Postpartum management focuses on supporting deficiency (Fuxu) and eliminating blood stasis (Xiaoyuxue). The pulse should ideally be deficient. Shuhe stated: A postpartum pulse that is slow and slippery is auspicious, whereas one that is excess-large, wiry, and rapid indicates death. A Cun Kou pulse that is choppy and rapid in both directions and irregular indicates death; a deep and fine pulse that remains palpable even when pressed to the bone indicates survival. After delivery, regardless of whether there is abdominal pain or not, and regardless of the presence or absence of disease, mix one cup of child's urine with wine, warm it, and take it orally. This prevents all diseases. For a short time after birth, sit or lie on the bed with the back propped up high, knees bent, lying supine. Wake the mother frequently, apply vinegar to the nose, or burn vinegar into charcoal for fumigation, or burn lacquerware for fumigation. Additionally, massage from the heart region down to below the umbilicus to prevent stagnation of lochia. Maintain this for three days to prevent syncope due to blood loss (Xueyun) or counterflow of blood (Xueni). Although wine promotes blood circulation, it should not be consumed in excess, as it may draw blood into the limbs and cause dizziness. It is advisable to consume small amounts of white rice porridge frequently. After one month, small amounts of pig trotters can be eaten. One must also be cautious with speech, emotions (Seven Emotions), cold, and heat. Combing hair and washing feet should continue for one hundred days.
If the constitution was previously weak in Qi and Blood, regardless of the number of days postpartum, otherwise, one may suffer from soreness and pain in the hands, feet, waist, and legs, a condition named "Chorea Laboris" (Rulao), which is difficult to treat. Immediately after childbirth, one must not become angry due to love or hatred, as these can cause disease. One should not sleep alone, lest it lead to fright from deficiency. One should not scrape the tongue coating, lest it damage Heart Qi. One should not brush teeth, lest it cause counterflow of blood. Only when Qi and Blood have stabilized should daily affairs be attended to. Violating these precautions initially seems insignificant like an autumn hair, but once illness sets in, it becomes heavy as a mountain. Can one not be vigilant?
There are three causes of postpartum syncope due to blood loss (Xueyun): fainting due to excessive mental or physical exertion, fainting due to excessive bleeding, and fainting due to insufficient bleeding. Although the symptom of fainting is the same, the treatments differ. If fainting is caused by excessive bleeding, tonify Blood using Xiong Gui Tang (Safflower and Angelica Decoction) as the main formula. If lochia does not stop, double the dose of charred Gan Jiang (Dried Ginger) to stop bleeding. If fainting is caused by insufficient bleeding, use Hei Shen San (Black Divine Powder) as the main formula. In cases of syncope with unconsciousness due to blood loss, place hot charcoal next to the mother and pour vinegar on it so that the vinegar fumes enter the mouth and nose. Those with mild symptoms will wake up, and those with severe symptoms will regain consciousness.
One method: Finely chop Chinese chives, place them in a spouted bottle, pour hot vinegar over them, seal the bottle quickly, and insert the spout into the mother's nostrils. Inhalation leads to immediate awakening.
Another method: Fumigate with smoke from burning old lacquerware; inhalation leads to immediate awakening.
Another method: Grind Jing Jie Sui (Schizonepeta Flower/Tip) into powder, blow one fen into the nose; immediate awakening occurs.
Another method: Burn Lu Jiao (Deer Horn) until the exterior is charred but the interior retains its properties (Cunxing). Take one qian, administer with wine; immediate awakening occurs.
Postpartum fever may arise from excessive blood loss, excessive labor injury, incomplete lochia discharge, dietary irregularities, contraction of Wind-Cold, Cold-Heat complex with food retention, internal injury with external pathogen invasion, or milk engorgement on the third day. All can cause fever with aversion to cold, body ache, and abdominal pain. Do not categorize them together and prescribe indiscriminately.
First discussion: Fever due to excessive blood loss presents with a deficient, large, and forceless pulse, without internal pain. This is not an Excess Heat, but Internal Heat generated by Yin Deficiency. Treat with Gui Zhu Bao Chan Tang (Angelica and Atractylodes Postpartum Protection Decoction).
Second discussion: Fever due to labor injury or rising early to work is also treated with Gui Zhu Bao Chan Tang.
Third discussion: Fever and aversion to cold due to incomplete lochia discharge are accompanied by distension and fullness in the hypochondrium radiating to the lower abdomen with mass-like pain. Treat with Hei Shen San (Black Divine Powder).
