Lei Tou Feng (Thunderhead Wind) phlegm syndrome presents with the most acute onset. Its symptoms resemble Taiyang disease (disease of the Taiyang channel), with head pain as if split by an axe, eye pain as if drilled by an awl, body sensation as if scorched by fire, unsmooth bowel movements, red and astringent urination, unbearable pain, and unpredictable severity due to severe blood stasis and stagnation. It is essential to know that pathogenic factors must be expelled rapidly; medical attention should be sought immediately without delay. The primary treatment principle is to purge fire, but one must guard against damaging Stomach fluids, lest pure Qi be forced and depleted, ultimately leading to therapeutic failure.
This condition, regardless of whether the headache is unilateral or central, occurs when headache is accompanied by phlegm. If the pain is extreme and unbearable, accompanied by fever, eye pain, and constipation, it is called Da Lei Tou Feng (Great Thunderhead Wind). If the headache is accompanied by stools that are initially moist then dry, and urine that is initially clear then astringent, it is called Xiao Lei Tou Feng (Small Thunderhead Wind). The major form causes rapid harm, while the minor form is slightly slower. Although there is a distinction between major and minor forms, the treatment principles are identical. Delaying treatment leads to unpredictable complications, inevitably damaging the eyes: mild cases result in exophthalmos (protrusion of the eyeball), while severe cases result in toxic nodules. Early intervention is necessary to prevent disaster. Suitable prescriptions include:
Qing Zhen Tang (Clear and Shake Decoction): Indicated for cases accompanied by fever, aversion to cold, and thirst.
Shengma (Rhizoma Cimicifugae), Chishao (Radix Paeoniae Rubra), Gancao (Radix Glycyrrhizae), Jingjie Sui (Cacumen Schizonepetae), Gegen (Radix Puerariae), Su Bohe (Herba Menthae Haplocalycis), Huangqin (Radix Scutellariae), Qing Heye (Folium Nelumbinis Viride), Cangzhu (Rhizoma Atractylodis Lanatae - soaked overnight in rice wash water, stir-fried, equal parts).

Grind into fine powder, mix with water to form pills the size of mung beans. Take 2 qian per dose, swallowed with tea infusion after meals and before bed.
Yao Zhen Fang (Medicated Pillow Formula): Treats Tou Feng (Wind Headache) and vertigo.
Tongcao (Medullae Tetrapanacis), Fangfeng (Radix Saposhnikoviae), Shichangpu (Rhizoma Acori Tatarinowii), Gancao (Radix Glycyrrhizae), Xijiao (Cornu Rhinocerotis - powdered), Lingyangjiao (Cornu Saigae Tataricae - powdered), Manjingzi (Fructus Viticis Triphyllae, each 3 qian), Xixin (Radix Asari), Baizhi (Radix Angelicae Dahuricae), Ben (Radix Ligustici Wallichii), Zhen Chuanxiong (Rhizoma Chuanxiong), Baizhu (Rhizoma Atractylodis Macrocephalae), Heidou (Semen Sojae Nigrum - 1.5 jin, cleaned).
Grind into fine powder, mix evenly, fill tightly into a raw silk bag, and place inside a box shaped like a pillow. When sleeping, remove the lid to allow medicinal qi to penetrate the head; cover when not sleeping to preserve medicinal qi. As the pillow compresses over time, refill with the previous powder, ensuring it is full, or add Heidou. After three to five days, if the medicinal scent weakens, replace with fresh medicine. Using the pillow for ten days to one month, thunder-like sounds in the ears indicate the successful extraction of wind pathogens.
