The *Yellow Emperor's Inner Canon* states: If one is injured by summerheat in summer, malaria will inevitably occur in autumn. "Malaria" implies invasion and torment. There are five names for it: Dan malaria (febrile malaria), Han malaria (cold malaria), Shi malaria (damp malaria), Pin malaria (yin malaria), and Mu malaria (yang malaria). Although the syndromes differ, they are all caused by external contraction of the Six Excesses (Wind, Cold, Summerheat, Dampness, Dryness, Fire) and internal injury by the Seven Emotions (Joy, Anger, Worry, Pensiveness, Sorrow, Fear, Fright). Cold qi lodges in the Couli (interstices of skin and muscles), and phlegm-fluids accumulate in the chest and diaphragm. Symptoms include yawning, cold extremities, thirst, vexation, bluish-yellow complexion, bodily stiffness and tension, paroxysmal chills, and intermittent fever. It is also said: Wind, cold, summerheat, and dampness enter from outside; improper diet and lifestyle generate pathogens from within. Generally, summerheat pathogen contends with healthy qi, causing a struggle between Yin and Yang, with mutual victory and defeat. When pathogen combines with Yang, heat prevails; when it combines with Yin, cold prevails. Yang excess leads to more heat; Yin excess leads to more cold; alternating excess and deficiency of Yin and Yang result in severe high fever. When Yin and Yang separate, relief is achieved through sweating. Wei Qi (Defensive Qi) travels in the Yang meridians during the day and in the Yin meridians at night; emerging due to Yang, attacks occur during the day. Ying Blood (Nutritive Blood) travels in the Yin meridians at night and in the Yang meridians during the day; contending internally due to Yin, attacks occur in the evening. Ancient texts say: Attacks once daily indicate recent infection and are easier to treat. Attacks every two or three days indicate deep-seated infection and are harder to treat, though this rule is not absolute. Some cases occurring every two or three days are easier to treat, while daily attacks may be harder to manage with medication. General principles: A wiry and rapid pulse indicates heat; a wiry and slow pulse indicates cold. Wind is dispersed; cold is warmed; phlegm is cleared; food stagnation is resolved. Observe symptoms and palpate pulses, treating according to the meridians. Why do infants who have not consumed raw/cold foods or been exposed to summerheat contract malaria? This is because caregivers may expose them to wind and cold by undressing them improperly, allowing pathogens to enter the Couli. Additionally, food stagnation can cause tidal fever around dusk (Shi hour, 3-5 PM), resembling but not being malaria; this should be treated with digestion-promoting methods. Careful differentiation is required.
