Various febrile conditions at their onset have different etiologies. Fevers that occur at specific times are termed tidal fever (Chao Re). Fevers that come and go intermittently belong to deficiency syndromes (Xu Zheng). Fevers that arise without chills are classified as bone-steaming heat (Gu Zheng).
Regarding fetal heat (Tai Re), it manifests within three days to one month after birth, characterized by eyes closed and red, swollen eyelids, frequent groaning, incessant crying, occasional fright and irritability, generalized high fever, and yellow urine. This is caused when the mother, during pregnancy, contracts seasonal heat-toxins, mistakenly takes warm-tonifying medicines, or overconsumes the "five pungents" (Wu Xin: garlic, chives, leeks, wild onions, and asafoetida), leading to internal heat accumulation that steams and affects the fetal qi. Consequently, these symptoms appear after birth, named fetal heat. If left untreated for a long time, it may develop into thrush (E Kou), double tongue (Zhong She), wooden tongue (Mu She), or red-purple carbuncles (Chi Zi Dan Liu). It must not be treated with harsh cold-purging medicines, as clearing the heat may lead to cold pathogenesis and transmission to other syndromes; caution is essential.
Treatment methods: Push San Guan (Three Passages) point, Retire Liu Fu (Six Bowels) point, Clear San Jiao (Triple Burner), Divide Yin and Yang, Clear Tian He Shui (Heavenly River Water), Rub Wai Lao Gong (Outer Labor Palace) point, Move Ba Gua (Eight Trigrams) [from Kun to Kan position, perform twice more], Dig Shen Shui (Kidney Water) point, Pinch Wu Zong (Five Summits) lines, Pinch Shi Wang (Ten Kings) points, Move Zhou (Elbow) area, Water Fishing for Bright Moon technique, apply fire-moxa once to Qu Chi (Pool at the Elbow) point on the Tiger's Mouth.
Tidal fever (Chao Re) involves alternating fever and defervescence, recurring daily at fixed times like the tide, hence the name. Generally, this results from abundant and stagnant Qi and Blood, with Heat generated in the Five Zang organs due to fright, steaming outward. It may also involve latent heat (Fu Re) or retained cold (Su Han). Latent heat presents with yellow, foul-smelling stool; retained cold presents with white, sour-smelling stool.
Treatment methods: Push San Guan point, Tonify Xin Jing (Heart Meridian), Move Ba Gua, Divide Yin and Yang, Drain Wu Jing (Five Meridians) points, Dig Shi Wang points, Dig Zhong Zhi (Middle Finger), Retire Liu Fu point, Water Fishing for Bright Moon technique, Move Zhou area.
Fright-heat (Jing Re) occurs when the infant is startled by strange objects seen or metallic sounds heard, causing fright and fear. The Heart is affected by fright, leading to Qi stagnation, slight fever, restless sleep with frightening dreams, shiny complexion with sweating, rapid pulse, and irritability. Treatment should follow the same method as acute convulsion syndrome (Ji Jing Feng).
Treatment methods: Push San Guan point, Clear Fei Jing (Lung Meridian), Divide Yin and Yang, Tui Men Shan (Push Door Gate) point, Clear Xin Jing, Clear Tian He Shui, Clear Wu Jing points, Dig Zong Jin (General Tendon) line, Move Zhou area, Water Fishing for Bright Moon technique, Fei Jing Zou Qi (Flying Meridian Moving Qi) technique.
Wind-heat (Feng Re) presents with body heat, bluish-green facial color, hot sensation in the mouth, and intermittent irritability and calling. It is suitable to disperse wind and resolve heat. If the heat is severe accompanied by constipation, purgation is appropriate.
Treatment methods: Push San Guan point, Drain Da Chang Jing (Large Intestine Meridian), Dig Xin Jing, Drain Shen Shui point, Move Ba Gua, Dig Zong Jin line, Clear Tian He Shui, Er Long Xi Zhu (Two Dragons Playing with Pearl) technique, Move Zhou area.
