*Chao Shi Bing Yuan* (Source of Diseases by Master Chao) records: Infantile convulsion arises from disharmony of blood and qi, with excess heat inside the body, causing instability of the heart spirit (Shen), thus triggering convulsions; in severe cases, limb twitching and contraction occur, evolving into epilepsy. Additionally, during "bian zheng" (physiological developmental changes in infants), children may experience mild convulsions. This is also due to heat. Mild convulsions are permissible as they help nourish the blood vessels, but severe convulsions should be avoided; if severe convulsions occur, moxibustion on the "Jing Mai" (Convulsion Meridian) is required. If moxibustion is performed within fifty to sixty days after birth, convulsions may worsen. However, performing moxibustion on the Jing Mai after one hundred days of age is appropriate.
*Tai Ping Sheng Hui Fang* (Imperial Grace Formulary of the Taiping Era) discusses: Infantile acute convulsive wind (Ji Jing Feng) results from disharmony of blood and qi, internal excess heat, and invasion by wind pathogen interfering with the Heart collateral. The Heart houses the Shen and governs the blood vessels. When heat is excessive, blood circulation becomes chaotic; chaotic blood causes Qi to congregate with it, leading to conflict between Qi and Blood, compounded by wind pathogen attack, resulting in fright and restlessness. Clinical manifestations include: high fever throughout the body, accumulation and stagnation of phlegm-saliva, stiffness and urgency of the four limbs, spasms and twitching of tendons and vessels, rigidity of the neck and back, and lockjaw.
Master Mao discusses: Children from one year old up to ten years old may suffer from acute convulsive wind, Ke Wu (fright caused by external stimuli), or sudden death. These three conditions share similar management principles but have distinct initial causes. The presentation of acute convulsive wind includes: noisy breathing with phlegm, bilateral hand and foot convulsions, fixed staring eyes, cyanosis of the face and mouth, and loss of consciousness. This condition stems from the infant's inherent Yang excess, improper care such as overdressing, or fright from dogs/dogs near the infant's resting place, or loud noises startling the child. This leads to accumulation of fright Qi in the Heart, combined with deficiency and wind pathogen invasion.
Qian Yi discusses: Convulsions triggered by loud noises or fright, resolving afterwards, indicate a lack of Yin. Purging methods should be used, primarily with Li Jing Wan (Benefit Fright Pill). (Formula see this section.) Infantile acute convulsive wind fundamentally arises from heat generating in the Heart, presenting with body heat, red face, thirst, hot breath, yellow-red urine and stool, and severe convulsions. Heat extreme generates wind; wind corresponds to the Liver, indicating excess Yang and deficient Yin. Thus, Li Jing Wan is used to clear phlegm-heat. Do not use Radix Euphorbiae Pekingensis (Ba Dou) or warm purgatives, as this may exacerbate convulsions and fail to resolve deficient heat. Infantile Ke Wu involves phlegm-heat in the Heart and Stomach; hearing unusual sounds triggers fright and convulsions. If heat is extreme, spontaneous convulsions may occur even without auditory triggers.
Zhang Huan discusses: Infants often have unstable Shen and accumulated phlegm-saliva in the Pericardium, generating evil heat. Excessive heat interferes with the Shen, combined with external wind pathogen invasion, causing high fever, phlegm stagnation, limb spasms, and lockjaw, termed acute convulsive wind disease.
*Tong Ying Bao Jian* (Precious Mirror for Infants and Children) discusses: Infantile acute convulsive wind occurs when fright-phlegm floods the Heart, causing upward deviation of the eyes, convulsions of hands and feet, body heat, rigid jaw, and inability to open the mouth.
*Miyao Zhimi* (Secret Essentials Pointing to Enlightenment) discusses: After recovery from infantile acute convulsive wind, recurrence after several days followed by recovery is common. If this happens three or four times, subsequent severe episodes indicate a "Xing Xing" (lucid but weak) state, which is unreliable for prognosis.
*Yu Jue* (Jade Decree) discusses: Infantile acute convulsive wind is caused by wind-heat interfering with the Heart, preceded by fright. Previous fright-phlegm enters the meridians, presenting with convulsions, upward-deviated eyes, black lips, lockjaw, and hand-foot spasms. Treatment relies on emesis and purgation to calm the Heart. Using conflicting cold and hot medicines may endanger life.
