The disease of chuangzhen (skin eruptions and rashes) is caused by heat-toxin. It presents with aversion to cold and fever, yet appetite and drinking remain normal. This indicates the onset of chuangzhen. If alternating chills and fever occur, accompanied by thirst and a slippery pulse, and sores erupt on hot spots of the body, it is called Chuangyang (sores and ulcers). If there is aversion to cold and fever, a slippery pulse, and red macules on the skin, it is named Dandu (erysipelas). If there is shivering chills and redness of the muscles with abnormal pulse, it is named Hongsi Ding (red-thread carbuncle). Hongsi Ding spreads rapidly; the treatment method requires urgent bloodletting with a bi stone (lancet) to drain toxic blood. The main formula is Yelangya Tang (Wild Wolf's Tooth Decoction). For pediatric erysipelas, presenting as cloud-like patches similar to tinea or scabies, or toxic heat causing throat swelling, and capable of killing various intestinal parasites, Yelangya Tang is indicated.
Yelangya Tang Formula
Yelangya Cao (Herba Siegesbeckiae, 5 fen) Fangfeng (Radix Saposhnikoviae, 5 fen) Eshi (Fructus Arctii, 1 qian) Gancao (Radix Glycyrrhizae, 3 fen) Shengjiang (Rhizoma Zingiberis Recens, 3 fen)
Take the above five herbs, add one sheng of water, boil to obtain seven ge of liquid, remove the dregs, add Sanmoli Bai (Pistacia weinmannifolia resin, 3 fen), mix evenly, and take warm in divided doses. If a child’s body has dry blood and forms white crusts at the lesion, it is called Baiyou Feng (white wandering wind); Tuoli Tang (Support the Interior Decoction) is indicated.
Tuoli Tang Formula
Danggui (Radix Angelicae Sinensis, 5 fen) Huangqi (Radix Astragali, 3 fen) Honghua (Flos Carthami, 3 fen) Yecan (Bombyx Batryticatus, 1 piece) Eshi (Fructus Arctii, 5 fen) Gancao (Radix Glycyrrhizae, 3 fen) Dazao (Fructus Jujubae, 2 pieces)
Chop the above seven herbs, add one sheng of water, boil to obtain seven ge of liquid, remove the dregs, and take warm in divided doses. If a newborn has erosion and ulceration at the nose, mouth, or anus, this is the activation of residual toxin; Tuodu Tang (Expel Toxin Decoction) is indicated.
Tuodu Tang Formula
Xiao Lianqiao (Fructus Forsythiae, 1 qian) Guiban (Carapax et Plastrum Testudinis, soaked in vinegar and roasted until crushed, 1 qian) Hongqu (Massa Medicata Fermentata, stir-fried, 5 fen)
Add two sheng of water to the above three herbs, boil to obtain one sheng of liquid, remove the dregs, and take warm in divided doses. If the erosion is severe, add Renzhongbai (Concretio Saxea, 3 fen), mix well, and take warm in divided doses. If pediatric residual toxin causes foul-smelling erosion and other formulas are ineffective, Yizi Hui (One-Character Ash) is indicated.
Yizi Hui Formula
Xama (Bufo bufo gargarizans, burnt to ash, 1 qian) Fanshi (Alumen, roasted, 2 fen) Duanshi (Calx, water-washed, 2 fen) Longnao (Borneolum Syntheticum, 2 fen) Shexiang (Moschus, 1 fen)
Grind and mix the above five herbs, apply to the sore, two to three times daily, or mix with egg white for application. If pediatric fetal toxin or residual toxin causes erosive infiltration, Qiliang Gao (Qiliang Ointment) is indicated.
Qiliang Gao Formula
Qiliang (Herba Siegesbeckiae, 1 liang 8 qian) Guiban (Carapax et Plastrum Testudinis, 1 liang) Jizi Qiao (Testudinis Oöthecae, 5 qian)
Grind and sieve the above three herbs, mix with maltose, and take pills the size of a bullet. For pediatric malignant sores with pain and itching, or dripping pus that does not cease, or adults with mild toxic bone pain, old or recent cases with rotting and foul odor, or destruction of the throat, lips, and nose where other medicines fail, Zibao Dan (Zibao Pill) is indicated.