Irritable heat (Fan Re) arises from excessive Qi and Blood, excess heat in the Zang-Fu organs, affecting both interior and exterior, causing restlessness and high skin temperature. Severe heat in the palms and soles is termed "Five Hearts Irritability" (Wu Xin Fan Re).
Treatment methods: Push San Guan point, Dig Zhong Zhi, Drain Wu Jing points, Dig Shi Wang points, Move Ba Gua, Rub Wai Lao Gong point, Divide Yin and Yang, Retire Liu Fu point, Water Fishing for Bright Moon technique, Da Ma Guo Tian He Shui (Horse Crossing Heavenly River Water) technique, Move Zhou area.
Spleen heat (Pi Re) involves slight retraction of the tongue tendons and frequent tongue protrusion. This is due to accumulated heat in the Spleen; cool-cold medicines should not be used arbitrarily.
Treatment methods: Push San Guan point, Tonify Pi Tu (Spleen Earth) point, Drain Xin Huo (Heart Fire) point, Tonify Shen Shui point, Move Ba Gua, Divide Yin and Yang, Dig Zong Jin line, Push Shang San Guan (Upper Three Passages) 24 times, Tui Xia Liu Fu (Lower Six Bowels) 80 times, Water Fishing for Bright Moon technique, Move Zhou area.
Deficiency heat (Xu Re) occurs post-illness when Qi and Blood are unsettled, limbs are emaciated, and fever occurs frequently (3-5 times a day). This is exogenous heat taking advantage of deficiency. It is suitable to regulate Qi and tonify deficiency, after which the heat will subside naturally.
Treatment methods: Push San Guan point, Tonify Wu Jing points, Nian Wu Zhi (Twist Five Fingers), Move Ba Gua, Water Fishing for Bright Moon technique, Dig Zong Jin line, Push Shang San Guan 24 times, Tui Xia Liu Fu 80 times, Divide Yin and Yang, Fei Jing Zou Qi technique, Move Zhou area.
Excess heat (Shi Re) presents with dizziness, flushed cheeks, oral heat, dark and scanty urination, constipation, and abdominal distension. This is an excess heat syndrome. Purging is indicated to drain the heat from the Zang-Fu organs, bringing relief.
Treatment methods: Push San Guan point, Drain Wu Jing points, Push Da Chang Jing, Clear Shen Shui point, Move Ba Gua, Push Pang Guang Jing (Bladder Meridian), Divide Yin and Yang, Water Fishing for Bright Moon technique, Retire Liu Fu point, Da Ma Guo Tian He Shui technique, Fei Jing Zou Qi technique, Move Zhou area.
Accumulated heat (Ji Re) manifests with swollen eyelids, sallow complexion, cold feet, fever intensifying from head to abdomen, aversion to food odors, vomiting, nausea, and abdominal pain.
Treatment methods: Push San Guan point, Move Wu Jing points, Tonify Pi Tu point, Push Da Chang Jing, Clear Xin Jing, Clear San Jiao, Tonify Shen Shui point, Move Ba Gua, Dig Zong Jin line, Divide Yin and Yang, Water Fishing for Bright Moon technique, Retire Liu Fu point, Fei Jing Zou Qi technique, Rub Zhou area.
Gan heat (Gan Re) results from overeating, causing food stagnation in the middle jiao, which郁 (depresses) and transforms into heat. If spleen stagnation is untreated, it transmits to other organs, forming the five types of Gan disease. Once the spleen pathology resolves, the other organs will be at peace.
Treatment methods: Push San Guan point, Tonify Pi Tu point, Push Da and Xiao Chang Jing (Large and Small Intestine Meridians), Clear San Jiao, Move Ba Gua, Dig Zong Jin line, Divide Yin and Yang, Water Fishing for Bright Moon technique, Push Shang San Guan 24 times, Tui Xia Liu Fu 80 times, Fei Jing Zou Qi technique, Move Zhou area.