Fourth discussion: Spleen-Stomach weakness leading to indigestion and stagnation fever presents with belching, acid regurgitation, aversion to food odors, tastelessness, chest and diaphragm fullness, and a tight, vigorous Qi Kou pulse, along with fever, aversion to cold, and headache. Treat with Li Pi Tang (Regulate Spleen Decoction).
Fifth discussion: Fever due to Wind-Cold invasion presents with a wiry and tight pulse, possibly unsmooth lochia, headache, body ache, fever with aversion to cold, cold extremities, and abdominal pain. Treat with Wu Ji San (Five Accumulations Powder). This formula expels stale blood, generates new blood, harmonizes Ying and Wei, nourishes Zang-Fu, balances Yin and Yang, and prevents pathogenic interference, thus resolving cold and heat issues. Add a little rice vinegar to decoct. Remove Ma Huang (Ephedra) from this formula. (Formula see Zhong Han chapter).
Sixth discussion: Cold-Heat complex with food retention presents with food aversion, chest stuffiness, abdominal pain, headache, fever, and a tight, vigorous Qi Kou pulse. Treat with Xing Qi Xiang Su San (Move Qi and Perilla Powder) to resolve food and induce sweating. (Formula see Nei Shang chapter).
Seventh discussion: Internal injury to Yuan Qi with external Wind-Cold invasion presents with a surging, large yet deficient pulse, fever with irritability, headache, aversion to cold, thirst, spontaneous sweating, and panting. Treat with Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) with modifications. (Formula see Nei Shang chapter).
Eighth discussion: Fever and aversion to cold due to milk engorgement present with swollen, hard, and painful breasts. Massage the breasts to promote milk flow; the fever will subside spontaneously without medication.
Ninth discussion: Postpartum fever with aversion to cold, or facial paralysis (mouth and eye deviation), indicates extreme deficiency of Qi and Blood. Prioritize major tonification of Qi and Blood. If the Left Hand pulse is deficient, use more Blood-tonifying herbs than Qi-tonifying ones. If the Right Hand pulse is deficient, use more Qi-tonifying herbs than Blood-tonifying ones.
Tenth discussion: Postpartum stroke should not be immediately treated as typical stroke, nor should Xiao Xu Ming Tang (Minor Continuation of Life Decoction) be used. Prioritize major tonification of Qi and Blood. If the mouth is locked (Jin), it is due to Blood deficiency allowing Wind to invade the jaw muscles, causing spasms. If opisthotonos (angular rigidity) occurs, it is due to constitutional weakness allowing Wind to invade the Yang meridians, causing only the back and waist to spasm like a drawn bow. Use Gui Zhu Bao Chan Tang removing Xiang Fu and Gan Jiang, adding Qin Jiao and Qiang Huo. Alternatively, use equal parts Dang Gui and Jing Jie, decocted in water with a little wine. If the mouth is locked, pry it open gently and administer. Effect is seen upon swallowing. Alternatively, stir-fry Jing Jie slightly, grind to powder, take 3 qian with black bean wine infusion or child's urine.
Eleventh discussion: Various debility conditions postpartum are best treated with Shi Quan Da Bu Tang (Comprehensive Great Tonifying Decoction). This formula is warm in nature, gently tonifies Blood and Qi, strengthens the Spleen and Stomach, and is the premier formula for various deficiencies and damages.
Twelfth discussion: Postpartum syncope due to excessive bleeding involves dizziness. Unconsciousness results from massive loss of Qi and Blood rendering Shen (Spirit) inactive. Use Ren Shen (Ginseng) cut into two doses, decocted in water and taken warm. Ren Shen is sweet and warm, tonifies Yuan Qi, restores Qi, and generates Blood. If there is body heat and rapid breathing, add one cup of child's urine. If there is body cold and weakness, add Fu Zi (Aconite) 3 qian. Externally, use charcoal fire with vinegar fumes to enter the nose for astringency; this is highly effective.
Thirteenth discussion: Various postpartum diseases should be treated by addressing the root, prioritizing major tonification of Qi and Blood. This formula treats all conditions involving Qi and Blood deficiency, weak Spleen-Stomach, stagnant lochia, excessive bleeding, dietary irregularities, or anger-induced counterflow, leading to fever, aversion to cold, spontaneous sweating, dry mouth, irritability, panting, heart and abdominal pain, dizziness, blurred vision, tinnitus, muteness, confusion, and unconsciousness.
