The clinical manifestations of Slow Spleen Wind (Man Pi Feng) include: a bluish complexion, particularly on the forehead; a short tongue with the head drooping; eyes closed and difficult to open; shaking the head and protruding the tongue while in a drowsy sleep; frequent vomiting of foul-smelling substances; clenched teeth and grinding; mild twitching of the hands and feet that cannot be voluntarily controlled; or cold body temperature, or warm body temperature with cold limbs. The pulse is deep and faint. This indicates extreme excess of Yin Qi (Yin energy) and extreme deficiency of Stomach Qi (Stomach energy). Only one or two out of ten patients may survive. This condition typically arises after Slow Convulsion (Man Jing), where vomiting and diarrhea damage the Spleen. The disease has progressed to an extreme stage, ultimately returning to deficiency, specifically affecting the Spleen, hence it is called "Spleen Wind." If one attempts to dispel wind, there is no wind to dispel; if one attempts to treat convulsions, there are no convulsions to treat. There is merely phlegm turbidity obstructing the diaphragm and alternating deficiency heat. The closed eyes result from Spleen fatigue and Qi exhaustion, causing the spirit and consciousness to become submerged due to stagnation of saliva and phlegm. What the world calls "difficult-to-treat slow wind" refers to Slow Spleen Wind. However, Slow Spleen Wind is also known as "Deficiency Wind." Whenever infants or children experience vomiting or diarrhea, followed by a sallow complexion and general deficiency, if fever ensues due to this deficiency, it will inevitably progress to Slow Spleen Wind. As soon as symptoms such as head shaking, staring straight ahead, reaching out to touch people, drowsiness and preference for sleep, excessive sweating on the forehead, sticky sweat on the body, and a low, small, and hoarse voice appear, these are signs of Spleen Wind. It is not necessary for these symptoms to always arise from the sequential transmission of Acute Convulsion or Slow Convulsion; this point must be recognized. The general principle of treatment is to generate Stomach Qi and restore Yang. One may consider using half doses each of Hei Fu Tang (Black Aconite Decoction), Chuan Wu San (Chuanwu Powder), Jin Ye Dan (Golden Liquid Pill), Bai Wan Zi (White Pellet), and Sheng Fu Si Jun Zi Tang (Raw Aconite Four Gentlemen Decoction), adjusting according to the situation. Once Stomach Qi gradually recovers, use formulas like Yi Gong San (Extraordinary Efficacy Powder) to regulate gently and mildly. Formulas such as Xie Fu San (Scorpion and Aconite Powder) and Yin Xian San (Yin Epilepsy Powder) can also be consulted. If the patient's eyes are half-open and half-closed, and the hands and feet are not cold, the condition remains within the scope of Slow Convulsion, so restoring Yang methods should not be used. If the condition has already entered Slow Spleen Wind but Yang Qi has not severely collapsed, Sulfur (Liu Huang) and Aconite (Fu Zi) should not be used. For those taking Yang-restoring decoctions who show gradual warming of the hands and feet, follow up with formulas like Xing Pi San (Awaken Spleen Powder) for regulation. Do not lightly use extremely potent agents.
Hei Fu Tang (Black Aconite Decoction): Treats severe Slow Spleen Wind with cold limbs. Discontinue subsequent doses once the hands and feet warm up and consciousness returns.
Ingredients: Prepared Aconite Root (Pao Fu Zi, 3 qian), Costus Root (Mu Xiang, 1.5 qian), Prepared White Aconite Rhizome (Pao Bai Fu Zi, 1 qian), Prepared Licorice Root (Zhi Gan Cao, 0.5 qian).
Preparation and Administration: Coarsely powder the above ingredients. Take 3 qian per dose, add five slices of fresh ginger, decoct until half the liquid remains, and administer with a spoon.
Chuan Wu San (Chuanwu Powder): Dispels wind and restores Yang.
Ingredients: Raw Chuanwu Tuber (Sheng Chuan Wu, 1 qian), Whole Scorpion (Quan Xie), Costus Root (Mu Xiang) (each 0.5 fen).
Preparation and Administration: Grind into fine powder. Take 1 qian per dose, add four slices of fresh ginger, decoct until half the liquid remains, and drip into the mouth intermittently. Add Clove Bud (Ding Xiang) if vomiting occurs.
Jin Ye Dan (Golden Liquid Pill): Treats prolonged vomiting and diarrhea, Spleen and Stomach deficiency, cold limbs, mental confusion and fatigue, excessive sleep with exposed eyes, cold breath from mouth and nose, and impending development of Slow Convulsion.
Ingredients: Imported Sulfur (Bo Shang Liu Huang, 10 liang). Process: Fly the sulfur first, remove sand and stones, weigh, grind into fine powder, place in a jar, seal the seams with Red Stone (Chi Shi Zhi), secure with salt mud, calcine over charcoal fire for three days and nights, remove, and let cool before use.
Preparation and Administration: Grind the processed sulfur into fine powder using a willow wood pestle in a mortar. Take 2 qian per dose, swallow with ginger water.
Sheng Fu Si Jun Zi Tang (Raw Aconite Four Gentlemen Decoction): Assists the Stomach and restores Yang.
Ingredients: Based on the aforementioned Si Jun Zi Tang (Four Gentlemen Decoction), add one-quarter amount of Raw Aconite Powder. For patients with cold limbs, double the amount. Take 0.5 qian per dose, add five slices of fresh ginger, decoct slowly, and administer with a spoon.
Yi Gong San (Extraordinary Efficacy Powder): Treats loss of appetite after vomiting and diarrhea, warms the middle and harmonizes Qi, and addresses Spleen and Stomach deficiency-cold. Administer several doses initially to rectify Qi.
Ingredients: Ginseng Radix (Ren Shen), Poria (Fu Ling), Atractylodes Macrocephala Rhizome (Bai Zhu), Licorice Root (Gan Cao), Tangerinae Pericarpium Rubrum (Ju Hong), Costus Root (Mu Xiang) (equal parts).
Preparation and Administration: Grind into fine powder. Take 2 qian per dose, decoct with fresh ginger and jujube. Another variant omits Costus Root.
Xie Fu San (Scorpion and Aconite Powder): Restores Yang Qi and expels wind-phlegm.
Ingredients: Whole Scorpion (Quan Xie, 1 individual), Prepared Aconite Root (Pao Fu Zi, 2 qian), Prepared Arisaema Rhizome (Pao Nan Xing), Prepared White Aconite Rhizome (Pao Bai Fu Zi), Costus Root (Mu Xiang) (each 1 qian).
Preparation and Administration: Grind into fine powder. Take 0.5 qian per dose, add four slices of fresh ginger, decoct slowly, and take orally.
Xing Pi San (Awaken Spleen Powder): See Question 16.
Bao Ming Dan (Life-Preserving Pill): See Zhi Sheng Bao Ming Dan (Supreme Life-Preserving Pill) in Question 14.
Yin Xian San (Yin Epilepsy Powder): Dispels wind, expels phlegm, restores Yang, and corrects the Stomach.
Ingredients: White Aconite Rhizome (Bai Fu Zi), Black Aconite Root (Hei Fu Zi), Arisaema Rhizome (Nan Xing), Pinellia Tuber (Ban Xia) (all raw, each 1.5 fen).
Preparation and Administration: Grind into fine powder. Soak well water for seven days, changing the water daily. After soaking, drain well. Then add 2 qian of powdered whole scorpion. Take one "zi" measure per dose, swallow with ginger water.
