Wang of Handong's *Jiabao* discusses: Whenever infants and young children suffer from convulsive internal retraction (Jingfeng Neidiao, a pediatric emergency characterized by limb spasms and head retraction), coiling intestinal qi (Panchangqi, a pediatric condition marked by abdominal pain, curved waist, and tearless crying), and worm pain, the onset of these three conditions appears similar. In convulsive internal retraction, if fine, fragmented red veins appear at the corners of the eyes, it is this condition. When coiling intestinal qi attacks, the waist first curves, there is empty crying without tears, the upper lip is dry, and sweat appears on the forehead; this is coiling intestinal qi. Worm pain manifests as spitting saliva and purple lips and mouth; this is worm pain. If it is convulsive internal retraction, immediately administer Taofu Wan (Peach Talisman Pill). After administration, follow with Ruxiang Wan (Frankincense Pill) (formula see in this section) and Daqibao San (Great Seven Treasures Powder) (see in Cholera and Vomiting/Dysentery section). If it is coiling intestinal qi, immediately administer Gouteng Gao (Uncaria Paste) two to three doses, and Weixiang San (Wei Fragrance Powder) two to three doses (formula see in Coiling Intestinal Qi section), and also must administer Daqibao San two to three doses. If it is malnutrition-related worm pain with abdominal pain, immediately administer Ganxi San (Dry Lacquer Powder) two to three doses, and combine with Huachong Wan (Worm-Dissolving Pill) three to five doses (formula see in Ascariasis Malnutrition section), and can also intermittently administer Daqibao San. (Later text says: Pain from roundworms/clams indicates ascariasis malnutrition.)
The song verse for the "Convulsive Internal Retraction" syndrome in the Thirty-Six Syndromes of *Shibi Jing* states:
Internal retraction often involves crying; observe carefully,
Do not equate worm pain with a single statement.
Saliva at the lips and purple nose indicate worm pain, (Another version: Worm pain involves spitting saliva and purple lips/mouth.)
For internal retraction, one must observe for blackness.
Simply look for red veins growing at the eye corners,
And you will know the root source of the wind syndrome. (Forty-Eight Syndromes state: Just look for red veins appearing at the eye corners, mixed with blood presenting as dotted spots.)
Using heart-calming medicines as the prescription is excellent,
Only then does one feel the skilled physician follows the ancient sages.
Its symptoms involve alternating cold and heat, internal pressure, stagnation of Qi and Blood, accumulation and failure to dissipate, causing abdominal pain. One need only regulate Qi, remove accumulated heat and cold, harmonize toxins, and recovery will ensue.
The song verse in *Fengsui Jing* is identical, with a note stating: Suitable to be combined with Shengyin Wan (Raw Silver Pill) and Yuetao San (Greengage Powder). (Shengyin Wan formula see in Acute/Chronic Convulsions section, Yuetao San formula see in Coiling Intestinal Qi section.)
Furthermore, in the syndromes regarding convulsions and internal retraction in the Thirty-Six Syndromes: This syndrome is suitable for Xieshao Wan (Scorpion Tip Pill); if not relieved, then mildly purge. (Formula see in Tremors/Spasms section.)
The song verse for the Forty-Eight Syndromes of convulsive internal retraction in *Xiao'er Xingzheng Lun* is identical, followed by: This syndrome is similar to the roundworm syndrome. Purple-black lips and mouth indicate worm syndrome; red veins in the eyes, hands extending backward, and high chest indicate internal retraction.
Wang of Handong's *Jiabao* treats infant/child convulsive internal retraction. Taofu Wan (Peach Talisman Pill):
Yinzhu (Cinnabar, Zhu Sha) and Ruxiang (Frankincense, Ruxiang) (each 1 qian, ground); Dasuan (Garlic, Dacong - specifically bulb) (one clove, roasted, ground).
First grind Ruxiang extremely fine, then add Yinzhu and grind again, then grind together with Dasuan. Adjust consistency appropriately, form pills the size of a large sesame seed. For half-year-old infants, 5 pills per dose; for one-year-olds, 7 pills; for two-to-three-year-olds, 10 pills. Adjust according to circumstances, dissolve and administer with Bohe Tang (Mint Decoction).
Wang of Handong's *Jiabao* treats child convulsive internal retraction with unbearable pain. Ruxiang Wan (Frankincense Pill):
Ruxiang (Frankincense, Ruxiang) (1 qian); Xieshao (Scorpion Tip, Quanxie) (14 pieces); Moyao (Myrrh, Moyao) (0.5 qian); Chenxiang (Agarwood, Chenxiang) (1.5 qian).
Grind all into powder, mix with refined honey to form pills the size of millet seeds. For infants, 3 pills per dose; for one-year-olds, 5 pills; for three-year-olds, 7 pills. Adjust according to circumstances, swallow with Ruxiang Tang (Frankincense Decoction).
*Jubao Fang*'s Chensha San (Cinnabar Powder) treats pediatric cold-injury and accumulated retention causing convulsions, which over time transforms into internal retraction. People do not recognize it and call it "Sui" (demon illness).
Zhusha (Cinnabar, Zhusha) (0.5 fen); Hongshaoyao (Red Peony Root, Baishao/Hongshao) (1.5 fen); Qianbaishuang (Lead White Frost, Qianbaishuang) (1.5 fen); Hupo (Amber, Hupo) (ground); Zhenzhu (Pearl, Zhenzhu) (non-drilled, ground; each 1 fen).
Grind the above five ingredients into powder. Each dose is "yi zi" (a small measure), adjusted with Yinjin Bohe Tang (Silver Flower and Mint Decoction).
*Zhangshi Jiazhan*: Child/infant internal retraction pain-relieving pill.
Mubiezi Rou (Seed of Momordica charantia, Mubiezi) and Hujiao (Pepper, Bihua/Jiaopei) (equal parts).
Grind into fine powder. Use black bean powder and vinegar to make a paste, form pills the size of mung beans. Each dose 3-4 pills, taken with Jingjie Tang (Schizonepeta Decoction).
*Jishi Jiazhan* treats pediatric convulsion-retention internal retraction, frequent abdominal pain, night crying, and startled screaming. Zhanxie Dan (Evil-Slaying Pill):
Ruxiang (Frankincense, Ruxiang); Moyao (Myrrh, Moyao); Gouteng (Uncaria Rhynchophylla, Gouteng); Muxiang (Aucklandia Root, Muxiang - unroasted); Bochashuhuixiang (Imported Fennel, Huixiang) (stir-fried; each 1 qian).
Grind into powder. First grind Ruxiang and Moyao finely in a mortar. Then evenly mix other herbs. Take three slices of garlic white, grind until red, and mix with the previous medicine. Form pills the size of paulownia seeds. Each dose 10-15 pills, swallowed with Gouteng and Huixiang Decoction, at any time.
*Zhushi Jiazhan* treats pediatric convulsive internal retraction with unbearable abdominal pain. Meishizi Wan (Diospyros Pill):
Muxiang (Aucklandia Root, Muxiang); Luofen (Conch Shell Powder, Wuleifen) (calcined); Caowutou (Aconite Root, Caowutou) (raw, skin and tip removed).
Equal parts of the above, grind into powder. Use vinegar-boiled paste to form pills the size of millet seeds. Each dose 10 pills, swallowed with light vinegar water.
