Angelicae Sinensis Radix (Washed with wine) Paeoniae Radix Alba (Stir-fried with wine) Ligusticum Chuanxiong Rhizome Rehmanniae Radix Preparata Chenpi Pinelliae Rhizoma (Soaked in ginger juice and stir-fried) Poria Cocos (Peeled) Aurantii Fructus Immaturus (Stir-fried with bran) Platycodonis Radix (Aerial part removed) Peucedani Radix Perillae Caulis Puerariae Radix Ginseng Radix et Rhizoma Aucklandiae Radix Glycyrrhizae Radix et Rhizoma
The above herbs are crushed into a powder. Add fresh ginger and jujube, decoct with water, and take orally.
A postpartum woman suffered from coughing with profuse phlegm. Her face was red, her mouth dry, she had internal heat, and fever worsened in the late afternoon, occurring at irregular intervals. This was due to Yin fire flaming upward; it was necessary to tonify the Spleen and Stomach. Thus, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) and Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) were used, and she recovered.
In one discussion regarding a postpartum woman who experienced bleeding after defecation which did not respond to various medicines, had poor appetite, and felt limb fatigue, this was identified as deficiency of Middle Qi. Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) was administered, with added Evodiae Fructus-stir-fried Coptidis Rhizoma (0.5 qian). After four doses, the bleeding stopped abruptly. However, palpitations, insomnia, and night sweats persisted. Guipi Tang (Restore the Spleen Decoction) was then used, and she recovered.
Regarding postpartum constipation caused by excessive blood loss leading to dryness of the Large Intestine, or dryness due to Blood deficiency and Fire燥 (dryness), the number of days and dietary intake should not be strictly counted. Medicines that promote bowel movement and moisten dryness should be used frequently. One must wait until abdominal distension is felt and there is an urge to defecate but inability to do so, indicating fecal impaction in the rectum. In such cases, porcine bile is suitable for moistening. If bitter-cold medicines are taken to promote bowel movement, they may damage the Yuan Qi (Original Qi) of the Middle Jiao, making defecation even more difficult, or causing unceasing diarrhea after passage, inevitably leading to a critical condition (Bai Zheng). If it belongs to Blood deficiency with Fire燥, Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) is used. For combined Qi and Blood deficiency, Bazhen Tang (Eight-Treasure Decoction) is used. It is strictly forbidden to use seeds like Hemp Seed (Huo Ma Ren), Apricot Seed (Xing Ren), or Aurantii Fructus Immaturus.
Regarding constipation before and after childbirth:
Angelicae Sinensis Radix (Washed with wine) Ligusticum Chuanxiong Rhizome Saposhnikoviae Radix (Aerial part removed) Aurantii Fructus Immaturus (Stir-fried with bran, each 1 qian) Glycyrrhizae Radix et Rhizoma (Roasted, 2 qian)
Crush the herbs. Decoct with fresh ginger and jujube and take orally. Avoid foods that stir up Wind. Using honey suppositories is also excellent.
Regarding postpartum constipation lasting five to seven days: Do not recklessly administer pill medicines. Use Coeli Globuli Germinatus (Sprouted Hordei Fructus) stir-fried and ground into powder. Take 3 qian per dose, mixed with boiling water. Administer between meals with porridge.
Regarding postpartum uterine injury causing continuous dripping of urine:
Ginseng Radix et Rhizoma (2.5 qian) Atractylodis Macrocephalae Rhizoma (Aerial part removed, 2 qian) Poria Cocos (Peeled, 1 qian) Astragali Membranacei Radix (Honey-roasted, 1.5 qian) Chenpi Peach Kernel (Pulp and tip removed, 1 qian) Glycyrrhizae Radix et Rhizoma (Roasted, 0.5 qian)
Crush one dose. Decoct with pig or sheep bladder in water, then add the herbs and decoct further before taking.
Regarding uterine enlargement in women, where a piece resembling pork liver falls out daily, followed by sallow complexion, physical fatigue, lack of taste in food, internal heat, evening fever, spontaneous sweating, and night sweats: Shiquan Dabu Tang (Ten-Agent Great Tonic Decoction) was used for over twenty doses. All symptoms resolved, and she subsequently regained fertility.
Regarding extreme pain in the vulva after childbirth: Soak Peach Kernel, remove skin and tip, grind into a paste, and apply topically. Healing occurs immediately.
Regarding extreme itching of the vulva after childbirth: Apply one liang of edible salt topically to stop the itching.
