Pulse that is deep, fine, and slow indicates a condition that is easy to treat. A pulse that is floating, large, wiry, and long indicates a condition that is difficult to treat.
Stomach epigastrium pain (Wei Wan Tong) is mostly caused by indulging in food and drink, favoring pungent, spicy, sour foods, consuming large amounts of hot wine or fried and roasted foods, and then eating cold and raw foods. Injury occurs in the morning and worsens in the evening; day by day, month by month, it accumulates. Self-stagnation leads to accumulation (Ji), and accumulation leads to phlegm-dampness (Tan). Phlegm-fire simmering causes blood to move recklessly. Phlegm and blood mix together, obstructing the ascending and descending of Qi mechanism. Thus, stomach epigastrium pain, acid regurgitation, belching, gastric discomfort with nausea occur. These are all precursors to Ge Ye disease (Dysphagia/Obstruction) and Fan Wei disease (Regurgitation/Vomiting). Quack doctors treat this with drying and heating medicines, using fire to assist fire, which is erroneous. Ancient formulas describe nine types of Heart Pain (Xin Tong): Yin (Fluid), Shi (Food), Feng (Wind), Re (Heat), Leng (Cold), Ji (Palpitation), Chong (Parasite), Zhu (Infection/Transmission), and Lai Lai Tong (Wandering Pain). Regarding so-called Cold Heart Pain, can it be treated uniformly with heating medicines? One must distinguish between new and chronic conditions. If it is known that the body was exposed to cold pathogenic factors, or illness began after ingesting cold substances at the onset, warming and dispersing or warming and promoting drainage medicines should be used. If the disease has been present for a long time, stagnation forms; stagnation generates heat. It is appropriate to use Stir-fried Gardenia Fruit (Shan Zhi) as the sovereign herb, with heating herbs as guides, so that pathogens are easily subdued and the disease recedes. After recovery, if dietary indulgence is not corrected, the disease will recur, making it difficult to treat. Although this disease may persist for a long time without eating, it is not fatal. One must wait for several doses of medicine to stop the pain, rest for a day, and gradually resume small amounts of food to achieve recovery. In cases of True Heart Pain (Zhen Xin Tong), severe cold invades the Heart君主 (Heart Monarch), or turbid blood rushes to the Heart. If hands and feet turn cyanotic past the joints, death occurs in the evening if onset is in the morning, or in the morning if onset is in the evening; this cannot be treated with medicine.
Discussion: For stomach epigastrium accumulation with stagnant heat, causing unbearable stabbing pain, this formula is the main prescription for treating Heart and Stomach pain.

For excessive heat-toxin from wine and wheat products, or Phlegm-Fluid causing Stomach pain, administer with Persimmi Calyx (Shi Di) decoction. Among all Qi pains, Hypochondriac pain caused by Bladder Qi is the most difficult to treat; only this medicine can eliminate it. There was a patient with neck tendon pain. All doctors treated it as Wind. After months without improvement, it flowed into the back spine. Later, it injected into the right hypochondrium, causing severe spasmodic pain. Taking one dose of this combined remedy cured him; a second dose eliminated the root cause.

Finish one dose, and it will not recur for life. Treats Heart pain radiating to the back like magic.
Discussion: Heart and Abdominal pain, always accompanied by chest fullness, vomiting, cold limbs, red face, numb lips, dry throat and mouth, alternating chills and fever, and large pulse. This is due to repeated use of cooling medicines damaging True Yang, resulting in True Cold inside and False Heat outside. The pulse is wiry and large with abnormal characteristics, indicating Spleen Qi deficiency with Liver Wood overacting. One should warm and tonify the Stomach. Use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) (See Internal Injury). Add Pinelliae Rhizoma Preparatum (Ban Xia), Poria (Fu Ling), and Evodiae Fructus (Wu Zhu Yu) to this formula.
A woman suffered severe pain under the heart in the Stomach Epigastrium. Right Cun and Guan pulses were absent. Left pulse was faint and nearly imperceptible. Hands and feet were cold. The disease was severe. Observation showed dark cyan color around the eyelids. This is Spleen deficiency with Liver Wood overacting.