Gui Zhu Bao Chan Tang (Angelica and Atractylodes Postpartum Protection Decoction)
Dang Gui (Washed in wine 1.5 qian) Chuan Xiong (1 qian) Bai Shao (Washed in wine 1 qian) Shu Di Huang (Steamed in wine 1 qian) Bai Zhu (De-rhizome, fried 1 qian) Gan Cao (Honey-fried 0.3 qian) Bai Fu Ling (Peeled 1 qian) Chen Pi (0.8 qian) Gan Jiang (Charred 0.8 qian) Xiang Fu Mi (Stir-fried in child's urine 1 qian) For Qi deficiency, add Ren Shen 0.7 qian.
Decoct one dose with 3 slices of Sheng Jiang (Fresh Ginger) and one Jujube. Take warm. For excessive bleeding, double Chuan Xiong, Dang Gui, and Gan Jiang. For chest/diaphragm fullness, add Zhi Shi (bran-fried), Sha Ren, Hou Po (ginger-juice fried), Shan Zha Rou. For hypochondriac pain, add Qing Pi and Rou Gui. For intermittent lower abdominal pain, add Yuan Hu Suo, Tao Ren, Hong Hua, Su Mu. For severe cases, add San Leng and E Zhu, all ginger-roasted and vinegar-fried. For sweating, add Huang Qi (honey-fried) and Suan Zao Ren (fried). For dry/bitter mouth, add Mai Men Dong (de-heart). For no fever but unbearable lower abdominal pain, use Tao Ren (peeled, pounded 5 qian) with Chinese chive juice and wine; effect is immediate. For stagnant lochia, add Yi Mu Cao, Mu Dan Pi, Tao Ren, taken with child's urine and wine. For phlegm vomiting, add Ban Xia, Bei Mu. For persistent cough, add Bei Wu Wei Zi, Sang Bai Pi. For anger, add Wu Yao. For confusion, mouth locking, and muteness, add Jing Jie Sui.
Fourteenth discussion: Eighteen postpartum syndromes in women; this formula works miraculously.
Hei Shen San (Black Divine Powder)
Zong Pi Hui (Ash of Palm Fruit Skin) Yuan Hu Suo Dang Gui (Washed in wine) Chi Shao Bai Shao Sheng Di Huang Wu Ling Zhi (Each 1 qian) Pu Huang (1 liang) Shu Di Huang (1 liang) Xiang Fu Mi (Fried 1 liang) Gan Jiang (Pao 1 liang) Chen Xiang (0.5 liang) Ru Xiang (0.5 liang) Da Hei Dou (0.5 liang) E Zhu (0.5 liang) Hong Hua (0.5 liang)
Grind to fine powder. Take 2 qian each time, mixed with warm wine and child's urine.
Indications: Retained placenta with stale blood attacking the Heart, causing severe dizziness; ingestion awakens the patient. Excessive bleeding during labor where residual blood wraps the placenta making expulsion difficult; ingestion saves the mother at the expense of the fetus. Premature exertion during labor causing malpresentation; ingestion rescues the mother. Fetal death in utero causing cold/painful limbs, foaming at the mouth, cyanotic nails; ingestion expels the fetus. Incomplete lochia neglected, combined with excessive sour/salty diet leading to metrorrhagia. Blood syncope causing blurred vision and inability to sit. Blood confusing the Heart orifice causing muteness. Stale blood flowing into limbs causing edema. Stale blood causing thirst, dry tongue, alternating chills and fever resembling malaria. Stale blood entering the Heart causing irritability, mania, confused speech, or hallucinations resembling epilepsy. Stale blood lingering in joints causing generalized pain. Drinking cold beverages within a month causing stale blood coagulation, unbearable abdominal pain, or dysentery. Evening fever with blackness in nostrils. Stale blood accumulation causing urinary retention and constipation. Stale blood rushing to the Heart causing panting. Stale blood stagnating in Spleen/Stomach causing abdominal distension and vomiting resembling inverted stomach. Various strange postpartum syndromes indescribable are mostly caused by stale blood; ingestion provides immediate relief. True Immortal Formula. Postpartum heart/abdominal pain due to non-circulating blood stasis or "Child Pillow" pain (afterbirth pain); critical condition.
