Postpartum regulation is not only about expelling lochia; (if needed) Bletillae Rhizoma (Baizhi, 白芷), Crataegi Fructus (Shanzha, 山楂), Citri Reticulatae Pericarpium (Chenpi, 陈皮), Platycodonis Radix (Jiegeng, 桔梗), Achyranthis Bidentatae Radix (Niuxi, 牛膝), Zingiberis Rhizoma Recens (Ganjiang, 干姜), Cinnamomi Cortex (Rougui, 肉桂), Corni Fructus (Shanzhuyu, 山茱萸), Evodiae Fructus (Wuzhuyu, 吴茱萸), Linder Acanthocarpi Fructus (Wuyao, 乌药), Cyperi Rhizoma (Xiangfu, 香附), Trogopteri Dungus (Lingzhi, 五灵脂), and Corydalis Rhizoma (Yanhusuo, 玄胡索) can be included in the prescription.
If chills and fever occur after three days postpartum, Bupleuri Radix (Chaihu, 柴胡) and Angelicae Sinensis Radix Caulis (Dangguwei, 当归尾) must not be omitted.
If chills, fever, and lower abdominal pain occur seven days postpartum, Pruni Semen (Taoren, 桃仁), Curcumae Pharyngomatis Rhizoma (Penglang, 蓬莪术), and Curcumae Longae Rhizoma (Sanleng, 三棱) are the precise three herbs to use.
If deficiency-cold manifests fourteen days postpartum, Angelicae Sinensis Radix (Danggui, 当归), Chuanxiong Rhizome (Chuanxiong, 川芎), Bupleuri Radix (Chaihu, 柴胡), Poria (Fuling, 茯苓), and Rehmanniae Radix (Dihuang, 地黄) are paramount.
If abdominal pain occurs twenty-one days postpartum, one must differentiate whether it is due to Qi rebellion or sinking; Amomum Fructus (Caoguo, 草果), Arecae Semen (Binglang, 槟榔), Amomi Fructus Rotundus (Sharen, 砂仁), and Myristicae Semen (Kouren, 蔻仁), these four herbs added yield miraculous effects.
If there is definite pain in the heart and abdomen, it is originally because lochia has not been fully expelled; quickly add Curcumae Longae Rhizoma (Sanleng, 三棱), Atractylodis Macrocephalae Rhizoma (Baizhu, 白术), Angelicae Sinensis Radix Caulis (Dangguwei, 当归尾), and Pruni Semen (Taoren, 桃仁); once lochia is expelled, nothing remains.
There are also syndromes where Qi stagnation and food retention interfere with each other, causing unbearable pain due to abnormal ascending and descending of Qi; Amomum Fructus (Caoguo, 草果), Arecae Semen (Binglang, 槟榔), Immature Bitter Orange (Zhishi, 枳实), and Crataegi Fructus (Shanzha, 山楂) together, and even Curcumae Longae Rhizoma (Sanleng, 三棱) and Atractylodis Macrocephalae Rhizoma (Baizhu, 白术) can be advanced.
Between three and seven days after initial childbirth, if the lower abdomen feels as if pregnant, this is caused by wind invasion during delivery; abdominal wound wind invasion has been passed down since ancient times; medicinals include Schizonepetae Spica (Jingjie, 荆芥), Saposhnikoviae Radix (Fangfeng, 防风), Sappan Wood (Sumu, 苏木), Atractylodis Macrocephalae Rhizoma (Baizhu, 白术), Puerariae Radix (Gegen, 葛根), with additional Angelicae Dahuricae Radix (Baizhi, 白芷), Curcumae Longae Rhizoma (Sanleng, 三棱), and Dolichos Lablab Seed (Duhuo, 独活).
If fresh blood flows on the twenty-eighth day postpartum while lochia is incomplete, persistent dripping postpartum is a great harm; Siwu Decoction (Four Substances Decoction) plus Trichosanthis Radix (Huafen, 花粉), Mantidis Oötheca (Suoluo, 桑螵蛸), Scutellariae Radix (Huangqin, 黄芩), Sanguisorbae Radix (Diyu, 地榆), and Ostrea Concha (Muli, 牡蛎) as guides; strictly avoid Allii Cepae Bulbus (Congbai, 葱白), Zingiberis Rhizoma Recens (Shengjiang, 生姜), Linder Acanthocarpi Fructus (Wuyao, 乌药), Cinnamomi Cortex (Rougui, 肉桂), and Corni Fructus (Shanzhuyu, 山茱萸), because with fresh blood, there is fear of severe hemorrhage.
If deficiency-cold develops after one month, do not use all five flavors of the previous formula; Ginseng Radix et Rhizoma (Renshen, 人参), Atractylodis Macrocephalae Rhizoma (Baizhu, 白术), Chuanxiong Rhizome (Chuanxiong, 川芎), Angelicae Sinensis Radix (Danggui, 当归), Poria (Fuling, 茯苓), and Paeoniae Radix Alba (Shaoyao, 芍药) tonify Qi and Blood to relieve anxiety.
