Question: There are Yang and Yin types of Jing (startle/fright), how to distinguish them?
Answer: When a child's Liver Qi is abundant and flourishing, and it clashes with Wind Qi, resulting in upward staring eyes and opisthotonos (arched back), with a floating and wiry pulse, this is named Yang Xian (Yang Epilepsy). When a child's Yuan Zhen (Original True Qi) is weak, and Liver Qi alone is hyperactive, resulting in waking easily from sleep with half-open eyes (Lu Jing), and a fine and wiry pulse, this is named Yin Xian (Yin Epilepsy). When a child experiences startle jumps due to Wind-Heat or food retention, or due to heat from sores/ulcers or moving parasites, these are all cases of Fa Jing (onset of startle) because the Yuan Zhen cannot bear the burden.
When Yang Xian occurs, the body is very hot and stiff, with upward staring eyes, mouth puckering, and convulsions of the four limbs; Huan Hun Tang (Soul Returning Decoction) is the primary treatment.
Huan Hun Tang Formula: Ephedrae Herba (Mahuang, 5 fen), Armeniacae Semen Amarum (Xingren, 5 kernels), Paeoniae Radix Alba (Shaoyao, 1 qian), Uncariae Ramulus cum Uncis (Gouteng/Diao Teng, 3 fen), Belamcandae Rhizoma (Shegan, 3 fen), Glycyrrhizae Radix et Rhizoma (Gancao, 2 fen).
The above five herbs are boiled in one sheng of water until seven ge remain. Strain and take warm in divided doses, administered frequently.
If a child cries softly, it indicates weak Qi; if the sound is heavy and turbid, it indicates pain; if shouting loudly, it indicates Heat欲狂 (Heat approaching madness); if the voice is urgent, it indicates Shen (Spirit) fright; if the voice is obstructed, it indicates Phlegm; if the voice trembles, it indicates Cold.
If a child has fever, red cheeks, drowsiness with startle jumps, teeth grinding while nursing, Tiao Teng Tang (Uncaria Decoction) is indicated.
If a child has body heat, colorless face, arching back while crying, this is a sign of impending Jing (Fright).
If Yang Xian occurs with opisthotonos, fixed staring, and rapid pulse, and if severe, with cold extremities approaching death, one must stop Wind and remove Phlegm. If Heat does not subside, other treatments are insufficient; one must urgently attack the pathogen using Da Cheng Qi Tang (Major Order the Qi Decoction).
If there is aversion to cold and fever, yawning, red face, no sweat, and a large pulse, Jing Ti (startle/fright) will likely occur; Da Qing Long Tang (Major Green Dragon Decoction) is indicated.
If a child has Jing (Fright), with excessive phlegm-saliva, short breath, irritability, hard abdomen, and wiry pulse, Zi Shuang Wan (Purple Frost Pill) is indicated to drain the excess Qi.
Ma Pi Feng (Horse Spleen Wind), which is Fei Zhang (Lung Distension), characterized by shoulder shaking, rolling abdomen, open-mouthed panting, should be treated with Tie Jiang (Iron Slurry) mixed with Ginger Juice.
If the body is hot and lethargic, with sounds like a water frog in the throat, rapid pulse, and red face, indicating impending Fright, Ma Huang Xing Ren Gan Cao Shi Gao Jia She Ye Tang (Ephedra, Apricot Seed, Licorice, and Gypsum Decoction plus Belamcandae Rhizoma) is indicated.
Jing disease, if accompanied by upward staring and convulsions, originates from Yang; if accompanied by downward staring and slight twitching, it originates from Yin.
If a child is lethargic for five to six days, vomiting milk, jumping with startle, and fever does not resolve, suddenly developing Fright: if the illness reverses to improvement, it is good; if it does not improve and the eyes stare fixedly, it is difficult to treat.
Childhood startle jumps, resembling Cold or Heat, vomiting or diarrhea, is named Yin Yang He Bing (Combined Disease of Yin and Yang); Shao Yao Gan Cao Tang (Peony and Licorice Decoction) is indicated for regulation.