*Shi Bi Jing* (Stone Wall Classic) Song on Initial Acute Convulsive Wind Signs:
Observe clenched fists upon onset, carefully noting finger positions. (*Feng Sui Jing* notes: refers to the thumb.) Palm interior is Yin, exterior is Yang; this is a favorable sign. (At onset of spasms, if palm is red and fingers curled, boy's thumb out/girl's thumb in is favorable.) Boys left, girls right spasm should be observed first. (Alternatively: induce cessation of spasms.) Apply medicine below eyes to open them. (*Xian Sui Jing* says: insufflate medicine into nose.) Re-examine symptoms. (*Feng Sui Jing* says: sneezing is acceptable.) Strictly avoid phlegm flooding lungs and lodging in Heart. (If boy spasms right/girl spasms left, it is unfavorable/incurable. Continuous sticky phlegm requires clearing diaphragm/phlegm. If eyes open/phlegm disperses, administer anti-fright/Qi-regulating herbs; stop if eyes closed/phlegm continuous implies death.) Avoid cooling Heart treatments; resolve fright and purge to seek peace. (*Feng Sui Jing* variant: Resolve meridians/regulate Qi with decoctions/pills.) Note pulse inversion of Yin/Yang. (*Feng Sui Jing* variant: Focus intent during spasm control.) Use wisely, knowing life may be lost. (Left/Right spasm gender rule explained. *Feng Sui Jing* note: First insufflate Du Yue San, then Zhen Xin Wan, then Sheng Yin Wan.)
*Shi Bi Jing* Song on Acute Convulsive Wind Signs:
Startled by objects within seven days. (Within seven days, fright generates heat/convulsions. If infant bian zheng, also causes fright; avoid excessive cold herbs.) Hair stands dry, throat parched, diarrhea green. (Hair straight/dry like hemp; frequent nursing/water drinking causes "dry throat"; green diarrhea indicates spleen/stomach involvement.) Check upper lip for slight sweat, observe bluish veins under eyes (or abdomen), tender peach-blossom palms, fearfulness with noisy phlegm. Experts calm fright effectively; do not confuse with general wind-heat.
These are external signs. Treatment: first calm fright, then resolve thirst/phlegm for cure. Even if Qi is excess, regulate Stomach Qi before using fright-resolving herbs; avoid excessive cold. (*Feng Sui Jing* weak wind song: recommend Zhen Xin Wan, Sheng Yin Wan.)
*Hui Yan Guan Zheng* (Compassionate Eye Observing Signs) on Acute Wind: Internal wind-stagnant heat-phlegm surges to mouth, back rigidity, eyes closed, hand-foot spasms, or staring with noisy phlegm, unconsciousness. Urgently use Shui Jing Gao (Sleep Fright Paste - see section), ground with honey, sugar, mint decoction. Vomiting two mouthfuls of phlegm lowers eyes, induces sleep. Later, pass 3-4 stools of green-white sticky phlegm, follow with Qi-regulating herbs, next day sleep more, use Xing Pi San (Awaken Spleen Powder - see vomiting/diarrhea section). If no appetite, use An Wei Wan (Peace Stomach Pill). For residual phlegm, use Hua Xian Tang (Resolve Phlegm Decoction). Intermittent treatment brings peace in three days. Fever on day 4-5 indicates wind-stroke or cold damage; use Mai Tang San (Wheat Decoction Powder - see end) and Ping Wei Wan (Level Stomach Pill - see hiccups section) for recovery in two days.
*Xiao Fang Mai Lun* (Small Formula Pulse Discussion) states: Acute wind originates in the Heart. All fright diseases accumulate in the Heart. Onset features high fever, then spasms, extreme body heat, limb restlessness, full-body heat, red cheeks, dry mouth/tongue. Caused by fright/shock; treatment focuses on the Heart.
Li Gangzhong, physician from Changsha, states: Ancient texts mention only Yin/Yang epilepsy, not fright. Yang type is Ji Jing (Acute Fright), Yin type is Man Jing (Slow Fright). Yang moves/restless/fast; Yin still/slow/delayed. Infantile acute spasms result from excessive heat affecting both interior/exterior. Long-standing unremoved heat combines Liver wind and Heart fire; these two organs contend, Qi burns fiercely, and Kidney Water cannot control them. Liver governs tendons/eyes; heat contracts tendons, wind triggers spasms, hands/feet uncontrollable, wind pulls eyes upward. Left/right spasms depend on organ dominance. Treating with Black Snake, Centipede etc. treats branch not root. Qian Yi uses Xie Qing Wan (Drain Green Pill) for Liver wind, Dao Chi San (Direct Red Powder) for Heart fire. These are superior herbs.
