Postpartum hemorrhage involves profuse vaginal bleeding. One must carefully examine whether the blood color is red or purple, and observe the patient's physical constitution and qi deficiency or excess. If the blood is dark purple with clots, this indicates retained blood (stasis); it should be treated by promoting blood circulation. Using astringent hemostatic agents in such cases will instead cause pain; this condition cannot be discussed as metrorrhagia (uterine bleeding). If the blood is bright red and flows heavily, this results from fright damaging the Heart, causing it to fail to govern the blood vessels; anger damaging the Liver, causing it to fail to store blood; or overexertion damaging the Spleen, causing it to fail to control blood within the meridians. This should be treated as metrorrhagia. Initially, administer Shenghua Tang (Decoction for Generating New Blood) frequently for several doses. This allows for tonification amidst blood activation, thereby promoting vigorous blood generation. If there is collapse of form (prostration), sweating, or rapid breathing, use Shenghua Tang with doubled Ginseng to benefit qi. Thus, yang grows and yin generates, and blood naturally becomes vigorous. This cannot be stopped by simple hemostatic medicines like Zonghui (Charred Palm Fibre). If metrorrhagia occurs one month after delivery, Shengju Dabu Tang (Decoction for Lifting and Great Tonification) is appropriate. For elderly and weak individuals suffering from metrorrhagia, Shengju Dabu Tang is also suitable.
Shenghua Tang (Decoction for Generating New Blood): Treats postpartum metrorrhagia.
Chuanxiong Rhizome (1 qian) Angelicae Sinensis Radix (4 qian) Dried Ginger (0.4 fen, charred black) Licorice Root (0.5 fen, honey-fried) Schizonepeta Spike (0.5 fen) Peach Kernel (10 grains, skin and tip removed)
Decoct with jujube water. If the blood is bright red and flows heavily, add Schizonepeta Spike and Angelica Dahurica Root (each 0.5 fen); if there are blood clots and prostration, add Ginseng (1 qian); if there is excessive sweating and rapid breathing, add Ginseng (3-4 qian); if there is no sweating, no prostration, and no rapid breathing, simply taking Shenghua Tang will stabilize the blood. The world says: Chuanxiong and Danggui activate blood and cannot treat metrorrhagia; this is erroneous!
Zirong Yiqi Tang (Decoction for Nourishing and Benefiting Qi): Treats metrorrhagia to stop bleeding.
Chuanxiong Rhizome Ophiopogon Tuber Astragalus Membranaceus Root (each 1 qian) Ginseng Angelicae Sinensis Radix Rehmannia Glutinososa Root Prepared (Bai Zhu) Atractylodis Macrocephalae Rhizoma Tangerine Peel Glycyrrhizae Radix et Rhizoma Preparata (each 2 qian) Tangerine Peel Glycyrrhizae Radix et Rhizoma Preparata (each 0.4 fen) Angelicae Dahuricae Radix Schizonepetae Spica Cimicifugae Rhizoma (each 0.4 fen) Coptidis Rhizoma (0.3 fen, to clear heart fire) Jujube (3 pieces)
Decoct with water. If sweating increases, add Ephedrae Herba Root (Mahuang Gen, 1 qian) and Triticum Aestivum Fruit (Xiao Mai, 1 bunch); if constipation, add Cistanche Deserticola Stem (Rou Cong Rong, 1 qian); if qi stagnation, grind Costus Root (Mu Xiang, 0.1 fen); if phlegm, add Fritillaria Thunbergii Bulb (Bei Mu, 0.6 fen), Arundo Donax Juice (Zhu Li, a little), and Ginger Juice (Jiang Zhi, a little). If cold cough, add Apricot Seed (Xing Ren), Platycodon Root (Jie Geng), and Anemarrhena Rhizome (Zhi Mu, each 1 qian); if palpitations due to fright, add Sour Jujube Seed (Suan Zao Ren) and Biota Seed (Bai Zi Ren, each 1 qian).
