Take with mugwort decoction. If there is a slight fetal movement in the lower abdomen after one or two hours, it indicates pregnancy. If there is no movement, take another dose. If there is still no movement, then it is not pregnancy.
For those who are pregnant, it is crucial that the Chong (Penetrating) and Ren (Conception) vessels are vigorous and the Yuan Qi (Original Qi) is sufficient. This ensures normal appetite, robust health, lustrous complexion, freedom from disease invasion, and充盈 blood and qi, allowing for a full ten-month gestation without danger during delivery, ensuring the safety and strength of both mother and child. How could illness arise? However, if blood and qi are insufficient and the Chong and Ren vessels are deficient, menstruation will be delayed, making conception impossible. Even if conception occurs, due to the deficiency and cold of the uterine gate and womb (uterus), the fetus cannot be securely retained.
If the Chong and Ren vessels are deficient and accompanied by heat, it may lead to restless fetus (Tai Dong Bu An) in mild cases, or miscarriage in the third, fifth, or seventh month in severe cases. Additionally, external contraction of the Six Evils (Wind, Cold, Summerheat, Dampness, Dryness, Fire), internal injury from the Seven Emotions (Joy, Anger, Worry, Thought, Sorrow, Fear, Fright), dietary damage to the Spleen and Stomach, or excessive sexual activity damaging the True Yuan (True Origin), are all causes of disease. Therefore, for pregnant women, stabilizing the fetus is paramount, assisted by nourishing blood and regulating qi. Since excess blood facilitates fetal growth, Si Wu Tang (Four Substances Decoction) is a key formula. If qi flows smoothly, the Middle Qi (Central Qi) circulates freely, appetite increases, maternal qi strengthens, and consequently fetal qi also strengthens; thus, Amomum Fruit (Sharen) and Cyperus Rhizome (Xiangfu) are needed to regulate qi. For blood deficiency, add Xiangfu and Sharen to Si Wu Tang; for qi deficiency, use Si Jun Zi Tang (Four Gentlemen Decoction) with Xiangfu and Sharen. Ancients often used Ginseng Root (Renshen) and Amomum Fruit together in treating prenatal conditions, taking advantage of one tonifying and one regulating. Tonification prevents qi deficiency and miscarriage risk; regulation ensures harmonious blood and qi flow and prevents difficult labor. This is an excellent method.
Now, let us detail the symptoms of prenatal conditions: From the third month of pregnancy onwards, mental emptiness and restlessness are called E Zu (Morning Sickness). Ren Shen Ju Pi Tang (Ginseng Peel Decoction) or Ren Shen Mu Gua Tang (Ginseng Papaya Decoction) are suitable. Some may experience body fatigue, limb laziness, dizziness, lethargy, nausea, vomiting, inability to eat even gruel, or even aversion to cold and fever; Ding Zhu Tang (Turdan and Atractylodes Decoction) is appropriate. For obese individuals with E Zu, phlegm-dampness is likely abundant; Er Chen Tang (Two-Cured Decoction) modified is suitable. For thin individuals with E Zu, fire-heat is likely excessive; Er Chen Tang added with Gardenia Fruit (Shanzhi) and Forsythia Fruit (Lianqiao) is suitable. In summary, regarding E Zu, ancients distinguished it by observing physical symptoms while the pulse remained normal. I suggest that since patients invariably crave sour foods, distinguishing by this craving is more precise. There are also cases where blood and qi are harmonious and organs are flourishing, resulting in no E Zu or very mild/transient symptoms, which must also be known.
Between the third and fourth months, sudden loss of voice is called Zi Yin (Prenatal Aphonia). This is not cured by medicine but resolves spontaneously after delivery. It is caused by fetal qi invading the lung system and throat; although aphonic, it is not considered a disease. From the fourth to fifth months, the Jun Fire (Ministerial Fire) and Xiang Fire (Ministerial Fire) nourish the fetus. Those with pre-existing internal heat may experience fetal heat rising against the heart, causing restlessness and chest/diaphragm distension, called Zi Xuan (Prenatal Suspension). Zi Xuan Tang or Zi Su Yin (Perilla Decoction) are suitable.
