Bian Du (Sores in the Groin), also known as Ma Yi Yong (Horse-riding Carbuncle), is a disease of the Chong Mai (Penetrating Vessel) and Ren Mai (Conception Vessel). The carbuncle manifests in the area governed by the Jueyin Meridian (Liver Meridian of Foot-Jueyin). This meridian is rich in blood, hence it is also called Xue Shan (Blood Hernia). It may develop after the occurrence of Gan Chuang (Chancre) or suddenly present as a painful nodule. All such cases are caused by the stagnation of heat and the accumulation of blood. In the initial stage, treatment should focus on promoting circulation and purging to resolve the swelling. If pus has formed, standard formulas for supporting the healthy qi and tonifying the interior should be used.
One discussion on Bian Du attributes it to damp-heat in the Jueyin Meridian, triggered by fatigue and overexertion. Treatment involves decocting 3 cun of Shé Gān (Belamcandae Rhizoma) with fresh ginger and administering it before meals. Relief is immediate upon having two or three bowel movements. The Shé Gān used should be the variety that bears purple flowers.
Another highly effective formula for Bian Du:
Zhui Du San (Pursuing Toxin Powder)
Dang Gui Wei (Tail of Angelicae Sinensis Radix), Chi Shao (Red Peony Root), Bai Zhi (Angelicae Dahuricae Radix), Jin Yin Hua (Lonicerae Japonicae Flos), Tian Hua Fen (Trichosanthis Radix) (each 1 qian); Bai Jiang Can (Bombyx Batryticatus) (6 pieces, stir-fried); Mu Bie Zi (Strychni Semen); Chuan Shan Jia (Pangolin Scales) (2 slices); Da Huang (Rhei Radix et Rhizoma) (3 qian); Mang Xiao (Natrii Sulfas) (2 qian). An alternative version suggests adding Wu Ling Zhi (Trogopterori Faeces) for better efficacy.
Grind these ingredients into one dose. Decoct in good-quality rice wine, leave exposed overnight, and take warm at dawn (Wu Geng). Cover thickly to induce sweating. Recovery occurs after one or two bowel movements. Add Mang Xiao and Da Huang only when the other herbs are nearly fully decocted, and boil twice more. Another variant adds She Gan and omits Mang Xiao.
A formula for Yu Kou Bian Du (Fish-mouth Sores in the Groin):
Shen Yi San (Miraculous Difference Powder)
Jin Yin Hua (Lonicerae Japonicae Flos), Tian Hua Fen (Trichosanthis Radix), Mu Bie Zi (Strychni Semen) (each 2 qian); Gan Cao (Licorice Root) (0.3 qian); Lian Qiao (Forsythiae Fructus), Huang Qin (Scutellariae Radix) (each 0.8 qian); Shan Zhi Zi (Gardeniae Fructus) (0.7 qian); Chuan Shan Jia (Pangolin Scales) (2 slices, roasted); Zao Jiao Ci (Gleditsiae Spina); Mu Xiang (Aucklandiae Radix) (0.5 qian); Da Huang (Rhei Radix et Rhizoma) (3 qian).
Grind into one dose. Decoct in a mixture of wine and water. Take on an empty stomach.
A miraculous formula for treating swollen and painful Bian Du, transmitted by Xu Wanyu:
Da Huang (Rhei Radix et Rhizoma), Quan Xie (Scorpio), Chan Tui (Cicada Slough), Jiang Can (Bombyx Batryticatus), Chuan Shan Jia (Pangolin Scales) (stir-fried in earth until pearl-like), Bai Zhi Shao (Tip of Angelicae Dahuricae Radix), Bei Mu (Fritillariae Bulbus), Dang Gui Wei (Tail of Angelicae Sinensis Radix) (each 2 qian).
Combine into one dose. Decoct in water, then add Da Huang and boil twice more. Strain, and take with good raw rice wine. If the swelling does not dissipate, add one Zhu Gong (Centipede) and decoct together.
A formula for treating Yu Kou Chuang (Fish-mouth Chancre):
Use one pig gallbladder, mix with one bowl of hot rice wine, and take warm. This will cause the lesion to resolve internally.
A formula for treating Yu Kou Bian Du:
Grind the above ingredients into powder. Mix with yellow rice wine and swallow.
