若水火两绝、赤脉贯矣。)向上直视不转睛。(向上直视不动、肾腑俱绝。)手足不收毛发竖。

Red vessels appear in the eyes, traversing the pupil. (The pupil belongs to the Kidney; there are two tendons of the Kidney extending from the spine to the forehead, connecting both eyes. The Heart and Kidney interact, with Water and Fire mutually assisting. If the Qi of Water and Fire is severed, red vessels will traverse the pupil.) Upward fixed gaze without eye movement. (Fixed upward gaze indicates complete exhaustion of Kidney Qi.) Inability to control hands and feet, with hair standing erect. (The Stomach governs skin, muscles, and four limbs; when Stomach Qi is exhausted, hair stands erect, and hands and feet lose control.) Fontanelle swells or sinks into a pit. (The Heart governs Blood; when Heart Qi is exhausted, Blood fails to ascend.) Collapse of the Tianzhu bone (neck), with the head tilting backward. (Exhaustion of Heart Qi, an untreatable condition.) Teeth grinding, tongue protruding, and unclear speech. (The tongue is the external manifestation of the Heart; when Heart Qi is exhausted and Blood circulation is poor, the flaccid tongue may fall out of the mouth, or the tongue becomes short and unable to speak.) Biting teeth or biting others indicates exhaustion of the Kidney pulse. (The Kidney is the master of bones, and teeth are the surplus of bones; exhaustion leads to itchy teeth, grinding, or biting. When the Kidney is exhausted, the Heart is also exhausted.) Dry and black nostrils indicate severe invasion. (The nose is the external manifestation of the Lung; dryness and blackness indicate Lung Qi exhaustion.) Fish-mouth breathing with coarse breath and inability to cry. (The Lung is the master of Qi; when Lung Qi is exhausted, Qi exits but does not return, or breathing is急促 and difficult for crying. "Fish-mouth" refers to the nostrils opening and closing alternately.) Sudden crow-like sounds indicate exhaustion of the Large Intestine pulse. (Sudden crow-like sounds indicate exhaustion of the Large Intestine Qi; "crow sound" refers to altered voice that persists.) Hands holding the head, eyes lacking luster. (The Liver stores Blood, and the eyes are its external manifestation; when Blood vessels fail to nourish, nails turn cyan-black, and eyes lack luster. Tendon contraction causes hands to hold the head, indicating Liver Qi exhaustion.) Nails turn black with a touch of cyan. Sawing sounds in the throat with foamy saliva. (Any case where saliva flows out of the mouth.) Wind disease blocking orifices, with dark complexion and abnormal form. (Dryness of the five orifices is an untreatable condition, caused by wind-phlegm blocking the orifices, preventing blood circulation and the intake of decoctions, resulting in a cyan-black complexion.) Cold Heart and exhausted pulse causing Lung distension. Drinking water passes straight through, with no retention in the Stomach. (Both Lung and Stomach Qi are exhausted; continuous drinking with water passing directly to the Large Intestine indicates a fatal condition.) Dysentery resembling blood from dead ducks or geese. (Heart Qi exhaustion.) Foul-smelling bloody fluid mixed with undigested food. (Complete failure of Zang-Fu organs.) Sweating like oil on the four limbs. (Exhaustion of Ying and Wei Qi, chaotic Yin and Yang, with body fluids scattering to the limbs like sticky glue, an untreatable condition.) Throwing and shaking of hands and feet—how can life be sustained? (Heart Qi exhaustion.) Dark complexion and retracted lips unable to cover teeth. (Spleen Qi exhaustion.) Ascaris worms expelled, showing signs of death, confirming the prognosis. (Spleen Qi exhaustion.)