Jing disease with absent-mindedness, unilateral movement of hands and feet, vomiting or diarrhea, and a faint and choppy pulse indicates deficiency of both Yin and Yang; sweating or purging methods should not be used again; Tong Mai Si Ni Jia Zhu Dan Dan Tang (Pulse Restoring Four Reversals Decoction with Pig Bile) is indicated.
If a child has slight fever and drowsiness, inevitably showing Lu Jing (exposed eyes), and occasional limb twitching, this is a sign of Man Jing (Slow Fright).
If a child has slight body heat, slow twitching, loose stool, and enuresis, this is Pure Yin syndrome; Si Ni Jia Ren Shen Tang (Four Reversals Decoction plus Ginseng) is indicated.
If a child's whole body is slightly cold but only the fingertips are hot, this is Re Jue (Heat Reversal), which will inevitably develop high fever; if Fright and convulsions occur, true Heat is Yang, false Heat is Yin. Yang develops Ji Jing (Acute Fright), Yin develops Man Jing (Slow Fright). Ji Jing should be treated with Cold/Purging formulas; Man Jing with Warming/Nourishing formulas.
In Jing disease, if the body is hot with strong force, it is Yang Xian; if the body is cold with weak force, it is Yin Xian.
If the child beats the ground making sounds and flows saliva, it is Dian Xian (Epilepsy).
In Jing disease, looking upward is called Tian Diao (Heavenly Hook); looking inward is called Nei Diao (Internal Hook).
Opisthotonos with rapid and hurried pulse is Dou Bing (Smallpox Disease).
Jing disease requires support and careful nursing. Forcing restraint will excite the pathogenic Heat further, driving it into the bones. Even if the illness resolves, hand and foot contracture will result.
Fever, upward staring, surging phlegm-saliva, locked jaw, and convulsions indicate coagulation of Ying and Wei (Nutritive and Defensive Qi) and blockage of all vessels, hence the violence of the disease.
Jing has Yin and Yang. Tian Diao is Heat-Yang; Nei Diao resembles Yin Xian.
If a child has Fright, and the pulse is floating, rapid, full, tight, or wiry, it is Ji Jing (Acute Fright); if the pulse is deep, slow, scattered, relaxed, or faint, it is Man Jing (Slow Fright). Ji Jing should be treated slowly with Cold formulas; Man Jing should be treated urgently with Warm formulas.
Newborns who are startled and uneasy while nursing have Tai Jing (Fetal Fright); Zhu Mi (Cinnabar Honey) is indicated.
Zhu Mi Formula: Cinnabar (size of a bean), Honey (size of an egg).
Grind and mix the above two ingredients. Contain in the mouth daily, two or three times.
For infants born in the twelfth lunar month, with fixed staring, teeth grinding, limb contracture, and rapid/wiry pulse, this is Tai Jing; Zi Xue (Purple Snow) is indicated.
Zi Xue Formula: Gold (10 liang), Han Shui Shi (Cold Water Stone, 4 liang 8 qian), Shi Gao (Gypsum, 4 liang 8 qian), Xuan Shen (Scrophulariae Radix, 1 liang), Xi Jiao (Rhinoceros Horn, 1 liang), Ling Yang Jiao (Antelope Horn, 1 liang), Gan Cao (Licorice, 8 qian), Sheng Ma (Cimicifugae Rhizoma, 6 qian), Chen Xiang (Aquilariae Lignum, 5 qian), Mu Xiang (Aucklandiae Radix, 5 qian), Ding Xiang (Caryophylli Flos, 5 qian).
Boil the eleven herbs in five dou of water. First boil Gold and the two stones until three dou remain. Add remaining herbs and boil until one dou remains. Strain, add Mang Xiao (Mirabilite, 3 liang 2 qian), boil on low fire, stirring constantly with a willow branch. When nearly solidifying, pour into a porcelain basin. Add Zhu Sha (Cinnabar) and She Xiang (Musk), each 3 qian, stir quickly until naturally solidified into Zi Xue.
