Infantile umbilical tetanus presents with abdominal distension, stiffness and straightening of the four limbs, frequent crying but refusal to suckle, cyanosis of the lips, perioral contraction (crouching mouth), occasional spitting of white foam, and the appearance of bluish veins on the abdomen extending upward and branching. Careful examination is required; if the bluish veins reach the heart region, it is considered intractable. Observe the end point of the bluish veins and dissipate them using lamp-wick therapy. Alternatively, moxibustion can be applied to the umbilicus, first placing a slice of fresh ginger or garlic over the umbilicus before applying moxa. Lamp-wick therapy from the Lantern Festival period may also be used to dissipate the condition. If white blisters are observed on the fontanelle of the infant, gently puncture them with a silver needle. Healing is certain if bleeding occurs.
Ma Ya Qi Fang (Marble Tooth Miraculous Formula) for Infantile Umbilical Tetanus:
Borneol (Yang Pian), Musk (Cun Xiang), Alum (Ku Fan, 2 qian), Borax (Peng Sha, 5 fen). Grind all into a fine powder. After bathing the newborn upon landing, immediately apply this powder to the umbilical stump. Reapply during each diaper change. Upon completion of one dose, umbilical tetanus and related syndromes will be permanently prevented.
Lantern-Wick Therapy:
Indicated for infantile umbilical tetanus, crouching mouth syndrome, cyanotic or blackish complexion, opisthotonos (body arched backward like a bow), and strabismus. It is urgently indicated for sudden death caused by wind-closure, fire-closure, or phlegm-closure. However, it is strictly contraindicated for patients with chronic illness and weakness, emaciation, spontaneous sweating, night sweats, vomiting and diarrhea due to smallpox/measles, post-hemorrhagic states, or mental fatigue.
Song of Lantern-Wick Therapy:
The fifteen points of lantern-wick therapy include the fontanelle, the two bones at the glabella, the seven areas around the umbilical wheel and center, and the four areas along the shoe-lace lines to relieve the child's suffering. For convulsive wind where the infant lies supine, apply wick therapy to the fontanelle, both sides of the glabella, and above and below the umbilicus. For inverted eyes that cannot look down, apply to above and below the umbilicus. For those without salivation, apply to above and below the mouth, and the palms and soles. This reveals the secret method for treating convulsive wind.
When administering medicine for umbilical tetanus, observe if the breath from the mouth is hot, if the lips are red, if there is ocular discharge and crusting, and if the fingerprint (Guan Wen) is purple-blue, coarse, and floating. Quickly use the following prescription:
Uncaria rhynchophylla (Gouteng), Cicada slough (Chantui), Gastrodia elata (Tianma), Bombyx batryticatus (Jiangcan), Scutellaria baicalensis (Huangqin), Xi Feng (Xifeng), Exocarpium citri grandis (Guangpi), Forsythia suspensa (Lianqiao), Glycyrrhiza uralensis (Gancao) (each 1 qian), Arisaema cum bile (Danning) (3 fen). Use Plantago asiatica (Cheqian Cao) as a guide, decoct in water and administer.
If the complexion is cyanotic, lips pale, extremities cold, breath from the mouth is cold, jaw is rigid, eyes are looking upward, and the fingerprint is blue-black, not floating but faint, quickly use the following prescription:
Codonopsis pilosula (Dangshen), Jian [Herb], Gastrodia elata (Tianma), Dioscorea opposita (Huai Shanyao) (each 2 qian), Aconitum carmichaelii preparatum (Xiong Pian), Zingiber officinale (Ganjiang), Pinellia ternata preparata (Faxia), Exocarpium citri grandis (Guangpi), Poria cocos (Fuling), Bombyx batryticatus (Jiangcan), Angelica sinensis (Danggui) (each 1 qian), Glycyrrhiza uralensis (Gancao) (5 fen), Jujube (Dazao, 2 pieces, as a guide).
