For patients with smallpox or chickenpox presenting with fever but without chills, this prescription may be administered.
Xilai Ganlu Yin (West-coming Sweet Dew Decoction): Possesses miraculous efficacy in clearing heat and detoxifying. It is indicated for the initial stage of fever; if fever persists within five days of onset, accompanied by red-purple macules/pustules, thirst, and dry, constipated stool, administration will cause the lesions to turn rosy and healthy. It also treats irritability, dry mouth, cough, dry and rough rash texture, delirium, rapid breathing, and restlessness in patients with rashes.
[Preparation] Si Gua Teng (Luffa Vine): Three days after the Frost Descent solar term, cut a two-foot section near the root, boil until severed. Take one joint from the root head, insert it upside down into a new earthenware bottle, cover the top to prevent dust entry. The next day, pour the collected juice into a fine new jar, re-insert the vine segment as before. After three days, when the juice is fully collected, seal the jar tightly for storage and use as needed.
If fever, irritability, and thirst are present but no red spots have appeared yet, take one liang of Qian Gen (Madder Root), decoct in water to obtain two wine-cups of concentrated juice. Mix with two wine-cups of Si Gua Teng juice and administer. Symptoms will stabilize immediately, and the eruption of pustules will be mild. If the eruption has already begun, characterized by red-purple color and density, decoct Zi Cao (Arnebia Root/Lithospermum Root) into a thick liquid to flush/dissolve the medication, which will restore a rosy hue. If accompanied by macules, decoct Xi Jiao (Rhinoceros Horn - *Note: Substitute with Buffalo Horn in modern practice*), Zi Cao, and Qian Gen in water to flush/dissolve the medication; in cold months, use wine for decoction and flushing. This juice is highly effective in clearing heat and detoxifying, particularly for epidemic febrile diseases (Tianxing Shi Yi). A small amount of Sheng Jiang Zhi (Fresh Ginger Juice) mixed with honey can be taken orally for miraculous results.
Hunyuan Zhi (Primordial Fluid): Refers to Zi He Che (Placenta Humanum). Regardless of gender, placenta from a first pregnancy is especially efficacious. Place it in a new earthenware jar, seal it tightly, cover the opening with a bowl, and bury it in the soil. Over time, it transforms into water, which is the final product. It specifically treats conditions of Qi deficiency with Blood heat, such as red-purple, dry, shriveled, or blackened/collapsed pustules. This fluid provides both clearing and tonifying effects, yielding immediate results. For severe Qi deficiency, decoct Ren Shen (Ginseng Root) and Zi Cao together in a thick liquid to flush/dissolve the medication.
Chantui Gao (Cicada Slough Ointment): Chantui (Cicada Slough, toxic parts removed), Dang Gui (Angelica Sinensis Root), Fang Feng (Saposhnikovia Root), Gan Cao (Licorice Root), Chuan Xiong (Ligusticum Rhizome), Jing Jie Sui (Schizonepeta Flower Spike), Sheng Ma (Cimicifuga Rhizome) — all in equal proportions. Add Bai Shao Yao (White Peony Root). Grind all ingredients into fine powder. Use refined honey to form pills the size of a Chinese chestnut. Administer one pill per dose, dissolved and taken down with Bo He Tang (Mint Decoction).
Chensha San (Cinnabar Powder): Hao Chensha (Good Quality Cinnabar, one qian), Si Gua (Luffa Fruit, three inches near the stem, including seeds, burned to ash while retaining medicinal properties; this herb is excellent for promoting the eruption of smallpox). Grind into fine powder. Administer mixed with honey water. It is said that for abundant eruptions, reduce the dosage; for sparse eruptions, it may be omitted. Alternatively, mix with Zi Cao and Gan Cao decoction for superior efficacy.