For Fright occurring in the twelfth lunar month, fake cases are treatable, true cases are not. Why? Fake cases are external Wind; true cases are internal Xian.
Ke Wu (Guest Intolerance) refers to external Qi offending the host Qi.
With fever, nursing, and sleeping accompanied by slight jumping, Xing Xing San (Awakening Powder) is indicated.
For Ke Wu with fever and slight convulsions, if severe, resembling mania or epilepsy, with backward staring, mouth puckering, and uneasiness, Hong Xue (Red Snow) is indicated.
Children who are easily startled by human speech or object noises have weak and clumsy Yuan Zhen, inevitably developing Jing Xian; Zhu Ru Wen Dan Tang (Bamboo Rhamus Warming Gallbladder Decoction) is indicated.
Zhu Ru Wen Dan Tang Formula: Fu Ling (Poria, 1 qian), Chen Pi (Citrus Reticulatae Pericarpium, 5 fen), Ban Xia (Pinelliae Rhizoma, 7 fen), Huang Lian (Coptidis Rhizoma, 3 fen), Zhi Shi (Aurantii Fructus Immaturus, 5 fen), Zhu Ru (Bambusa Textilis Caulis Internodium, 3 fen), Gan Cao (Licorice, 2 fen), Sheng Jiang (Zingiberis Rhizoma Recens, 3 fen), Da Zao (Jujubae Fructus, 5 pieces).
Chop the nine herbs, boil in two sheng of water until one sheng remains. Strain and take warm in divided doses. Adding Chen Sha (Cinnabar, 3 fen) is better.
Children who cry angrily, with leg and abdominal contracture, and deep/wiry pulse, grinding teeth while nursing when severe, have Chong Jing (Parasite Fright); Shao Yao Gan Cao Tang is indicated. Chong Jing resembles Xian, with loud wailing, red face, chest/hypochondriac rigidity, and intermittent pulses; Liu Gan Jia Jiao Yi Tang (Willow Liver Decoction plus Malt Sugar) is indicated.
Liu Gan Jia Jiao Yi Tang Formula: Chai Hu (Bupleuri Radix, 5 fen), Chuan Xiong (Ligustici Chuanxiong Rhizoma, 3 fen), Dang Gui (Angelicae Sinensis Radix, 5 fen), Bai Zhu (Atractylodis Macrocephalae Rhizoma, 3 fen), Fu Ling (Poria, 5 fen), Diao Teng (Uncariae Ramulus, 5 fen), Gan Cao (Licorice, 3 fen), Jiao Yi (Malt Sugar, size of an egg).
Boil the first seven herbs in two sheng of water until one sheng remains. Strain, add Jiao Yi, melt over low fire, and take warm. Three doses daily.
If the body is warm, drowsy, with chest/abdominal fullness, slight startle, food odor, or vomiting/reversal, Nong Po San Wu Tang (Magnolia Bark Three Substance Decoction) is indicated.
If a child has robust heat, red face, cold buttocks, chills and trembling, suddenly developing Fright, this is a sign of Dou Zhen (Smallpox/Rash); Xu Ming Tang (Continuing Life Decoction) is indicated.
Xu Ming Tang Formula: Ge Gen (Puerariae Lobatae Radix, 5 fen), Mahuang (Ephedrae Herba, 3 fen), Guizhi (Cinnamomi Ramulus, 2 fen), Shaoyao (Paeoniae Radix Alba, 5 fen), Diao Teng (Uncariae Ramulus, 3 fen), Huang Lian (Coptidis Rhizoma, 2 fen), Shi Gao (Gypsum, 5 fen), Gan Cao (Licorice, 3 fen), Sheng Jiang (Zingiberis Rhizoma Recens, 2 fen), Da Zao (Jujubae Fructus, 3 pieces).