Regarding failure of the vulva to close after childbirth, accompanied by fever and aversion to cold: Shiquan Dabu Tang (Ten-Agent Great Tonic Decoction) with added Schisandrae Fructus was used for several doses, resolving the chills and fever. Then, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with added Schisandrae Fructus was used for several doses, resulting in closure. If swelling occurred initially after delivery, or if there was pain without closure, Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) was used. If swelling subsided but closure failed, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) was used. Cold-natured agents must be strictly avoided. For non-closure of the vulva, decoct calcined alum (Duanshi) for fumigation and washing. Another formula uses Schizonepetae Spica, Pogostemonis Herba, and Ailanthi Cortex, decocted for fumigation and washing.
Regarding prolapse of the rectum after childbirth, caused by Qi deficiency and Blood weakness leading to hanging down: Nourishing Qi and nourishing Blood will cause the rectum to retract naturally.
Ginseng Radix et Rhizoma Atractylodis Macrocephalae Rhizoma (Aerial part removed and stir-fried) Astragali Membranacei Radix (Honey-roasted) Angelicae Sinensis Radix Ligusticum Chuanxiong Rhizome Glycyrrhizae Radix et Rhizoma (Roasted)
Crush the herbs, decoct with water, and take orally. If prolapsed for two days, add Cimicifugae Rhizoma (0.5 qian) to lift and raise. Daily warming of the lower back and abdomen with hot palms helps; when the area feels warm, the rectum retracts naturally.
Another treatment for postpartum rectal prolapse: Grind Castor Seed (Ricini Semen) peeled into a paste and apply to the center of the crown of the head to induce retraction. Internally, administer Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with doubled Cimicifugae Rhizoma, removing Cimicifugae Rhizoma (Note: Source says remove Chaihu/Saposhnikoviae, keep Shengma/Cimicifuga), and adding Motherwort Herba (1 qian).
Regarding postpartum condition where fetal death leads to cessation of menstruation, sudden lower abdominal pain, and a stone-hard object blocking the vagina causing unbearable pain: This is Shi Jia (Stone Mass).
Angelicae Sinensis Radix (Washed with wine) Ligusticum Chuanxiong Rhizome Paeoniae Radix Alba (Stir-fried with wine) Rehmanniae Radix Fresh Peach Kernel (Pulp and tip removed) Carthami Flos Rheomi Radix et Rhizoma Sparganii Rhizoma Arecae Semen Alismatis Rhizoma Cyperi Rhizoma Corydalis Rhizoma Dragon's Blood
Decoct with water and take on an empty stomach.
Regarding postpartum illness where Yu Mao (depression and dizziness) causes excessive sweating, leading to constipation, making medication difficult: This remedy is best.
Er Zi Yin (Two-Seeds Decoction)
Perillae Fructus Cannabis Sativa Seed (Shelled)
Take half he of each, clean thoroughly, grind extremely finely, grind again with water, extract one cup of juice, and divide into three portions to cook with porridge. This porridge is suitable not only for postpartum women but also generally effective for elderly patients with various deficiencies and wind-induced constipation.
Regarding postpartum vulvar swelling: Wash with decoction of Evodiae Fructus.
Case of postpartime aversion to cold and fever: I intended to treat with Bazhen Tang (Eight-Treasure Decoction) plus roasted Ginger. The family, knowledgeable in medicine, believed it was Wind-Cold and used Xiaochaihu Tang (Minor Bupleurum Decoction). I stated that irregular chills and fever indicated Qi and Blood deficiency, but they did not believe me. After one dose, they experienced incessant sweating, delirious speech, irritability, thirst, and limb convulsions. I then administered two doses of Shiquan Dabu Tang (Ten-Agent Great Tonic Decoction), which worsened the condition. The pulse was surging and large but empty upon heavy pressure. I reverted to the previous decoction plus Aconiti Lateralis Radix Preparata. After four doses, stability was achieved.
Case of a postpartum woman suffering from dysentery for over a year, emaciated to the point of being skeletal, with internal heat, worsening evening fever, spontaneous and night sweats, oral ulcers, expectoration of approximately three bowls of phlegm, and a surging pulse that vanishes upon heavy pressure: This indicates decline of Mingmen Fire (Gate of Life Fire) and deficiency-cold of the Spleen Earth manifesting as false heat. The phlegm arises because the Spleen is deficient and cannot control the return to origin. Therefore, Bawei Wan (Eight-Ingredient Pill) was used to tonify Fire to generate Earth, and Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) was used to simultaneously tonify Lung Metal and strengthen the Spleen and Stomach.
