Fetal shock in pregnancy refers to a condition occurring when the gestational period is nearly full, at which time the fetus's consciousness and spirit are already formed. It may result from falling from a height during the time of impending labor, damaging the uterine collaterals, leading to bleeding and fetal movement, followed by Qi and Blood rushing upward to the chest and heart, causing Qi obstruction and unconsciousness. If the mother's face is red and her tongue is cyanotic, with no foam coming from her mouth, it indicates that the fetus is dead but the mother survives; if both the lips and mouth are cyanotic with foam exuding, it indicates the death of both mother and child; if both the face and tongue are cyanotic with foam exuding, it indicates the death of the mother but the survival of the fetus. If vaginal bleeding continues without stopping, causing the uterus to dry up and the fetal vitality to wither, the fetus will die.
Shengcai Shenmiao Foshou San (Miraculous Buddha Hand Powder for Promoting Labor) (also known as Xiong Ku Tang). It treats women who, during the fifth or seventh month of pregnancy, suffer fetal injury due to external impact, or where the fetus has died in the womb, with lochia flowing, unceasing pain, and jaw clenching approaching collapse. This medicine is used for probing/testing; if the fetus is undamaged, the pain ceases, and both mother and child are safe. If the fetus is damaged, it is expelled immediately. This formula originates from Wen Zhong, Xu Zhicai, and Wang Bing, and its efficacy is miraculous and verified. The *Tai Dong Fang* states: It treats blood and Qi rushing upward causing fullness and oppression in the heart and abdomen, with effects as rapid as snow melting in hot water. The *Jiuji Fang* and *Jingxin Lu* contain the same formula. (From *Waitai Miyao*). It also treats body heat before and after delivery, and abdominal pain caused by retained blood.
Dang Gui (Radix Angelicae Sinensis) (6 liang) Chuan Xiong (Rhizoma Chuanxiong) (4 liang). In Zhang's formula, they are used in equal proportions.
Grind the above into coarse powder. Take 3 qian per dose. Add one large cup of water, boil until the liquid becomes thick and nearly dry, then add one large cup of wine, boil only once more, remove the dregs, and take warm. If the patient has jaw clenching, administer via force-feeding. Repeat the dose after the time it takes a person to walk five li. Delivery usually occurs within two or three doses. Another version states: This medicine treats excessive blood loss due to fetal injury; excessive blood loss due metrorrhagia; excessive blood loss due to traumatic wounds; excessive blood loss due to tooth extraction, presenting with fainting and impending collapse. Boil with water and take orally. Or, if there is initial leakage of blood and abdominal pain, add Bai Shao (Radix Paeoniae Alba) and Guan Gui (Cortex Cinnamomi), reducing the dosage by half, yielding immediate results. (See General Formulas).
Treats fetal injury due to falling in pregnancy, where the fetus does not move, accompanied by abdominal pain, heavy sensation in the waist, and retention of a dead fetus in the womb. Three doses taken within a short time will lead to immediate expulsion.
Chuan Xiong (Rhizoma Chuanxiong) (1 liang)
Grind into fine powder. Mix with hot wine and take one fang cun bi (a square-inch measure) per dose. Take three to four times daily.
Treats pregnancy complicated by sudden fright causing exhaustion and collapse. The *Qianjin Fang* also uses this for Zi Fan (fetal restlessness/irritability in pregnancy).
Take Zhu Li (Bamboo Sap), drink one sheng, and recovery is immediate.
Treats pregnancy complicated by falling from a height, resulting in abdominal pain, vaginal bleeding, and mental agitation/oppression.
Sheng Di Huang (Radix Rehmanniae Preparata) Yi Mu Cao (Herba Leonuri) (each 1 liang) Dang Gui (Radix Angelicae Sinensis) Huang Qi (Radix Astragali) (each 0.5 liang)
Cut the herbs into pieces. Take 4 qian per dose. Add one cup of water and four slices of fresh ginger, boil down to 60% of the original volume, remove dregs, and take at any time.
The *Jiyan Fang* treats conditions from the second or third month up to the seventh or eighth month of pregnancy, involving sudden falls without support, fetal restlessness, injury to the waist and abdomen with severe pain bordering on death, visual hallucinations; as well as fetal Qi rushing upward to oppress the heart, shortness of breath, and unceasing vaginal bleeding.