Zibao Dan Formula
Guiban (Carapax et Plastrum Testudinis, vinegar-processed, 2 liang) Shijueming (Concha Haliotidis, soaked in red-infused child’s urine) Tianling (Cranium humanum, black-roasted) Yinlu Po (Residue from silver smelting, each 6 qian)
Pound and sieve the above four herbs, make pills with vinegar paste. Children take 5 fen, adults take 1 qian. For joint pain, take with warm wine. For rotting and foul odor, take with Qiliang Tang. In Shanghan (Cold Damage) Yangdu (Yang Toxin), if the rash is red and sparse, it heals spontaneously in five to six days. If purple-black and dense like honey, causing high fever, it is incurable. If Yangdu presents with cold body but extreme thirst, it is difficult to treat. If the body is hot with irritability and thirst, Huaban Jia Xijiao Tang (Banxia Decoction plus Rhinoceros Horn) is indicated. If Yangdu rash resembles brocade patterns, it is due to extreme heat; Huoji Tang (Fire-Ji Decoction) is indicated. If Yindu (Yin Toxin) affects the chest, back, or limbs, appearing like mosquito bites, Dajianzhong Tang (Greatly Tonify the Middle Decoction) is indicated; Huangqi Jianzhong Tang (Astragalus Decoction to Tonify the Middle) is also applicable. If a child’s limbs or back/abdomen have emerging and disappearing rashes, it is called Fengzhen (Wind Rash); Guima Gehan Jia Jingjie Fangfeng Tang (Guima Half-Decoction plus Schizonepeta and Saposhnikovia) is indicated. Fengzhen is an acute toxin; severe itching leads to rash upon scratching, known as Xiefeng (Draining Wind); applying ginger juice resolves it. For irritable and severe itching from wind rash:
External Fomentation Formula for Yin Toxin Rashes
Fanshi (Alumen, 4 qian) Yanxi (Sal, 2 liang) Zhangnao (Camphora, 3 qian)
Boil the above three herbs in five sheng of water until reduced by two ge, and foment repeatedly. If a child has aversion to cold and fever, and after the fever subsides, blisters resembling smallpox appear, it is called Tianpao Zhen (Heaven Blister Rash); Xiaodu Yin (Detoxifying Drink) is indicated. If a child suffers from epidemic febrile disease with clustered heat, dry mouth and thirst, rapid pulse, and snoring sleep, Huaban Tang (Banxia Decoction) is indicated. If a child has millet-like rashes in summer, it is called Sha Chuang (Sand Sore); bath with Kushen Tang (Sophora Root Decoction).
Kushen Tang Formula
Kushen (Radix Sophorae Flavescentis, 1 jin) Shaonao (Borneolum Naturalis, 2 liang)
Chop the above two herbs, boil in one hu of water, and bathe. Two to three times daily. If a child’s body is wet and rotting with infiltrating pain and itching, it is called Jinyin Chuang (Infiltrating Sore); Fangfeng Paidu Yin (Saposhnikovia Decoction to Expel Toxin) is indicated.
Fangfeng Paidu Yin Formula
Fangfeng (Radix Saposhnikoviae, 1 qian) Yecan (Bombyx Batryticatus, 1 piece) Fushe (Agkistrodon, stir-fried and ground, 3 fen) Shengjiang (Rhizoma Zingiberis Recens, 3 fen) Dazao (Fructus Jujubae, 3 pieces) Gancao (Radix Glycyrrhizae, 3 fen)
Add two sheng of water to the above six herbs, boil five herbs (note: text specifies five, likely implying specific preparation order or omission of one in boiling phase, but standard practice boils all; following text strictly: boil five) to obtain one sheng, remove dregs, add Fushe powder, mix, and take warm in divided doses. If a patient originally had sores that suddenly healed, it is an evil omen; if they originally had no sores and suddenly develop them, it is a good omen. If rashes emerge from finger webs, with spreading irritability and itching, it is called Xuanjie (Tinea and Scabies); Fangfeng Jiedu Tang (Saposhnikovia Decoction to Resolve Toxin) is indicated.
