Occasional sores and swellings on the tongue of infants are caused by wind-toxin and pathogenic heat clashing with blood and qi, lodging in areas of deficiency to induce disease. When purple swelling occurs on the tongue, it is named "Purple Tongue Distension" (Zi She Zhang). If the swelling is coarse, large, numb, stifling, and hard, it is named "Wooden Tongue Distension" (Mu She Zhang). If sores arise from the clash of wind-dampness, it is named "Tongue Sores" (She Chuang). If excessive wind causes the tongue to swell like a blown blister, it is named "Drum Tongue" (Gu She). Severe cases of Purple, Wooden, or Drum Tongue distension may overflow the entire mouth, blocking the air gate (vital opening), all of which can be fatal.
To treat Purple Tongue Distension: Administer sheep milk for drinking.
Another prescription: Treat as above. Take fresh crucian carp, slice the meat thinly, and frequently apply slices to the tongue, changing them often.
To treat sores on the tongue preventing nursing: Grind alum into powder, mix with an egg yolk, place in vinegar, and apply to the sole of the child's foot. It will heal within seven days.
Another prescription: Treat as above. Apply Puhuang (Corydalis Rhizome) powder externally.
Another prescription: Apply juice from Mulberry Root Bark (Sangbaipi) to the nipple/breast for the infant to suckle.
Another prescription: Treat as above. Finely grind Hufen (Lead Carbonate), mix with marrow from pig femur bones, and apply to the lesion three times daily.
To treat Wooden Tongue distension and hardness spreading throughout the mouth, where qi blockage leads to death: Mix equal parts of Fu Di Mei (soot from the bottom of a cauldron) and salt. Blend with wine and apply internally and externally to the tongue; relief comes quickly. Wine is optional.
Another prescription: Treat as above.
Chuan Puxiao (Natrii Sulfas), Zhen Zixue (True Purple Snow), Bai Yan (White Salt).
Mix and grind these three ingredients uniformly to a fine powder. Use half a qian each time, add 3 liang of Zhuli (Bamboo Sap), drop in a little white boiled water, mix well, and apply to the tongue. Swallowing saliva is permissible.
In cases of sudden tongue swelling like a blown blister, overflowing the mouth, with blocked qi and no other remedies available: Take ten Banxia (Pinellia Tuber), wash thoroughly with hot water to remove slipperiness. Boil in Ku Jiu (bitter wine/vinegar) one sheng until reduced to five ge. Gargle slowly, do not swallow saliva. If irritating the throat, adding one liang of Shengjiang (Fresh Ginger) for boiling is also acceptable.
Another prescription: Decoct concentrated Licorice Root (Gancao) juice and gargle.
Another prescription: Gargle with sugar-vinegar mixture; this is effective.
Another prescription: For Purple, Wooden, or Drum Tongue where swelling blocks qi and other medicines fail, use this rescue method. On both sides of the tongue, scrape open with fingers or incise with a Pi Dao (scalpel) to drain toxic blood. Be careful not to damage the central vein under the tongue. If bleeding does not stop and amounts to several sheng, cauterize the sores several times with a red-hot iron comb to arrest bleeding. The heating of the iron comb must be moderate, not excessive.