For cough, use Asparagi Radix (Tian Dong, 天冬), Schisandrae Fructus (Wuweizi, 五味子), and Armeniacae Semen Amarum (Xingren, 杏仁); for cough due to wind, evaluate slowly; add Puerariae Radix (Gegen, 干葛), Peucedani Radix (Qianhu, 前胡), and Asteris Radix (Zisu, 紫菀); add Pulmonariae Herba (Sangbaipi, 桑白皮) for miraculous stopping of cough; for cough without wind, use Paeoniae Radix Alba (Shaoyao, 芍药), Atractylodis Macrocephalae Rhizoma (Baizhu, 白术), Chuanxiong Rhizome (Chuanxiong, 川芎), Angelicae Sinensis Radix (Danggui, 当归), and Poria (Fuling, 茯苓).
For nausea, adding Pinelliae Rhizoma Preparatum (Banxia, 半夏) is good; for fever, Bupleuri Radix (Chaihu, 柴胡) has been discussed since ancient times; for red and white discharge, only four herbs are used: Sanguisorbae Radix (Diyu, 地榆), Artemisiae Argyi Folium (Aiyi, 艾叶), Bletillae Rhizoma (Baizhi, 白芷), and Ostrea Concha (Muli, 牡蛎).
If neglect of regulation leads to deficiency and damage, add Atractylodis Macrocephalae Rhizoma (Baizhu, 白术), Cinnamomi Cortex (Rougui, 肉桂), and Ginseng Radix et Rhizoma (Renshen, 人参); Angelicae Sinensis Radix (Danggui, 当归), Rehmanniae Radix Preparata (Shengdi, 生地), Chuanxiong Rhizome (Chuanxiong, 川芎), and Paeoniae Radix Alba (Shaoyao, 芍药), along with Cyperi Rhizoma (Xiangfu, 香附), Astragali Radix (Huangqi, 黄芪), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草), and Poria (Fuling, 茯苓).
If after three days postpartum there is deficiency due to blood stasis, leading to madness and inversion resembling stroke, use Cinnabaris (Zhusha, 朱砂), Curcumae Longae Rhizoma (Sanleng, 三棱), Atractylodis Macrocephalae Rhizoma (Baizhu, 白术), Bupleuri Radix (Chaihu, 柴胡), and Angelicae Sinensis Radix Caulis (Dangguwei, 当归尾); Linder Acanthocarpi Fructus (Wuyao, 乌药), Zingiberis Rhizoma Recens (Ganjiang, 干姜), Crataegi Fructus (Shanzha, 山楂), Citri Reticulatae Pericarpium (Chenpi, 陈皮), Corni Fructus (Shanzhuyu, 山茱萸), and Platycodonis Radix (Jiegeng, 桔梗).
If after three days postpartum there is prolapse due to blood stasis, with swollen feet difficult to move, remove Cinnabaris (Zhusha, 朱砂) and Linder Acanthocarpi Fructus (Wuyao, 乌药) from the previous formula, and add Pruni Semen (Taoren, 桃仁), Bletillae Rhizoma (Baizhi, 白芷), Achyranthis Bidentatae Radix (Niuxi, 牛膝), and Tetrapanacis Medulla (Tongcao, 通草).
For masses, use Curcumae Longae Rhizoma (Sanleng, 三棱), Atractylodis Macrocephalae Rhizoma (Baizhu, 白术), Crataegi Fructus (Shanzha, 山楂), and Angelicae Sinensis Radix Caulis (Dangguwei, 当归尾); Corydalis Rhizoma (Yanhusuo, 玄胡索), Aconiti Lateralis Radix Preparata (Wutou, 乌头), Aconiti Radix Preparata (Fuzi, 附子), Costi Rhizoma (Muxiang, 木香), Arecae Semen (Binglang, 槟榔), and Citri Reticulatae Pericarpium (Chenpi, 陈皮), Trogopteri Dungus (Lingzhi, 五灵脂), Chuanxiong Rhizome (Chuanxiong, 川芎), Platycodonis Radix (Jiegeng, 桔梗), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草), and Amomum Fructus (Caoguo, 草果); carefully evaluate differentiation of Qi and Phlegm; for Qi stagnation add Green Tangerina Peel (Qingpi, 青皮), Evodiae Fructus (Wuzhuyu, 吴茱萸), and Myristicae Semen (Kouren, 蔻仁); Cinnamomi Ramulus (Guirui, 官桂) and Zingiberis Rhizoma Recens (Ganjiang, 干姜) are also flavorful; for Phlegm syndrome use Arisaematis Cum Bile (Xingban, 胆南星) and Pinelliae Rhizoma Preparatum (Banxia, 半夏), removing Trogopteri Dungus (Lingzhi, 五灵脂), Cinnamomi Ramulus (Guirui, 官桂), Zingiberis Rhizoma Recens (Ganjiang, 干姜), and Evodiae Fructus (Wuzhuyu, 吴茱萸).
For joint pain, add Notopterygii Rhizoma et Radix (Qianghuo, 羌活); for headache, Chuanxiong Rhizome (Chuanxiong, 川芎) and Schizonepetae Spica (Jingjie, 荆芥) are indicated.
If high fever occurs within twenty days, dried ginger (Ganjiang, 干姜) must be used and charred.
