Pediatric phlegm-wheezing occurs, or wheezing is induced by phlegm. The child does not know how to expectorate (spit out) the phlegm; specific techniques must be employed to induce vomiting. If expectoration is not induced, old phlegm becomes difficult to treat. Wheezing due to Lung deficiency presents with short breath sounds, while wheezing due to excess presents with long breath sounds. The therapeutic principle is tonification for deficiency and purgation for excess. Techniques include: Fen Yin Yang (Dividing Yin and Yang), Yun Ba Gua (Moving the Eight Trigrams), Yun Wu Jing (Moving the Five Meridians), and Qia Si Heng Wen (Pinching the Four Horizontal Lines). For dryness-heat conditions, heavy pushing on the Li trigram is applied; Tonify Spleen Earth if the Spleen is deficient. If urine is dark red, clear He Tian He (River of Heaven), push back Liu Fu (Six Bowels), and apply Fei Jin Zou Qi (Flying Metal Moving Qi). If lips are red, perform An Xian Zuo Cuo Mo (Pressing the String and Rubbing), rub the navel, Jian Jing (GB21), and Qu Chi (LI11) to relieve asthma. Then perform He Yin Yang (Combining Yin and Yang) and Zong Jin (General Tendon); clearing He Tian He can immediately stop wheezing. If accompanied by noisy breathing and fever, rub Cheng Shan (BL57), perform Tian Men Ru Hu Kou (Heaven's Gate Entering Tiger's Mouth), rub the elbow, apply Chi Feng Yao Tou (Red Phoenix Shaking Head), and Fei Jin Zou Qi. If phlegm is abundant with eyes rolling upward and head tilted back, pinch Qi Men (LR14), located one finger-width below both nipples, to stop symptoms. If phlegm confuses the Heart, clear Xin Jing (Heart Meridian), clear Fei Jing (Lung Meridian), rub Wai Lao Gong (EX-UE8), rub Xiao Dao (Guide Point), and pinch Wu Zhi Jie (Five Finger Joints); perform Tian Men Ru Hu Kou and elbow rubbing.
Perform Fen Yin Yang, Yun Ba Gua, and clear Xin Jing. Push from Da Ling (PC7) to Wai Lao Gong, rotate left three times, then rotate right once while massaging. If vomiting does not occur, recite the spell: "Without mundane fire, there is no heavenly fire; without mundane water, there is no immortal water." Use Alum (Sheng Fan): 3 li for infants, 5 li for older children. Dissolve in boiling water. Hold the cup with the left hand forming the Three-Forked Seal, and use the Sword Seal with the right hand to write the command "Da Jiang Jun Ling of Phlegm-Detoxifying General" over the bowl. After writing the command, draw a talisman. Write the character "Shi" (Food) if there is food retention; write "Huo" (Fire) if there is none. Give the water to the child to drink. Within half an hour, the phlegm will be vomited up.
To stop vomiting: Perform Fen Yin Yang, Yun Ba Gua, pinch Xin Jing and rotate left while rubbing, rub Gan Li points, pinch Wai Lao Gong and rotate left, push San Guan (Three Passes), pinch Da Ling and rotate left, clear Fei Jing, tonify Pi Tu (Spleen Earth), pinch Yong Quan (KI1) and rotate left. Vomiting will cease. If vomiting recurs, it indicates internal heat-fire; tonify Pi Tu, Yun Ba Gua, and heavily push Gan Li. Rub the palms until warm, then clockwise rub the child's epigastrium (heart pit) to stop vomiting.
For resolving phlegm: Frequently pinch Wu Zhi Jie. If phlegm is obstructive, rub Yong Quan. Rubbing left dissipates excessive Lung Qi; rubbing right dissipates Heart Fire.
