Pediatric patients often experience heat-type vomiting (Heat Vomiting) in autumn and summer due to latent summerheat pathogens. Symptoms include vomiting yellow saliva, warmth of the forehead and head, heat in the five centers (palms, soles, and chest), dark and scanty urine; this is classified as Heat Vomiting. Alternatively, it may present as dry heaving without expulsion of matter, for which Xiangru Yin (Elsholtzia Decoction) is indicated.
[Xue Ji] The cause of heat-type vomiting in children is often excessive consumption of sweet, fried, or roasted foods, or the nursing mother consuming rich and greasy foods, leading to accumulated heat in the Stomach channel; or it results from summerheat pathogens lying latent within the Stomach during summer. If muscle twitching occurs accompanied by irritability, fever, and thirst, this indicates summerheat damaging Stomach Qi; first use Xiangru San (Elsholtia Powder), followed by Zhuru Tang (Bamboo Shavings Decoction). If vomited milk appears yellow and cannot be retained, this indicates heat in the Stomach channel; first use Xiehuang San (Drain the Yellow Powder), followed by Renshen Anwei San (Ginseng to Stabilize the Stomach Powder). If sour and foul matter is vomited, this indicates stagnation of milk and food internally; use Baohe Wan (Preserve Harmony Pill). If undigested milk is vomited, this indicates weak Stomach Qi; use Yigong San (Extraordinary Ability Powder). If vomiting occurs with reduced food intake and abdominal pain relieved by pressure, this indicates Spleen Qi deficiency; use Liujunzi Tang (Six Gentlemen Decoction) with Muxiang (Aucklandiae Radix). For all these syndromes, examine the temperature of the child's fingers: if hot, it indicates Stomach heat, treated with Xiehuang San; if cold, it indicates Stomach cold, treated with Lizhong Tang (Regulate the Middle Decoction); if neither hot nor cold, regulate with Yigong San.
