During pregnancy, if there is excessive consumption of sweet and fatty foods, leading to food stagnation, putrefaction, and accumulation into phlegm; or due to the constraint of the Seven Emotions (Joy, Anger, Worry, Thought, Sorrow, Fear, Fright), causing Qi abundance transforming into Fire, which stagnates in the Stomach. If suddenly affected by Cold or Heat pathogens, with Evil and Righteous Qi contending and Yin and Yang mixing chaotically, it causes severe colic in the heart and abdomen, with simultaneous vomiting and diarrhea, rapid and chaotic changes, known as Huoluan (Cholera-like disease).
If the pathogen is in the Upper Jiao, there will be heart pain and profuse vomiting; if in the Lower Jiao, there will be periumbilical pain and profuse diarrhea; if in the Middle Jiao (Stomach region), there will be abdominal pain with both profuse vomiting and diarrhea.
Profuse vomiting damages Qi, while profuse diarrhea damages Blood. When Qi and Blood are injured, they cannot protect and nourish the fetus. The pathogenic factor agitates and strikes the fetal origin, resulting inevitably in miscarriage or death for both mother and child. This is a critical condition that must be treated urgently. It is suitable to use Xiangsu San (Fragrant Perilla Powder) added with Huoxiang Ye (Agastache Leaf), or to administer the medicine first followed by induced vomiting. Additionally, Jiawei Siwei Zisu Hetai Yin (Modified Four-Ingredient Perilla Fetus-Harmonizing Decoction) is effective for treating heart and abdominal colic with upper vomiting and lower diarrhea.
Xiangsu San: This formula treats Huoluan with extreme balance and propriety.
Ingredients: Xiangfu (Cyperi Rhizoma, stir-fried), Zisu (Perillae Folium) (each 2 qian), Chenpi (Citri Reticulatae Pericarpium) (1 qian), Huoxiang Ye (Agastaches Herba), Suosha (Amomi Fructus Rotundus), Zhigancao (Glycyrrhizae Radix et Rhizoma, honey-fried) (each 5 fen).
Usage: Decoct in water and take orally. If muscle spasms (Zhuanjin) occur, add Mugua (Chaenomelis Fructus) 1 qian; if fetal movement is restless, add Baizhu (Atractylodis Macrocephalae Rhizoma, earth-fried) 1.5 qian; if contracted in summer, add Huangqin (Scutellariae Radix) 1.5 qian, Chao Huanglian (Coptidis Rhizoma, stir-fried) 1 qian, Xiangru (Moslae Herba) 2 qian; if contracted in winter, add Renshen (Ginseng Radix et Rhizoma) and Chao Baizhu (Atractylodis Macrocephalae Rhizoma, stir-fried) each 1 qian, Paojiang (Zingiberis Rhizoma Preparatum) 5 fen.
Jiawei Siwei Zisu Hetai Yin: Treats heart and abdominal colic with upper vomiting and lower diarrhea.
Ingredients: Zisu (Perillae Folium), Huangqin (Scutellariae Radix), Baizhu (Atractylodis Macrocephalae Rhizoma, earth-fried each 1.5 qian), Zhigancao (Glycyrrhizae Radix et Rhizoma, honey-fried) (the above four constitute the base Hetai Yin); Huoxiang Ye (Agastaches Herba), Chenpi (Citri Reticulatae Pericarpium) (each 1 qian), Sharen (Amomi Fructus, stir-fried) (5 fen).
Sudden stabbing pain in the heart and abdomen during pregnancy, with suffocation and near-death sensation, is called Zhong'e (Sudden Evil Invasion), also commonly known as Changsha (Intestinal Sha). This is due to disharmony of Qi and Blood, weakness of Spirit, allowing toxic pathogenic factors to invade. If pregnant women suffer from this, it also damages the fetus. Dr. Xue states: For this condition, it is best to tonify and regulate the Righteous Qi. Use Yinjinteng (Lonicerae Japonicae Caulis) alone, decocted in water and drunk. In *Jiyin Gangmu* (Outline and Compendium of Women's Diseases), Sanzhi Tang (Disperse Stagnation Decoction) is used for treatment, and the Bo'ai Fang (Moxa Boiling Formula) is extremely effective for treating direct Zhong'e.
During pregnancy in the height of summer, contracting the toxicity of Summerheat and Heat, symptoms include fever with thirst, spontaneous sweating, mental confusion and lethargy, fatigue of the four limbs, and shortness of breath. The main formula is Qingsu Hetai Yin (Clear Summerheat and Harmonize Fetus Decoction).
Contracting Dampness during pregnancy may result from walking early in the morning and being exposed to mist and dew, wading through water, living long-term in damp lowlands, or bathing in cold water after profuse sweating. Symptoms include fever, painful and vexatious bone joints, heavy body, headache, and nasal congestion. The main formula is Huangqin Baizhu Tang (Scutellaria and Atractylodes Decoction).
Wind-stroke during pregnancy occurs because of constitutional deficiency allowing Wind invasion. It is the Wind Qi from the eight directions of the four seasons coming from the Chong direction; once it enters the person, illness ensues. If Wind invades the skin, hair, and meridians, it causes chills and fever, pain in the head, neck, and body, or muscular numbness and paralysis. If it invades sinews and bones, it causes spasms and rigidity. If it invades the Zang-Fu organs, it causes sudden collapse and coma, deviation of the mouth and eyes, convulsions of the hands and feet, trismus, and inability to speak. Pregnant women suffering from this should not use conventional wind-treatment methods; instead, the root should be tonifying deficiency and calming the fetus, supplemented with formulas that search out Wind. Use Soufeng Antai Yin (Search-out Wind and Calm Fetus Decoction) for treatment. However, true wind-stroke before delivery is rare; Zi Xian (Subtle Epilepsy/Eclampsia) symptoms are actually common and indistinguishable from wind-stroke manifestations. Physicians must carefully examine and confirm it is wind-stroke before using Soufeng Antai Yin; otherwise, treat it as Zi Xian.
Sanzhi Tang: Specifically treats touching evil Qi damaging Stomach Qi, injuring the fetus causing abdominal pain that cannot bear pressure, and loss of appetite.
Ingredients: Qingpi (Citri Reticulatae Pericarpium Viride) (3 qian), Huangqin (Scutellariae Radix), Shaoyao (Paeoniae Radix Alba) (each 2 qian), Guiwei (Angelicae Sinensis Radix tail) (1.5 qian), Chuanxiong (Ligustici Chuanxiong Rhizoma) (1 qian), Muxiang (Aucklandiae Radix) (5 fen), Zhigancao (Glycyrrhizae Radix et Rhizoma, honey-fried) (a small amount).
Usage: Divide the above into two doses. Use three zhan of water, first decocting Zhugen (Bletillae Rhizoma) two large slices until reduced to two zhan, remove Zhugen, add the previous herbs, decoct until one zhan remains, and take hot.
Qingsu Hetai Yin:
