Ren Zhai stated: Generally, blood stasis in the heart orifice and evil qi invading the heart lead to epilepsy due to accumulated fright. The key to treatment lies in unblocking the Heart meridian (Heart channel), harmonizing the blood vessels, regulating qi, and resolving phlegm. Suppose a child has heat and phlegm, refuses feeding, sleeps restlessly, and frequently experiences palpitations and fright; these are all precursors to the onset of epilepsy. One should immediately administer Zishuang Wan (Purple Frost Pill) to guide out the pathogen (formula see "Bianzheng Heat" chapter). Administering Zishuang Wan at appropriate times and dosages can reduce excessive pathogenic factors, thereby preventing complications such as wind-stroke and epilepsy.

When the signs of epilepsy are just emerging, green veins must appear in a chaotic, thread-like pattern between the ear and the high bone (mastoid process). Upon seeing this, one should scratch the skin with fingernails until it bleeds and causes crying, which further promotes the free flow of qi.

The reason why patients cannot speak during various epileptic seizures is that the throat is the pathway for qi. Wind damage obstructs this pathway, blocking the gate of voice; this is also caused by blood stasis in the heart leading to blockage of the heart orifice. Prepared Arisaemae Rhizoma (Pao Nanxing) ground into powder, mixed with bile from a male pig, given in small amounts orally, often yields effective results. As for Qian Yi's Wu Xian Wan (Five Epilepsy Pills) and Nanxing San (Arisaema Powder), they can be taken with decoction of Acori Tatarinowii Rhizoma (Shichangpu). Gansui Zhu Xin Tang (Kansui and Pig Heart Decoction) combined with one pill of Suhexiang Wan (Styrax Pill) are both essential medicines for treating epilepsy.

Liu Wansu said: Children's spirit (shen) is still weak; upon fright, the spirit does not remain in its residence. When the residence is empty, phlegm-fluids enter. Alternatively, irregular diet damages the Spleen and Stomach, accumulating into phlegm-fluids, which then confuse the heart orifice, causing disease. The treatment method should seek fire and seek phlegm. However, predecessors mostly used sedative and heart-clearing drugs. While these can treat heat and clear phlegm, they are not easy to expel stubborn, consolidated phlegm. For pathogens located above, emesis must be used first; only after emesis is it suitable to take such drugs. For pathogens firmly rooted inside, purgation is also required, applied according to the severity of the illness.

Li Dongyuan said: If there is exogenous phlegm causing fever with audible sounds, Dawang Jingjie Tang (Rhubarb and Schizonepeta Decoction) can be used to administer Wuse Wan (Five-Color Pills). Periodic tidal fever indicates accumulated heat, treated primarily with Tao Zhi Wan (Peach Twig Pill) (formula see "Accumulated Heat" chapter). Persistent high fever requires using Digupi San (Lycium Bark Powder) to administer Wuse Wan. Wind-heat syndrome requires using Fangfeng Huangqin Tang (Saposhnikovia and Scutellaria Decoction) to administer Daqing Gao (Great Blue Ointment). Warm body without fever requires using Baihu Tang (White Tiger Decoction) plus Cangzhu (Atractylodes Rhizome) to administer Wuse Wan.

Xue Ji said: These are all syndromes of insufficient primordial qi (Yuan Qi). Treatment must prioritize Ziheche Wan (Placenta Pill) (Ziheche refers to the human placenta), assisted by tonic herbs. If harsh draining and attacking methods are indiscriminately used, further damaging the primordial qi, relapses will inevitably occur frequently. Over time, the condition may become critical and incurable.

An elderly man had a son who was just one year old. In early autumn, sudden cold struck, and he suddenly developed convulsions resembling epilepsy. After the episode, his breathing became faint. This was caused by deficiency of primordial qi; he recovered after taking Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).

One ten-year-old child and one seven-year-old child each suffered from epilepsy, occurring twice a year. Later, due to smallpox and food stagnation, attacks became frequent. They were cured using Liujunzi Tang (Six Gentlemen Decoction) and Buzhong Yiqi Tang.

One child suffered from the aforementioned syndrome, vomiting phlegm and feeling fatigued with each attack, waking up after half an hour. Various medications failed. By age thirteen, attacks became frequent. Raw Placenta (Ziheche) was ground into a paste, mixed with powdered Ginseng Radix et Rhizoma (Renshen) and Angelicae Sinensis Radix (Danggui), and formed into pills the size of paulownia seeds. Each dose consisted of 30–50 pills, taken 3–5 times daily, dissolved in breast milk. Gradual improvement occurred within one month, and complete recovery followed with the addition of Bazhen Tang (Eight Treasures Decoction).

A seven-year-old child suffered from fright-induced epilepsy. Before my father, he was instructed to drink human breast milk freely during attacks. Subsequently, attacks became less frequent and milder. At age fourteen, the condition recurred. Breast milk alone was ineffective. I instructed him to use thick Placenta (Ziheche), ground into a paste, mixed with breast milk like mud, taken 2–3 times daily. Recovery occurred after prolonged administration. Afterwards, Jiajian Bawei Wan (Modified Eight-Ingredient Pill) was commonly used to maintain stability. At age twenty-three, the condition recurred with cold extremities. The previous method was repeated, assisted by Bawei Wan (Eight-Ingredient Pill) and Shiquan Dabu Tang (Ten Significant Tonifying Decoction), resulting in recovery.