In the fifth and sixth months, Shaoyin Jun Fire nourishes essence; in the seventh and eighth months, Shaoyang Xiang Fire nourishes qi. Individuals with pre-existing heat may feel irritated and restless due to internal and external fire interacting; this is called Zi Fan (Prenatal Restlessness). An Shen Wan (Calming Spirit Pill) is suitable. From the fifth to sixth months, edema resembling water retention is called Zi Zhong (Prenatal Edema), colloquially known as Liuli Tai (Glazed Fetus). Fang Ji Tang (Stephania Decoction) is suitable. If facial and eye swelling resembles water, accompanied by shortness of breath and weakness, Bai Zhu San (Atractylodes Macrocephala Powder) is suitable. Some prenatal edema is solely due to Spleen deficiency; Shao Ai Tang (Peony and Mugwort Decoction) is suitable.
Between the fifth and sixth months, abnormal abdominal enlargement with chest distension is called Tai Shui (Fetal Water), indicating fluid accumulation in the uterus. If not treated early, the newborn may have soft/short limbs or physical disabilities, or die upon amniotic fluid release; Li Yu Tang (Carp Decoction) is suitable.
In the seventh and eighth months, sudden collapse, rigidity, unconsciousness, followed by quick recovery, is called Er Yun (Fetal Syncope). Ge Gen Tang (Kudzu Decoction) is suitable. For syncope due to dual qi and blood deficiency, Ba Zhen Tang (Eight Treasures Decoction) is suitable.
From the seventh to eighth months, if only the feet swell while the head and eyes do not, it is also called Zi Zhong, differing from earlier edema, and is named Zhou Jiao (Wrinkled Feet). Strengthening the Spleen and tonifying qi with Ping Wei San (Peaceful Stomach Powder) is suitable. If foot swelling includes yellow fluid oozing from toes, it is due to water-dampness swelling; Tian Xian Teng San (Akebia Vine Powder) is suitable.
In the eighth and ninth months, urinary retention is called Zhuan Bao (Turning Bladder). This is due to weak qi failing to support the fetus, causing the fetus to obstruct the bladder and preventing urine outflow. Diuretics are contraindicated; Ren Shen Sheng Ma Tang (Ginseng and Cimicifuga Decoction) is suitable.
Near term, blindness, inability to see light, headache, dizziness, and swelling of cheeks/jaw, with inability to turn the head, is caused by Liver channel heat-toxin attacking upward, often from overconsumption of roasted/spicy/alcoholic foods. This is called Tai Re (Fetal Heat). Xiao Feng San (Wind-Expelling Powder) or Tian Men Dong Yin (Asparagus Decoction) are suitable.
Miscarriages occurring at three/four months, six/seven months, or recurrent miscarriages due to insufficient qi and blood are called Hua Tai (Slippery Fetus/Habitual Abortion). Gu Tai Wan (Stabilize Fetus Pill) is suitable. These are conditions identifiable by month.
Irregular vaginal bleeding during pregnancy is called Lou Tai (Leaking Fetus). Treatment depends on the specific deficiency indicated by the pulse. For qi deficiency, Si Jun Zi Tang added with Scutellaria Root (Huangqin) and Donkey-Hide Gelatin (Ejiao) is suitable. For blood deficiency, Si Wu Tang added with Huangqin, Coptis Rhizome (Lian), Atractylodes Rhizome (Bai Zhu), and Motherwort Herb (Yimucao) is suitable. Do not confuse these.
Urinary dribbling and pain during pregnancy, caused by excessive alcohol/sex damaging the uterus or dietary heat blocking the water passages, is called Zi Lin (Prenatal Strangury). An Rong San (Nourish Prosperity Powder) is suitable. For cases with internal heat, Wu Ling San (Five Ingredient Powder with Poria) is suitable.
Malaria during pregnancy is called Zi Nue (Prenatal Malaria). For more heat than chills, Qing Pi Yin (Clear Spleen Decoction) minus Pinellia Tuber (Banxia) is suitable. For more chills than heat, Ren Shen Yang Wei Tang (Ginseng Stomach-Nourishing Decoction) minus Banxia is suitable. For weak Yuan Qi, Sheng Jin Dan (Win Gold Pill) is suitable. Differentiate accordingly.
