Throat obstruction (Hou Bi) is attributed to phlegm, wind, and heat, all of which should involve stagnant fire combined with heat toxicity. Severe swelling with insensibility constitutes a severe condition of the throat. "Throat obstruction" is the general term for this category.
Removing wind-phlegm, resolving toxicity, and opening up stagnation will allow the symptoms to heal spontaneously. Mutton-beard tonsillitis (Ru E) can be unilateral, bilateral, or pearl-string type, mostly arising from excessive alcohol consumption and sexual indulgence, as well as emotional depression. On the first day of onset, there is pain; on the second day, redness and swelling; on the third day, distinct forms appear with fine white spots (if no fine spots are present, it is a throat abscess). If accompanied by chills and fever, the prognosis is ominous. By the fourth day, the condition stabilizes, and treatment over four to five days leads to recovery. The lesions occur beside the throat: those appearing on one side are unilateral mutton-beard tonsillitis, while those on both sides are bilateral. Unilateral cases are mild, bilateral are heavier, and pearl-string is even more severe. When the white spots below (corresponding to the Heart and Liver meridians) connect on both sides, resembling a wrapped bag, it is called pearl-string tonsillitis.
This disease belongs to melancholy-induced blood heat and qi stagnation, commonly affecting women. The shape resembles duckweed, slightly elevated and purulent, purple in color, occurring beside the throat. Mild cases last half a month to twenty days; severe cases may last a month or more. The key lies in appropriate treatment and the patient's adherence to dietary restrictions.
Deficient fire flaring upward, with excessive Fire of Metal (Lung), attacks the throat. The growths resemble silk threads, patterns like Jingdezhen porcelain glaze, or the red veins on the back of begonia leaves. Food intake is obstructed, causing swallowing pain. Although not immediately fatal, prolonged duration leads to hoarseness and loss of voice, which becomes incurable.
This results from excessive consumption of spicy, roasted, rich flavors, and strong wine, contracting heat pathogens. It is a Lung disease. There is no specific morphology in the throat, only redness, swelling, and pain. Severe cases also present with chills, fever, and headache, resolving in four to five days.
(Lingual frenulum pathology, pronounced 'Sha')
Obese individuals who contract heat and have an irritable temperament often suffer from this. Any growth under the tongue resembling a small tongue is termed lingual cyst (She). If connected to the throat with swelling and pain, it is a throat abscess; if painless, it is not. Generally, when lingual cysts co-occur with throat abscesses, the condition is perilous.
Onset after typhoid fever is difficult to treat due to qi closure and blockage, presenting without form or sound.
It arises from mental restlessness. Two days prior, there must be tightness in the chest and diaphragm, shortness of breath, yet accompanied by sore throat and swelling, cold hands and feet, neck sensation as if twisted, internal heat accumulation, external swelling surrounding it, numbness and itching, red filaments winding inside the throat, white fingernails, intense heat in the palms, large throat swelling, abundant wind-phlegm, and a sound like sawing wood. These are its signs, representing a highly contraindicated syndrome. If treated on the first day of onset, it is curable. If more than a day and night pass, and the patient exhibits staring eyes, thunder-like sounds in the throat, alternating redness and cold/heat, and an open mouth that cannot close, these are the signs. One must wait until a week has passed without incident before administering medication; early intervention is crucial.
