Pregnancy approaching labor requires composure; although abdominal pain occurs, do not panic. Extend the body and lie supine to allow fetal rotation, quietly waiting for delivery without haste.
[Note] When pregnancy reaches full term and labor is imminent, if one feels fetal movement and abdominal pain, one must remain composed and avoid self-induced panic. Extending the body and lying supine, while occasionally taking slow steps, helps correct the fetal position. Wait quietly; when the time for birth arrives, natural smooth delivery will occur without urgency.
The temperature of the delivery room should be appropriate; extreme cold or heat is always unsuitable. Summer requires coolness, winter requires warmth; both the patient and the physician must know this.
[Note] Inside the delivery room, the temperature must be moderate in all four seasons. If it is too hot or too cold, it is inappropriate. In summer, it must be cool to prevent the parturient from suffering from heatstroke and fainting; if this occurs, a small amount of cool water may be given to relieve it. In winter, it must be warm to prevent blood stasis leading to difficult labor. Many fire stoves should be prepared so that the parturient's waist, back, and lower body can be warmed by the fire. This is knowledge that families awaiting childbirth and physicians alike should possess.
A midwife for labor must be selected in advance; she should be mature, experienced, and astute. Without cause, do not instruct her to use manual techniques; instead, advise calmness and patience until the time of birth.
[Note] Families in labor must employ a midwife, who must be selected in advance as someone mature, experienced, and knowledgeable. Without valid reason, do not let her perform manual interventions beforehand, such as testing waters or examining amniotic fluid. Simply instruct her to maintain calmness and patience, waiting for the time of birth.
Noisy conversation startles the parturient; deficiency of heart qi and timidity are termed "startled birth" (Jing Sheng). It is urgent to stop noise and cease clamor; when the mother's mind is at peace, the fetus naturally becomes tranquil.
[Note] There should not be too many people in the delivery room. Too many people lead to noisy chatter, which necessarily startles the parturient. Startle causes heart qi to become deficient and timid, often leading to exhaustion at the time of labor, termed "startled birth." In such cases, irrelevant persons must be quickly removed, leaving only one or two servants, ensuring silence without noise. Then the mother's mind can settle; once settled, the fetus also becomes tranquil.
Abdominal pain before full term, with return to normal after pain subsides, is named "testing fetus" (Shi Tai); abdominal pain near term without lumbar pain, occurring intermittently, is named "playing fetus" (Nong Tai). Neither is a sign of true labor; one must remain calm and free from doubt.
[Note] During the eighth or ninth month of pregnancy, occasional abdominal pain that returns to normal after subsiding is called "testing fetus"; nourishing blood to calm the fetus is appropriate. If the months are full, abdominal pain occurs intermittently but there is no lumbar pain, it is called "playing fetus," and should not be lightly disturbed. Neither represents the time of true labor; one must not be restless or doubtful, but rather remain calm and wait for the proper time.
