Coldness of the female genitals can present as either a cold syndrome or a heat syndrome. The cold syndrome arises from deficiency of Yang, constituting a true cold syndrome; the heat syndrome arises from damp-heat, presenting as a false cold syndrome. False cold is invariably accompanied by signs of heat, such as dysuria with frequency, yellow and dark urine, dry and constipated stools, irritability, and thirst. True cold is characterized by clear and profuse urination, along with aversion to cold due to Yang deficiency. For true cold, it is appropriate to tonify Yang using formulas such as Liyin Decoction (Reinforce Yin Decoction), Shibu Wan (Ten-Tonifying Pill), or Jiajian Xusi Jiangsheng Dan (Modified Fertility and Birth Promoting Pill). For false cold caused by damp-heat, it is appropriate to clear fire and heat using Longdan Xiegan Tang (Gentian Liver-Draining Decoction) or Jiawei Xiaoyao San (Modified Free Wanderer Powder). In cases of deficiency-cold of the Liver and Kidney, Nuangan Decoction (Warm the Liver Decoction), Zhenyin Decoction (Anchor Yin Decoction), or Daying Decoction (Great Camp Decoction) are suitable. In cases of deficiency-cold of the Spleen and Stomach, Lizhong Tang (Regulate the Middle Decoction), Liyin Decoction (Reinforce Yin Decoction), or Shoupi Decoction (Sustain Spleen Decoction) should be used.
Liyin Decoction (See "Blood Cold and Delayed Menses")
Shibu Wan (See "Irregular Menstruation")
Xusi Jiangsheng Dan (See *Yilin Ce*)
Longdan Xiegan Tang (See "Prolapse of the Vagina")
Jiawei Xiaoyao San (See "Irregular Menstruation")
Nuangan Decoction (See "Blood...")
Zhenyin Decoction (See *Xinfang Bazhen·Rezhen*): Treats conditions where Yin deficiency in the lower jiao causes Yang to be repelled upwards, leading to the loss of control over True Yang and subsequent hemorrhage, resulting in severe hematemesis or epistaxis. The six pulses become fine and faint, the hands and feet are cold, and the condition is critical with blood flow uncontrolled. In such emergencies, this formula should be administered rapidly to allow the solitary Yang to return, thereby stabilizing the blood. If treating throat bi (obstruction/pain) caused by repelled Yang with upper heat, the decoction should be taken cold.
Ingredients: Shudi (Prepared Rehmanniae Radix, 1–2 liang), Niuxi (Achyranthis Bidentatae Radix, 2 qian), Zhigancao (Honey-fried Glycyrrhizae Radix et Rhizoma, 1 qian), Zexie (Alismatis Rhizoma, 1.5 qian), Rougui (Cinnamomi Cortex, 1–2 qian), Zhi Fuzi (Prepared Aconiti Lateralis Radix, 0.5–0.7 qian or 1–3 qian).
Decoct in two zhong of water and take quickly. If accompanied by vomiting and nausea, add Ganjiang (Dried Zingiberis Rhizoma, fried until yellow, 1–2 qian). If there is Qi collapse, fatigue, reluctance to speak, and extremely weak pulses, add Renshen (Ginseng Radix et Rhizoma) in large quantities according to circumstances.
Daying Decoction (See "Blood Heat and Early Menses")
Lizhong Tang (See "Blood Deficiency and Chaotic Menses")
Shoupi Decoction (See "Blood Deficiency and Chaotic Menses")
Vaginal bleeding occurring immediately upon sexual intercourse is mostly due to weakness of Yin Qi and insecurity of the Kidney Yuan, or the presence of fire in the Yin aspect. If Spleen Qi deficiency leads to sinking Qi that fails to contain blood, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) or Buyin Yiqi Jian (Nourish Yin and Augment Qi Decoction) is appropriate. If Spleen and Kidney deficiency leads to insecure Yin Qi, Shoupi Decoction (Sustain Spleen Decoction) or Guipi Tang (Restore the Spleen Decoction) is appropriate. If Liver and Kidney Yin deficiency leads to failure of Yin to hold, Guyin Decoction (Secure Yin Decoction) is appropriate. If Yin fire disturbs the blood causing hemorrhage, Baoyin Decoction (Protect Yin Decoction) is appropriate.
