Lu Bosi stated: "Xu Lei" (body weakness and emaciation) results from disharmony of the Spleen and Stomach, leading to an inability to digest milk and food, thereby causing emaciated muscles and skin. It may also occur after severe illness or following vomiting and diarrhea, where the Spleen and Stomach remain weak and fail to transform and transport grain qi. In cold-type cases, there is frequent loose stool, with bluish-white lips and mouth. In heat-type cases, there is elevated body temperature, feverish sensation, and slightly yellowish muscles; these constitute cold and heat Xu Lei. For cold types, Muxiang Wan (Aucklandia Pill) is the primary treatment; it should not be taken in summer, but if symptoms arise, a small dose may be administered. For heat types, Hu Huang Lian (Coptidis Rhizoma / Picrorhiza Root depending on context, here likely Coptis or Picrorhiza used for clearing deficiency heat) is the primary treatment; it should not be taken in winter, but if symptoms arise, a small dose may be administered. For Xu Lei following Shang Han (Cold Damage), Zhuye Tang (Bamboo Leaf Decoction) is the primary treatment. Regularly taking Sijunzi Tang (Four Gentlemen Decoction), Yigong San (Yi Gong Powder), or Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder) are also suitable. Jutian Wan (Citrus and Coptis Pill) taken over a long period promotes flesh gain; Fei'er Wan (Child-Nourishing Pill) can be taken concurrently.
