Qian Yi (courtesy name Zhongyang) stated: The pathogenic factor of heat-toxin invades the *coulī* (the interstitial spaces between skin and muscle), contends with the blood and qi, and manifests on the external skin. The affected area appears red like cinnabar. When heat-toxin contends with blood, and wind-pathogen takes advantage of this deficiency, it results in redness, swelling, pain, and a migratory nature that spreads throughout the body. This condition often arises from the wet nurse consuming excessive amounts of alcohol, wheat products, and fried or roasted foods, or from drying clothes with fire and dressing the infant before they have cooled down. Symptoms may appear on the head, face, chest, and back, causing the infant to feel irritable, stifled, and bloated, with internal heat resembling burning fire and unbearable pain. If the toxin penetrates into the abdomen or kidneys, the condition becomes incurable.
Alternatively, within the first week after birth, an infant's skin, muscles, sinews, bones, marrow, brain, five zang organs, six fu organs, *ying* (nutritive) and *wei* (defensive) qi, and blood and qi are not yet solidified. They are like the sprouts of grass and trees, untested by cold and heat, tender and weak; hence infants are called "sprout children." During this week, frequent bathing should be strictly avoided. Otherwise, damp-heat may stagnate and vaporize without dispersing, leading to *chiyou dandu* (migratory erysipelas) covering the whole body. Commonly known as "liu," these lesions resemble smears of rouge, presenting with swelling and high fever. If the toxin enters the abdomen, it causes abdominal distension, hardness, and qi obstruction, potentially leading to death. This is caused by improper bathing practices. If the muscles are lax and the *coulī* open, and if wrapping and care are inappropriate, allowing wind-pathogen to invade, *bai liu* (white migratory erysipelas) may appear on the body, also characterized by swelling and high fever. Other symptoms may include aversion to cold with high fever, nasal congestion and mental fogginess, upward rushing of qi with phlegm and wheezing, coughing, and vomiting. All these ailments result from exposure to temperature changes during bathing and dressing.
To treat *chi liu dandu*, one may use a small knife to gently puncture the red halo at the head of the lesion to drain evil blood and toxic fluid. Subsequently, *Binghuang San* (Ice and Yellow Powder), *Gegen Baizhu San* (Pueraria and Atractylodes Macrocephala Powder), or *Xingxiong San* (Awakening and Asarum Powder) should be administered as primary treatments. *Baidou San* (Defeat Toxin Powder), *Shengma Tang* (Cimicifuga Decoction), and *Fangji San* (Stephania Tetrandra Powder) are also optional choices. Furthermore, for conditions such as *dan re* (erysipelas heat), *shi re* (excess heat), and *long dai re* (dragon belt heat), *Dalianqiao Tang* (Large Forsythia Decoction) added with *Dahuang* (Rhubarb Rhizome) and *Dengxincao* (Juncus Effusus) should be decocted and taken orally, showing remarkable efficacy.
**Binghuang San (Ice and Yellow Powder)**
*Tuxiao* (Mineral salt/Niter) and *Dahuang* (Rhubarb Rhizome), both finely powdered, two qian each.
Mix the two ingredients, stir evenly with fresh cool water, dip a chicken feather into the mixture, and apply it topically to the affected area.
**Gegen Baizhu San (Pueraria and Atractylodes Macrocephala Powder)**
Indicated for red and white erysipelas swelling and toxicity.
*Bai Zhu* (Atractylodes Macrocephala Rhizome) 2.5 qian; *Fu Ling* (Poria) peeled, 2 qian; *Mu Xiang* (Aucklandiae Radix); *Gan Cao* (Licorice Root) 2 qian each; *Shaoyao* (Peony Root); *Ge Gen* (Puerariae Lobatae Radix) 3 qian each; *Zhi Ke* (Aurantii Fructus Immaturus) de-seeded and stir-fried with bran, 2.5 qian.

**Baidou San (Defeat Toxin Powder)**
See Question 50.
**Fangji San (Stephania Tetrandra Powder)**
Indicated for erysipelas syndrome. It occurs when heat-toxin contends with blood and wind-pathogen invades, causing redness, swelling, and migratory spread throughout the body, also known as *chiyou feng* (migratory red wind). If the toxin enters the abdomen and kidneys, it can be fatal.
*Han Fang Ji* (Stephaniae Tetrandrae Radix) half liang; *Puxiao* (Natrii Sulfas); *Xi Jiao* (Rhino Horn - note: substitute with legal alternatives in modern practice); *Huang Qin* (Scutellariae Radix); *Huang Qi* (Astragali Radix); *Chuan Sheng Ma* (Cimicifugae Rhizoma) 2.5 qian each.

*Huashi* (Talcum); *Hanshui Shi* (Calcitum) one liang each.
Grind into fine powder. Mix with rice vinegar and apply to the affected area. If swelling extends to the external genitalia with ulceration, mix with plain water only.
**Dalianqiao Tang (Large Forsythia Decoction)**
See *Dalianqiao Yinzi* (Large Forsythia Infusion) in Question 3.
**Xiaodu Yin (Detoxifying Infusion)**
Indicated for erysipelas and sores where toxins are obstructed, manifesting as high fever, mania, restlessness, constipation, sore throat, chest oppression, and post-variolous resolution. It can be taken after the rash has subsided and broken.
*Niu Bang Zi* (Arctii Fructus) stir-fried, two liang; *Jingjie Sui* (Schizonepetae Spica); *Gan Cao* (Licorice Root) honey-roasted, half liang each.

For heat patterns, add *Huang Qin* (Scutellariae Radix), *Fang Feng* (Saposhnikoviae Radix), and *Xi Jiao* (Rhino Horn) to form the *Xiaodu Yin*.