Boil the ten herbs in two sheng of water. First boil Mahuang and Ge Ge, reducing by five he. Remove foam, add other herbs, boil until seven ge remain. Strain and take warm in divided doses.
If a child has skin sores, fever, and Fright, it is named Po Shang Feng (Wound Injury Wind), which is Chi Bing (Spasm Disease). Xu Ming Tang is indicated. If extremities are cold, Shao Yao Gan Cao Fu Zi Tang (Peony, Licorice, and Aconite Decoction) is indicated.
If a patient has fever, no sweat, scanty urine, Qi rushing to the chest, locked jaw unable to speak, intending to develop Gang Chi (Stiff Spasm), Ge Gen Tang (Kudzu Decoction) is indicated.
If a patient has fever, cold feet, neck stiffness, aversion to cold, sometimes head heat, red face and eyes, only head shaking, sudden locked jaw, and opisthotonos, this is Chi Bing (Spasm Disease). Sweating will cause Cold-Damp to combine, worsening surface deficiency and aversion to cold. After sweating, the pulse becomes like a snake. In Tai Yang disease, excessive sweating in blood-deficient or deficient persons causes Chi Bing; Huang Qi Jian Zhong Tang (Astragalus Middle-Warming Decoction) is indicated.
Chi Bing with chest fullness, locked jaw, inability to lie flat, leg cramps, and teeth grinding can be treated with Da Cheng Qi Tang.
Tai Yang Zhong Re (Sunstroke) is He Bing (Heat Stroke). With sweat, aversion to cold, body heat, and thirst, severe cases with upward staring and opisthotonos are treated with Bai Hu Jia Ren Shen Tang (White Tiger Plus Ginseng Decoction).
He Bing with fever, red face, choking, and string-taut, thin, hollow, or slow pulse, chills after urination, cold extremities, dry throat/mouth: sweating worsens aversion to cold; acupuncture worsens heat; Sheng Mai San (Generate Pulse Powder) is indicated.
Sheng Mai San Formula: Ren Shen (Ginseng Radix, 3 fen), Mai Men Dong (Ophiopogonis Tuber, 5 fen), Wu Wei Zi (Schisandrae Fructus, 3 fen, fried).
Boil in one sheng of water until seven ge remain. Strain, add Ginger Juice or Zhu Li (Bamboo Sap), mix, and take frequently.
Chi Bing with irritability, thirst, red face, and rapid pulse, intending to develop Fright, is treated with Huang Lian Bai Hu Tang (Coptis White Tiger Decoction).
Sudden stroke with fixed staring, incontinence, and cold extremities is treated with Fu Zi Li Zhong Tang (Aconite Regulate Middle Decoction).
Fu Zi Li Zhong Tang Formula: Ren Shen (Ginseng Radix, 3 fen), Bai Zhu (Atractylodis Macrocephalae Rhizoma, 5 fen), Fu Zi (Aconiti Lateralis Radix Praeparata, 5 fen), Gan Jiang (Zingiberis Rhizoma, 5 fen), Gan Cao (Licorice, 3 fen).
Chop the five herbs, boil in one sheng of water until seven ge remain. Strain and take warm in divided doses.
For various sudden diseases with reversed eyes and convulsions, or with Phlegm-wheezing and short breath, regardless of Yin/Yang, first give Ren Shen Huang Lian Tang (Ginseng and Coptis Decoction); Ren Shen Xiong Dan Tang (Ginseng and Bear Bile Decoction) is also effective. Add Ginger Juice for severe Phlegm-wheezing.
Jing disease with upward staring is Yang Excess; downward staring is Yin Deficiency. Excess is attacked; Deficiency is warmed.
If a child's Yuan Zhen is not solid, easily startled by objects, this is Ke Wu.
Zhong E (Mid-Evil) refers to evil Qi touching and harming the body, which is severe Ke Wu. Bai Hu (Tiger) is mild Ke Wu.