Blood heat (Xue Re) occurs specifically during Chen and Si hours (7 AM - 11 AM), cooling down at night. Laypeople often mistake it for consumption (Xu Lao) or Gan heat, but it is actually blood heat.
Treatment methods: Push San Guan point, Push Shang San Guan, Tui Xia Liu Fu, Divide Yin and Yang, Move Ba Gua, Move Wu Jing points, Dig Shi Wang points, Dig Zong Jin line, Tonify Shen Shui point, Water Fishing for Bright Moon technique, Rou Zhou (Rub Elbow) point, An Xian Cuo Mo (Press String and Rub) technique, Fei Jing Zou Qi technique.
Bone-steaming heat (Gu Zheng Re) is a deep heat steaming from the bones, present without chills, stopping only after waking with night sweats, distinct from superficial burning. Etiologies include early introduction of meat, habitual consumption of roasted wheat foods, preference for fruits like peaches, plums, waxberries, or melons, or wearing thick cotton clothes in winter, all consuming body fluids. It may also stem from residual toxins of Gan disease transmitting to become bone-steaming, or from abdominal masses (Pi Xi) causing intermittent pain.
Treatment methods: Push San Guan point, Retire Liu Fu point, Move Wu Jing points, Divide Yin and Yang, Clear Tian He Shui, Water Fishing for Bright Moon technique, Tonify Shen Shui point, Dig Zong Jin line, Push Da Heng Wen (Major Transverse Crease), Da Ma Guo Tian He Shui technique.
High fever (Zhuang Re) persists continuously due to abundant and stagnant Qi and Blood, generating heat in the Five Zang organs, steaming internally. This causes high body temperature, restlessness in sleep, mental confusion, and external manifestation of heat in both interior and exterior. Severe cases may lead to convulsions.
Treatment methods: Push San Guan point, Retire Liu Fu point, Clear Fei Jing, Divide Yin and Yang, Tui Men Shan point, Clear Xin Jing, Clear Tian He Shui, Move Wu Jing points, Dig Zong Jin line, Move Zhou area, Water Fishing for Bright Moon technique, Fei Jing Zou Qi technique.
Warm-high fever (Wen Zhuang Re) is similar to high fever but less intense. It is called Wen Zhuang due to stomach Qi disharmony, Qi stagnation, and obstruction, leading to accumulated body heat. Yellow, foul stool indicates latent heat in the abdomen; white, sour stool indicates food stagnation. Mild purgation is suitable.
Treatment methods: Push San Guan point, Retire Liu Fu point, Move Wu Jing points, Push Da Chang Jing, Tonify Shen Shui point, Move Ba Gua, Push Pang Guang Jing, Divide Yin and Yang, Water Fishing for Bright Moon technique, Da Ma Guo Tian He Shui technique.
Bian Zheng Re (Transformation and Evaporation Fever): The interaction of Yin-Yang and Water-Fire steams within Qi and Blood to mature the physical form. "Bian" refers to the transformation and generation of the Five Zang organs; "Zheng" refers to the nourishment of the Six Fu organs. At 32 days after birth, there is one "Bian"; at 64 days, one "Zheng". After ten Bians and five Zhengs (totaling 320 days), the process completes, and the child matures. Infant Bian Zheng is analogous to silkworm molting, dragon shedding skin, or tiger changing claws—natural growth processes. Symptoms include mild fever resembling steaming, upward Qi rebellion with fright, cold ears with slight sweating, white pearl-like blisters on the upper lip, or mild swelling resembling a lying silkworm. Severe cases involve high fever, confusion, abdominal pain, crying, and inability to nurse; reduce milk intake slightly. Do not administer drugs, tuina, or moxibustion arbitrarily. Misdiagnosis and mistreatment can be fatal. Therefore, no treatment methods are prescribed here; this is recorded solely for warning.