Fangfeng Jiedu Tang Formula
Fangfeng (Radix Saposhnikoviae, 5 fen) Jingjie (Herba Schizonepetae, 3 fen) Eshi (Fructus Arctii, 5 fen) Xiao Lianqiao (Fructus Forsythiae, 5 fen) Dahuang (Radix et Rhizoma Rhei, 3 fen) Huotanmu (Herba Polygoni Ayacastanae, 5 fen) Gancao (Radix Glycyrrhizae, 3 fen)
Add two zhong of water to the above eight herbs, boil to obtain one zhong, remove dregs, and take warm in divided doses. Sores and rashes must be painful and itchy; if pus drips and transmission occurs upon contact, use Wutou Jian (Aconite Decoction) for washing.
Wutou Jian Formula
Chuan Wutou (Radix Aconiti Lateralis Preparata, 4 qian)
Add three sheng of water to the single herb, boil to obtain two sheng, and wash. Two to three times daily. If chuangzhen suddenly dries up, accompanied by abdominal distension, forced breathing, and irritability, it is due to internal retention of toxin; Mahuang Yecan Tang (Ephedra and Bombyx Decoction) is indicated.
Mahuang Yecan Tang Formula
Mahuang (Herba Ephedrae, 3 fen) Yecan (Bombyx Batryticatus, 1 piece) Xingren (Semen Pruni Armeniacae, 7 seeds) Sangbaipi (Cortex Morus Radicis, 2 fen) Chenxiang (Lignum Aquilariae Resinatum, 2 fen) Dingxiang (Flos Caryophylli, 2 fen) Hongdou (Semen Phaseoli, black-roasted, 1 qian) Muxiang (Radix Aucklandiae, 2 fen) Ruxiang (Resina Frankincense, 2 fen) Shengjiang (Rhizoma Zingiberis Recens, 2 fen) Gancao (Radix Glycyrrhizae, 2 fen) Dazao (Fructus Jujubae, 3 pieces)
Add two sheng of liaoshui (rainwater) to the above twelve herbs, first boil Mahuang, remove foam upon boiling, add remaining herbs, boil to obtain one sheng, and take warm in three doses. If sores/rashes are triggered by contact, causing irritability, itching, and pain upon scratching, but only itching remains, use Mangcao Tang (Metasphenide Decoction) for washing.
Mangcao Tang Formula
Mangcao (Herba Metasphenidis, 1 jin) Shaonao (Borneolum Naturalis, 1 liang) Yanxi (Sal, half jin)
Add one dou two sheng of water to the above three herbs, boil to obtain six sheng, remove dregs, and wash warm. Two to three times daily, until healed. If a child in summer has facial swelling and slight heat, with sand-like sores, it is caused by lacquer exposure. For lacquer sore with heat, pain, and irritability, use Wuhuanzi Pi Jian (Soapberry Peel Decoction).
Wuhoanzi Pi Jian Formula
Wuhoanzi (Fructus Sapindus, ten pieces, flesh removed)
Add two sheng of water to the single herb, boil to half sheng, remove dregs, and blow into the lesion using a tube. When blisters rise, collect the fluid and apply to the affected area. For lacquer sore with severe irritability and difficulty healing, use Tiejiang (Iron Slurry) irrigation; Fanshi Tang (Alumen Decoction) is also indicated.
Fanshi Tang Formula
Fanshi (Alumen, 1 jin)
Add one hu of vinegar and two hu of water to the single herb, boil, and bathe. For toxic bone pain or hong kong jiao (gouty arthritis) rushing to the heart, bathe with this. If a child’s rash fluctuates with slight fever or vomiting milk, it is caused by "Jionai" (transition of milk/toxin). Treating head sores with aggressive attack is a mistake. If attacked, the sore suddenly dries, phlegm rattles, breathing becomes short, abdomen distends, and limbs become cold; Mahuang Yecan Tang is indicated. Also, soak a slice in warm wine and foment the head. If the sore moistens, life returns; otherwise, death ensues. Head sores with white patches and crusts, itchy or not, are called Baitu Chuang (White Baldness Sore); Yelangya Tang is indicated. For stubborn pediatric Baitu Chuang, Songzi Hui San (Pine Seed Ash Powder) is indicated.
Songzi Hui San Formula
Songzi Hui (Ash of Pine Seeds, 4 qian) Shaonao (Borneolum Naturalis, 4 qian)
Grind and sieve the above two herbs, mix with egg white, and apply. If hair is smelly and itchy, falling off in patches when scratched, it is called Baixie Feng (White Scale Wind); Baixie San (White Scale Powder) is indicated.
