According to the commentary: Swelling (referring here to redness and swelling of the eye) can be caused by wind-heat attacking upwards, or by dry-fire pathogenic factors; or severe pain in the black pupil (cornea/pupil area), or swelling and pain of the white sclera. All these are due to excess heat in the Liver channel, or heat transferring to the Lung, and all should be treated with clearing fire and dispersing wind methods.
When the eye swelling is cup-shaped and most painful, with excessive tearing, aversion to heat, and photophobia, if the pain extends to the head and orbits, this is a calamity of Wood-Fire, which is not light; if sudden infusion of stasis blood occurs in the eye, it will inevitably lead to complications resembling wind-stroke.
This condition is called "red and painful eyes," where the eyelids swell like an inverted cup. This is due to excess pathogenic fire, with the Liver Wood being constrained, so Fire cannot generate properly; thus, Fire pathogen takes advantage of deficiency to become a scorching and drying disease. The black pupil must be painful, and the eyelids tight and hard. If the pain is caused by violent wind and guest heat, there will surely be abundant hot tears, and the pain in the black pupil will be slightly more relieved. Exogenous wind-heat is easy to treat; however, if internal attack by Wood-Fire occurs, the disease recedes slowly. In severe cases, stasis blocks the eye, blood infuses into the eyeball, and symptoms change unpredictably, requiring drainage/opening methods. Mild cases can be resolved with topical applications; severe cases require drainage. If topical application fails to resolve it, and it recurs after subsiding, or if drainage fails to eliminate it, and pain recurs extending to the head, with higher swelling and firmer eyelids, this indicates wind-heat transforming into toxicity. It is suitable to take the following prescription:
Sanre Xiaodu Yinzi (Heat-Dispersing and Toxin-Clearing Decoction)
Arctii Fructus (Niu Bang Zi, 牛蒡子) [crushed and stir-fried], Notopterygii Rhizoma et Radix (Qiang Huo), Coptidis Rhizoma (Huang Lian), Scutellariae Radix (Huang Qin), Menthae Haplocalycis Herba (Su Bo He), Saposhnikoviae Radix (Fang Feng), Forsythiae Fructus (Lian Qiao) [equal parts].
The above herbs are cut into pieces. Use two zhong of clear water, decoct until reduced to eight-tenths, remove dregs, and take after meals.
Jinsi Gao (Golden Thread Ointment) treats wind-heat attacking upwards, causing red, swollen, and painful eyes.
Coptidis Rhizoma (Huang Lian) [2 liang], Gentianae Radix (Long Dan Cao), Rheum Palmati Radix et Rhizoma (Da Huang), Phellodendri Cortex (Huang Bo) [peeled], Angelicae Sinensis Radix (Dang Gui), Gardeniae Fructus (Shan Zhi Ren) [1 liang each], Boswelliae Resina (Ru Xiang) [de-oiled and crushed], Borax (Peng Sha) [transparent type], Junci Medulla (Deng Xin) [2 qian 5 fen each], Young Bamboo Leaves (Qing Zhu Ye) [100 leaves], Jujubae Fructus (Da Zao) [20 fruits, pitted].
The above are soaked in five sheng of water, regardless of winter or summer, for one shichen (two hours), then removed. Slowly simmer in a silver or earthenware vessel. Do not allow vigorous boiling. Wait until the dregs are exhausted and juice is extracted, remove from fire, and let cool. Squeeze the juice through silk cloth in a dust-free place, clarify for one shichen, remove dregs, and simmer again in the vessel over low heat until reduced by half. Add half a jin of white honey and stir together. When lifting with hand, if threads form, stop heating. Let cool, then filter through a double-layered silk bag. Store in a porcelain box. Take about one tea spoonful each time, grind Borneolum Syntheticum (Long Nao) to a very fine powder (one zi amount), mix thoroughly with the ointment by grinding for one to two thousand times until uniform. Apply a small amount to the eye.