Heart pain during pregnancy is not true heart pain but caused by ascending fetal qi, also called Zi Xuan, distinct from the Zi Xuan in months four/five. Stabilize the fetus, nourish blood, and regulate qi. For stomach pain due to exogenous heat, Er Chen Tang minus Banxia added with Cang Zhu, Bai Zhu, and Huangqin is suitable. For stomach pain due to exogenous cold, Huo Long San (Fire Dragon Powder) is suitable.
Cough during pregnancy is called Zi Sou (Prenatal Cough). Caused by fetal qi, it resolves postpartum and usually requires no medicine. However, for cough due to wind-cold, Jie Geng San (Platycodon Powder) is suitable. For cough due to excessive fire affecting the Lung, Dou Ling San (Loquat Leaf Powder) or Bai He San (Lily Bulb Powder) is suitable. These require treatment.
Stroke-like symptoms during pregnancy, including stiff neck/head, muscle spasms, slurred speech, profuse phlegm, and coma, are called Zi Xian (Prenatal Eclampsia). Ling Yang Jiao San (Antelope Horn Powder) is suitable.
Violating taboos, consuming raw/cold foods, or exposing oneself to wind/coolness can cause cold fetus, leading to chest/abdominal pain, borborygmus, limb stiffness, and severe diarrhea. This is called Tai Han (Fetal Cold). An Tai He Qi Yin (Stabilize Fetus Harmonize Qi Decoction) is suitable.
Abdominal pain in early pregnancy due to blood heat is called Tong Tai (Painful Fetus). It is hard to cure quickly; cool-blood herbs should be taken occasionally. Zhi Qin Tang (Gardenia and Scutellaria Decoction) is suitable.
These are named prenatal diseases. Regarding prenatal Shang Han (Cold Damage), the Six Meridian treatments are the same as for non-pregnant individuals, but the focus must be on clearing heat and stabilizing the fetus. Sweating or purging must follow the pulse/symptoms of the five zang organs and exterior/interior patterns, avoiding harm to the fetus. Wan Mizhai treated prenatal Shang Han by using Xiang Su San (Agastache and Perilla Decoction) for exterior sweating, Huang Long Tang (Yellow Dragon Decoction) for half-exterior/half-interior resolution, and San Huang Jie Du Tang (Three Yellow Detoxification Decoction) for interior purging. He claimed this family secret saved many lives. For constitutionally weak pregnant women contracting Shang Han, add Si Wu Tang to prescriptions to avoid errors. Banxia (Pinellia) is often mistakenly used despite being contraindicated; check ancient formulas carefully. Qiang Huo Tang (Notopterygium Decoction) and Wan’s three formulas are suitable. For severe illness, add fetal protection methods.
For dysentery in pregnancy: If early stage with abdominal pain, tenesmus, and strong constitution, attack with Xiang Lian Hua Zhi Tang (Cinnamon and Coptis Stasis-Resolving Decoction). For chronic dysentery with deficiency, tonify with Wei Feng Tang (Wind-Stomach Decoction). For heat-straining pain, resolve with Huang Qin Shao Yao Tang (Scutellaria and Peony Decoction). Other bloody/purulent cases require case-by-case adjustment. Shang Han and dysentery are the most dangerous prenatal conditions.
For diarrhea without thirst and clear urine, use San Bai San (Three Whites Powder) added with Sharen, Houpo, Cang Zhu, and Gan Cao. For diarrhea with intestinal mucus, thirst, internal heat, and dark/scanty urine, use Huang Qin Tang (Scutellaria Decoction) added with Bai Zhu, Tong Cao, and Fu Ling. Add Sharen and Huang Lian for abdominal pain. For diarrhea with thirst, vomiting, and undigested food (協熱下利 -協热 diarrhea), use Huang Lian E Jiao Wan (Coptis and Donkey-Hide Gelatin Pill). For green/white watery diarrhea with borborygmus (Dong Xie - Hole Diarrhea), use Wu Ling San, followed by Huang Lian E Jiao Wan. For yellow frothy diarrhea with deep/tight/rapid pulse, use Wu Ji Wan (Wu-Ji Pill). Other diarrheas due to Spleen/Stomach injury from wind/cold/summerheat/dampness require symptomatic treatment.
