Postpartum chills and fever alternating, occurring daily at regular intervals, with symptoms resembling malaria; however, one must absolutely not use herbal prescriptions for treating malaria. This is because deficiency of blood and qi causes chills and fever to arise, and weakness of primordial qi allows external pathogens to invade. Although when chills come, even boiling water or fire cannot warm the body, and when fever burns like charcoal, even ice water cannot cool it; sometimes symptoms are lighter during the day and heavier at night, or chills and fever appear in the late afternoon (Bing hour). Although the observed symptoms are similar to malaria, the treatment prescription must employ nourishing Ying (nutritive) blood and replenishing Qi to retreat the chills and fever. If sweating occurs, sweat should be stopped urgently, such as using formulas containing Mahuang Gen (Ephedrae Caulis Et Rhizoma); if only the head sweats while the body and feet do not, this is a critical condition of isolated Yang and severed Yin; one must never take only Renshen (Ginseng Radix et Rhizoma) while reducing blood-nourishing herbs, but should add preparations containing Dihuang (Rehmanniae Radix) and Chuan Gui (Angelicae Sinensis Radix). If headache occurs without sweating, it is appropriate to add Qianghuo (Notopterygii Rhizoma et Radix), Fangfeng (Saposhnikoviae Radix), Lianxu (Scallion Stamen), Congbai (Scallion White), and several Jujubae Fructus (Ziziphus Jujuba) to Shenghua Tang (Shenghua Decoction) to disperse the pathogen; one must strictly avoid treating it as malaria to prevent harming people. If food stagnation involves rice and noodles, add Shenqu (Massa Medicata Fermentata) 1 qian and Maiya (Hordei Fructus Germinatus) 5 fen; if meat stagnation occurs, add Shan Zha (Crataegi Fructus) and Sharen (Amomi Fructus Rotundus) each 5 fen; if food retention has lasted long and the Spleen and Stomach are extremely weak, so that even medicine cannot be transformed and transported, for external applications involving drugs, methods such as rubbing, roasting Qu (fermented grain), and scalding can be used. For meat stagnation caused by taking too many digestive-promoting drugs, resulting in fasting for several days, the following formula "Changsheng Huoming Dan" is suitable.
Changsheng Huoming Dan (Long Life and Vitality Pill): Treats conditions of anorexia/food refusal due to excessive intake of digestive-promoting drugs, various miscellaneous diseases, and food refusal caused by mistaken depletion. It can also treat these conditions.
Use Renshen (Ginseng Radix et Rhizoma) 3 qian, add 1.5 zhong of water, decoct until half a zhong remains. First, take one bowl of rice, grind the scorched rice crust (Guojiao) into powder, three spoonsful, and gradually add the Ginseng decoction. Guojiao guides and opens the Stomach Qi, naturally opening the appetite and strengthening the Spleen, promoting food intake.
Generally speaking, food and drink are flavors that fill deficiencies, and postpartum women rely on them for supplementation. Because fatigue and overwork injure the Spleen, making it unable to bear sweet drinks, one should advance with thin and bland flavors gradually, so that transformation and transportation are easier and faster. Additionally, assist with warming and tonifying agents, accompanied by Shenqu and Maiya to digest rice and noodle injuries, and Shan Zha and Sharen to resolve meat injuries; if injured by cold substances, Wuyu (Evodiae Fructus) and Guizhi (Cinnamomi Ramulus) should also be added. Thus, tonification and digestion are treated simultaneously, ensuring safety. Repeatedly, I have seen practitioners who do not emphasize the fundamental weakness of the postpartum state, knowing only how to promote the discharge of stagnant matter, which反而 (instead) damages True Qi and increases fullness and oppression. When one dose is ineffective, they add harsh drugs, causing those who already eat little and crave grains to become doubly deficient and cease eating entirely. Patients attribute this to fate, while physicians believe they have done their best to save them. What a pity!
