Infants and children may experience constipation or complete obstruction of defecation. This is caused by heat in the fu organs invading the stomach and intestines. Heat in the stomach consumes body fluids (Jinye), leading to dry, hard, and compacted feces that are difficult to pass. This condition is termed "constipation" (bise). If severe enough to result in total inability to defecate, it is termed "obstruction" (butong).
Similarly, dribbling urination or complete urinary retention is caused by heat in the small intestine and bladder. Both meridians govern water metabolism; water enters the small intestine, transmits to the bladder, and exits via the water pathways as urine. When heat pathogen invades, it depletes fluids, causing sluggish urine flow resulting in dribbling. In severe cases, the water pathways become dry, leading to urinary retention. If heat enters both the large and small intestines, it causes stagnation and prevents the free flow and discharge, resulting in simultaneous obstruction of both stool and urine.