Obstipation (both constipation and urinary retention) is due to organ heat. Large Intestine heat causes constipation: Si Wu Tang added with Zhi Ke and Huang Qin. Or Large Intestine qi stagnation: Zi Su Yin added with Xing Ren and Huang Qin. Small Intestine heat causes urinary retention: Dong Kui Zi Tang (Malva Seed Decoction). Both intestines hot: treat both. Stomach deficiency: Liu Jun Zi Tang added with Zi Su and Xing Ren. Qi/Blood deficiency: Morning Ba Zhen Tang added with Tao Ren and Xing Ren; Evening Jia Wei Xiao Yao San added with Che Qian Zi. Heart/Liver empty heat: Jia Wei Xiao Yao San added with Che Qian Zi. Liver/Spleen accumulated heat: Long Dan Xie Gan Tang (Gentian Drain Liver Decoction). Anger injuring Liver: Jia Wei Gui Pi Tang or Jia Wei Xiao Yao San. All can cause obstipation.
Palpitations, irregular pulse, fright, insomnia, and confusion: Da Sheng Tang (Great Sage Decoction). Blood deficiency with spirit unrest: Yi Rong Tang (Nourish Prosperity Decoction). Empty heart instability: Ding Zhi Wan (Settle Will Pill). Excessive fire: An Shen Wan (Calming Spirit Pill).
Summerheat strike: Xiang Ru Yin (Ardisia Decoction). For severe heat/thirst: Xiang Ru Yin added with Mai Dong, Huang Qin, Hua Fen, Wu Wei Zi, and Shan Zhi.
Food stagnation with chest/flank pain and tight Right Guan pulse: Use digestion-promoting methods. For cold food stagnation with胸闷 and desire to vomit (slow pulse): Ding Xiang San (Clove Powder); add Ginger if vomiting.
Convulsions (Chi Zong) due to Liver Wind and Heart Fire: Gou Teng Tang (Uncaria Decoction).
Huo Luan (Cholera-like syndrome): Upper stomach evil causes heart pain/vomiting; Lower stomach evil causes umbilical pain/diarrhea; Middle stomach evil causes abdominal pain with both. Vomiting damages Qi; Diarrhea damages Blood. Evil striking the fetus endangers mother; Blood/Qi damage endangers fetus. This is an emergency. Treat urgently with Xiang Su San. Add Mu Gua for cramps, Bai Zhu for restless fetus, Huang Qin for summer, Renshen/Bai Zhu for winter.

Summary: Amenorrhea with normal pulses, acid regurgitation, aversion to food, slight chills/fever, and lethargy indicates pregnancy. Pathological pulses indicate otherwise. For confirmed pregnancy with nausea/vomiting, nourish blood, stabilize fetus, regulate qi, strengthen Spleen. Shou Shen He Zhong Tang is suitable. For restless fetus with back/abdominal pain and poor appetite, An Tai Yin is suitable. Instability is primarily due to qi/blood deficiency failing to nourish the fetus. Use Renshen for qi, Dang Gui for blood. For internal heat boiling blood/qi, use Huang Qin to cool. Since the fetus attaches to the Spleen, use Bai Zhu and Zhi Gan Cao to secure the root. Chen Pi, Xiangfu, Su Geng regulate qi; Sharen opens stomach; Du Zhong treats back pain; Bai Shao soothes abdominal pain. Remove Sharen and add Mai Dong for thirst/heat; add Sheng Di and Di Yu for bleeding. These are general prenatal methods.
Ghost Fetus: Due to defensive/wei deficiency and emotional interference, spirits enter organs, mimicking pregnancy. Abdomen feels like holding a jar, with chills/fever, sallow face, anorexia, fatigue. Liver/Spleen are damaged. Tonify Yuan Qi and assist with dispersing herbs like Xiong Huang Wan or Yuan Hua San.
Easy Delivery Methods: Do not induce labor; harmonize blood/qi. From 8-9 months, take Bao Chan Da Sheng San. At 9 months, take Bian Chan Fang one dose, and another dose near delivery. Avoids difficult labor. The Qin family’s monthly prenatal nourishing formula is highly effective.
