According to *Yingtong Baojian*: When a child receives toxic heat in the womb, which flows into the interstices of the bone marrow, and still suffers from convulsive diseases (Jing Ji) within one hundred days after birth, if cold-natured medicines are administered excessively causing diarrhea, leading to a cold abdomen and heat within the bones, this condition is termed Gan Lao (Gan Consumption).
The song verse regarding the signs of Gan Lao in children from *Huiji* states:
Loving seasonal fruits and sweets leads inevitably to the formation of Gan accumulation. Green veins appear on the abdomen with frequent distension; the skin turns yellow and the spirit listless. Mouth ulcers and swollen eyes manifest as facial discoloration; thirst drives unregulated drinking, while diarrhea produces turbid stools like rice wash water. Even though the whole body is emaciated, touching it reveals burning heat. Treatment requires first promoting movement and transformation; "Gan accumulation amidst deficiency" is the discussed pathology.
*Culun Jing* records the Dihuang Decoction for treating pediatric Gan Lao, cough due to Lung Qi Heat, gradual emaciation of the four limbs, and dryness of the Heart and Lungs:
Fresh Radix Rehmanniae Juice (Wu Liang), Toad Venom (Chansu), Deer Antler Glue (Ban Liang), three types of fresh juices, Honey (each Yi Liang).
First, place the Fresh Radix Rehmanniae Juice in a pan and simmer slowly, stirring continuously. After approximately five or six boils, add the Honey, then the Glue, followed by Ginger Juice. Simmer slowly until the consistency resembles thin syrup, then remove from fire. Administer half a spoonful twice after meals. Avoid toxic foods.
*Zhang Family Tradition* records Sanhe Yinzi (a formula by Elder Huihai) for treating Cold in the San Jiao and diaphragm, stagnation of the Five Zang organs, rebellious Qi and phlegm-saliva; aversion to saliva after eating rice, headache in the Taiyang region; mountain miasma causing vomiting and retching, poor appetite, sallow floating complexion, and cyanotic-black fingernails; pediatric Gan Lao with milk vomiting; and alternating chills and fever with incomplete recovery of Shen Qi after prolonged illness in adults and children. It treats all these conditions:
Purple Tuan Ginseng Root (San Liang Ban, washed, sliced), Prepared Licorice Root (Liang Liang Ban, honey-fried, sliced), Mian Huangqi (Wu Liang, soaked in wine overnight, washed, sliced).
Place the above three ingredients in a wooden mortar and pound them into a powder. Take three Qian per dose, with three slices of Fresh Ginger, two Zhan of water, and three Jujubes. Decoct together, strain, and take eight parts of the liquid. Take at any time.
*Ji Family Tradition* Jin Gua San for treating pediatric Gan Lao, sallow emaciation, bone-steaming fever, and night sweats:
Xuan Lian (Coptis Chinensis Rhizoma), Huangbai (Phellodendri Cortex, fried, peeled), Fried Licorice Root, Qingpi (Citri Reticulatae Pericarpium, white pith removed, fried).
Grind the above into powder equally. Mix with Pig Bile to form a paste, store inside a gallbladder bag, tie tightly with hemp and cotton, boil in water for several times, then remove. Hang in the wind overnight, remove the gallbladder bag, add a small amount of Moschus (Shexiang), grind well, and make pills the size of a wutong seed. Take five pills per dose, swallowed with Ophiopogon Decoction (Maidong Shui).
*Ji Family Tradition* Luhui Wan for treating pediatric Gan Lao, emaciation, bone-steaming fever, and night sweats:
Luhui (Aloe), Dingxiang (Caryophylli Flos), Chao Shi Junzi Ren (Fried Semen Quisqualis), Huhuanglian (Picrorhizae Rhizoma), Zhusha (Cinnabar), Roudoukou (Myristicae Semen), Anshixiang (Styrax), Xiongdan (Ursi Fel), each Yi Fen; Qingfen (Calomelas, Ban Qian); Shexiang (Moschus, a little).
Grind all into powder. Use Pig Bile cooked into a paste to bind and make pills the size of a wutong seed. Take five or seven pills per dose, swallowed with warm water.
Mr. Wang from Handong treated Gan Lao with high fever and emaciation in children over ten years old using Rouji Jian Wan:
Xuan Lian (Er Liang), Yinchaihu (Radix Stellariae), Wuyi, Heshi (Sichuan origin, each Ban Liang), Qinjiao (Cleaned), Zhimu (Rhizoma Anemarrhenae), Shi Junzi, Ziqin (Radix Scutellariae, each Yi Liang).
Grind into powder. Take one yellow hen weighing one Jin, feed exclusively with hemp seeds for five days. Open the vent, remove intestines and viscera, wash and dry. Insert the medicine into the cavity and sew with thread. Place black beans at the bottom of the steamer, about three Cun thick. Place the chicken around and on top, cover with beans again. Steam from sunrise until evening, then retrieve the medicine. Grind the clean meat of the chicken with the medicine. If too dry, add wine paste. Make pills the size of hemp seeds or mung beans. Take ten to twenty pills on an empty stomach with Maidong Tang. Adjust dosage according to circumstances. For those over fifteen, take with warm wine. Avoid pork.