Pediatric Fright-Heat cannot be moxa-ed, as it increases Qi stagnation; gentle pushing massage is required. Severe cases require acupuncture; Zhen Gan Tang (Settle Liver Decoction) is indicated.
Zhen Gan Tang Formula: Sheng Shi (Vermicompost/Soil Earthworm, 5 fen), Mahuang (Ephedrae Herba, 3 fen), Han Shui Shi (Cold Water Stone, 5 fen), Zhi Shi (Aurantii Fructus Immaturus, 3 fen), Shaoyao (Paeoniae Radix Alba, 5 fen), Gan Cao (Licorice, 3 fen), Sheng Jiang (Zingiberis Rhizoma Recens, 3 fen).
Boil the first six herbs in one sheng of water until seven ge remain. Strain, add Sheng Shi, grind and mix, take warm in divided doses.
With fever, startle jumps, short breath, irritability, chaotic Shen-Hun, thirst, or vomiting, Jin Xue (Golden Snow) is indicated.
Jin Xue Formula: Jin Gao (Gold Paste), Shi Gao (Gypsum), Mang Xiao (Mirabilite, each 1 qian), Long Nao (Borneolum Syntheticum, 2 fen), She Xiang (Musk, 1 fen).
Grind and mix. Take one zi per dose.
Jing Xian with rolling eyes is Tian Diao. All Jing treatments must soothe Qi and expel toxins; Tiao Teng Tang is indicated.
Tiao Teng Tang Formula: Diao Teng (Uncariae Ramulus, 5 fen), Ye Can (Wild Silkworm, 1), Shaoyao (Paeoniae Radix Alba, 5 fen), Chen Sha (Cinnabar, 3 fen), Xi Jiao (Rhinoceros Horn, 3 fen), Gan Cao (Licorice, 2 fen), Sheng Jiang (Zingiberis Rhizoma Recens, 3 fen).
Boil in one sheng of water until seven ge remain. Strain and take warm in divided doses.
If Yang Xian occurs two or three times daily and persists, turning into Yin Xian, this is Huai Bing (Ruined Disease). Yin Xian should not have sweat; if sweating occurs, fear of Wang Yang (Collapse of Yang) arises; Si Ni Tang (Four Reversals Decoction) is indicated.
Jing disease with lethargy, coarse breathing, and sawing sounds internally is treated with Mai Men Jia Zhu Li Tang (Ophiopogon Plus Bamboo Sap Decoction).
Jing disease with epistaxis is treatable; hematemesis is untreatable. Why? Epistaxis disperses pathogenic Heat; hematemesis indicates failure of Ying Blood.
Yin Xian is Man Jing. If nursing with loose stools, it is named Man Pi Feng (Slow Spleen Wind), difficult to treat.
Tian Diao involves coma and aversion to light; if nursing with heat. Nei Diao involves crying, drowsiness, twitching, and cold.
Infant Umbilical Wind or Fetal Fright with stone-hard chest/abdomen, backward staring, and inability to look down is treated with Zi Shuang Wan. If stool passes shortly after taking the pill, survival; if not, death.
Fright-Heat: do not turn off lights at night. Turning off lights allows Yang to retreat and Heat to rise, increasing startle jumps and rendering medicine ineffective.
Jing disease with fish-like eyes, wheezing, short breath, forehead sweat, and "chi chi" sound upon pressing nails indicates death.
Sudden fever, Phlegm-wheezing, shoulder breathing, "chi chi" crying, and inability to nurse is named Chan Hou Feng (Coiling Throat Wind), untreatable.
Yang Xian with cold nose/mouth, exposed eyes, slight startle, and sweating indicates Wang Yang; Gui Zhi Fu Zi Jia Huang Qi Tang (Cinnamon Twig, Aconite, plus Astragalus Decoction) is indicated.
Jing disease always involves Phlegm. Phlegm moves with Heat; when Heat subsides, it hides in the Spleen. If Heat is excessive and Phlegm moves, Jing Xian erupts.
If all four signs and eight manifestations of Jing Xian are present, it is untreatable.
