Lu Zhongyang stated: All types of Gan syndrome (malnutrition syndrome characterized by emaciation and digestive disorders) are diseases of the Spleen and Stomach, caused by the depletion of body fluids internally. This occurs after severe illnesses or following vomiting and diarrhea, where physicians inappropriately use emetic or purgative drugs, leading to Spleen and Stomach weakness and internal loss of body fluids. Moreover, children suffering from Gan syndrome often have their conditions worsened by incompetent physicians. For instance, tidal fever (fever that occurs at regular intervals) indicates one Zang organ is deficient and another is excess, generating internal deficiency heat. The treatment principle should be to tonify the mother organ and drain the excess of the affected organ for recovery. If tidal fever occurs at noon, it indicates Heart deficiency heat. Since the Liver is the mother of the Heart, one should first tonify the Liver; after the Liver qi is sufficient, then drain the Heart. Once the Heart receives the nourishment from its mother, internal balance is restored, and the tidal fever resolves. Physicians seeing tidal fever might wrongly assume it is an excess condition and use cold-purgative drugs such as Radix Rheum palmatae (Dahuang, 大黄) and Natrii Sulfas (Yaxiao, 牙硝). Excessive purging leads to an inability to retain fluids, causing internal depletion of body fluids, which results in Gan syndrome. There is also Pi accumulation disease, which presents with alternating chills and fever, thirst, a palpable mass under the hypochondrium that is hard and painful. The treatment for Pi accumulation should involve gradual dissipation. However, physicians mistakenly use drastic purgatives like Radix et Rhizoma Crotonis (Badou, 巴豆) and Semen Kochiae (Qianniuzhong, 牵牛子). Children's constitutions easily shift between deficiency and excess; excessive purging depletes stomach fluids, gradually leading to emaciation characteristic of Gan syndrome. Yang Shi said: Children's Zang-fu organs are delicate and tender; overeating causes injury easily. If breastfeeding and diet are irregular, few will escape developing Gan syndrome. Thus, Gan syndrome arises from damage. Indulgence in sweet, fatty, sticky, cold, raw, salty, or sour foods causes stagnation in the Middle Jiao, forming Gan syndrome due to food accumulation. Alternatively, if the nursing mother fails to regulate warmth and cold properly, eats irregularly, experiences emotional extremes or sexual exhaustion, and then breastfeeds the infant, Gan syndrome may enter through the transmission of Qi from the mother's illness. Is this not the fault of the family failing to provide proper care? The manifestations of Gan syndrome include: shiny and tight scalp, dry and sparse hair, sunken cheeks and dry nose, white lips with abnormal appetite, dim and ulcerated eyes, rubbing of nose and eyebrows, protruding spine, yellowish complexion, teeth grinding and nail biting, intense thirst with spontaneous sweating, clear urine and sour-smelling stool, abdominal distension with borborygmus, Pi accumulation with tidal fever, extreme craving for fruits, vegetables, salt, and sour foods, pica (eating charcoal, rice, mud), and incessant drinking. These are all symptoms.
Gan syndrome is classified into Five Gan syndromes based on the involved Zang organs:
1. Heart Gan (also called Jing Gan/Startle Gan): External symptoms include high fever, red face and lips, sores in mouth and tongue, chest oppression, dark and scanty urination, five-palm heat, night sweats, thirst, teeth grinding, and false fright.
2. Liver Gan (also called Feng Gan/Wind Gan): External symptoms include head shaking and eye rubbing, white membrane covering the pupil, blueish eyes with excessive tearing, dry and erect hair, cyanotic tendons and hot brain, irritability, thirst, profuse sweating, diarrhea with sores and tinea.
3. Kidney Gan (also called Ji Gan/Urgent Gan): External symptoms include hot brain and emaciated muscles, ice-cold hands and feet, alternating chills and fever, slippery diarrhea and abdominal pain, dry mouth and nose with thirst, gum sores, black nails, dark complexion, and frequent skin sores and scabies.
4. Lung Gan (also called Qi Gan or Gan Chuan/Gan Asthma): External symptoms include cough and wheezing, high fever with aversion to cold, miliary skin rash, nasal itching with runny nose, throat discomfort, facial ulcers with bloody saliva, abdominal distension, dry hair, and frequent diarrhea.
5. Spleen Gan (also called Shi Gan/Food Gan or Nai Gan/Milk Gan): Internal symptoms include yellowing of the body and face, large abdomen with thin legs, vomiting and abdominal fullness, indigestion of water and grain, sour-smelling diarrhea, sleeping prone with drowsiness, reduced appetite, and pica (eating dirt).
