birth, or by incomplete emptying of the breasts due to squeezing without full evacuation. This leads to the accumulation and condensation of milk, which contends with Qi and Blood, resulting in high fever, intense thirst with desire to drink, firm and rigid pulling pain, and tenderness to touch. Upon initial detection, one should manually express the milk and have others assist in suction to draw it out. Otherwise, sores with pus may form, and if the heat pathogen is strong, an abscess will certainly develop.
Chui Nai, Du Ru, and Ru Yong (Breast Abscess) are essentially the same condition, differing only in severity. Mild cases are termed Chui Nai or Du Ru, while severe cases are termed Yong. Although specialized treatments exist, they must be recorded here.
*To treat postpartum Chui Nai developing into an abscess:*
Kui Jing (Malva verticillata stem) and Kui Zi (Malva verticillata seed).
Crush and sieve into a powder. Take one fang cun bi (square-inch spoonful) with wine for cure.
*Another prescription:*
Ten stalks of Zao Jia (Gleditsia sinensis fruit), soaked in one sheng of wine, squeezed to extract juice, mixed with half a liang of Xiao Shi (Niter), and boiled into a paste for external application.
*Another prescription:*
Ji Shi (Chicken droppings) ground into powder. Take one fang cun bi with wine. Three doses within a short period will result in recovery.
*To treat early-stage postpartum breast distension and stasis, allowing stale milk to recede naturally:*
Gua Lou (Trichosanthes fruit, one piece, half raw and half stir-fried), Da Fen Cao (Liquorice root, one inch, half raw and half honey-fried), Sheng Jiang (Fresh ginger, one piece, half raw and half roasted).
Chop together, boil in one bowl of wine until reduced to one zhan. Strain and drink. If pain becomes unbearable momentarily, manually evacuate the stale milk. Take another dose before sleep. Lie on the side of the affected breast to facilitate the distribution of the medicinal action.
*To treat breast swelling progressing to abscess:*
Apply horse urine to the area; recovery is immediate.
*(Appendix: Prescription for small superficial heat sores on the nipples of women and girls, discharging yellow fluid upon scratching)*
The *Ji Yan Fang* states: Whenever women or girls develop small superficial heat sores on the nipples that discharge yellow fluid upon scratching, spreading and growing over time, and resisting various treatments for months or years, it is called "Du Ru Disease". In nursing mothers, milk secretion tends to cease; worldly theories attribute this to the 'plundering' of milk. It is advisable to wash with Chi Long Pi Tang (Red Dragon Bark Decoction) and Tian Ma Tang (Gastrodia Decoction). Applying Er Wu Fei Wu Gao (Two-Ingredient Flying Bird Ointment) and Fei Wu San (Flying Bird Powder) is excellent. At the onset, applying Huang Qin Lu Lu San (Scutellaria and Rhaponticum Powder) and Huang Lian Hu Fen San (Coptis and Lead White Powder) is also beneficial.
