心,故昏迷不省、气闭欲绝是也。然其由有三,有用心使力过多而晕者;有下血多而晕者;
It is stated: Postpartum blood syncope (a condition characterized by dizziness, vertigo, and loss of consciousness due to blood loss or blood stasis) occurs when retained blood (stasis from lochia that fails to discharge) flows into the Liver Meridian. The patient sees black spots before the eyes, experiences spinning vertigo of the head and eyes, and is unable to rise or sit up. In severe cases, it leads to fainting and unconsciousness, which is termed "blood syncope." It is best treated with Heishen San (Black Divine Powder) mixed with fine wine. Quack doctors sometimes treat it as An Feng (Dark Wind) or Zhong Feng (Wind Stroke). Generally, syncope results from blood-heat taking advantage of deficiency to surge upward to the Heart, causing coma, unconsciousness, closed qi, and a sense of impending death. However, there are three causes: syncope caused by excessive mental or physical exertion; syncope caused by excessive bleeding; and syncope caused by insufficient bleeding. Although the symptom of syncope is the same, the treatment differs significantly, requiring careful examination. For syncope due to excessive bleeding, where the patient is merely faint, confused, and restless, it should be treated with formulas that tonify blood and clear the Heart. For syncope due to insufficient bleeding, where lochia does not discharge and stasis rushes upward to the Heart, causing fullness and urgency in the epigastrium, coma, locked jaw, and complete unconsciousness, it should be treated with formulas that break stasis and promote blood circulation. Ancient methods state: Immediately after delivery, preheat a weight (such as a scale weight), yellow stones from rivers, or hard charcoal until red-hot, place them in a vessel, and urgently pour vinegar over them near the bed. The fumes of the vinegar can eliminate blood syncope. Using this method preventively within one month postpartum is excellent. Cui Shi said: All syncope is caused by deficient heat, with blood and qi rushing together due to emptiness in the abdomen. Before delivery, one must first apply strong vinegar to the mouth and nose, keep vinegar nearby for inhalation, and drink it slowly; this is the superior method. If one feels dizzy, apply vinegar to the face; once recovered, drink vinegar, and give a small amount of water to quench thirst (another version suggests diluting with water). Another method involves burning Gan Qi (Dried Lacquer) to produce thick smoke and fumigating the parturient's face to restore consciousness (if Dried Lacquer is unavailable, burning old broken lacquerware with intense fire is also effective).
Guo Jizhong discussed: What is postpartum blood syncope? The answer is: After delivery, qi and blood become suddenly deficient, and rest is not yet achieved. Blood follows the rebellious qi upward, confusing the Shen (Spirit) of the Heart, causing visual disturbances like seeing flowers. In extreme cases, it causes deep coma and unconsciousness, locked jaw, clouded spirit, and cold breath. If physicians do not recognize this and call it An Feng (Dark Wind), treating it as such will make the disease difficult to cure. Simply administering Qinghun San (Clear Soul Powder) will restore consciousness.
