睡则汗出,寤则自收也。钱氏曰∶小儿睡而自汗出者,肌肉虚也,止汗散主之。

Sleeping with sweating, and the sweat stops naturally upon waking. Qian Yi stated: When children sweat spontaneously during sleep, it is due to deficiency of the muscles; Zhihan San (Sweat-Stopping Powder) is the primary treatment.
For sweating over the entire body, Xianggua Wan (Melon Pill) is indicated. For sweating extending from above the stomach down to the umbilicus, this indicates Stomach deficiency; one should tonify the Spleen, using Yihuang San (Yellow Elixir Powder) as the primary treatment. [Xue Ji]
Spontaneous sweating belongs to Yang deficiency, while night sweating belongs to Yin deficiency. This is because Yang constitutes Wei Qi (Defensive Qi), and Yin constitutes Ying Blood (Nutritive Blood). The Heart governs the blood, and the Liver stores the blood. Heat pathogens congregate in the Heart, causing fluids to fail to remain internalized and instead leaking out onto the skin. When a person sleeps, the body is still and corresponds to Yin; when awake, the body moves and corresponds to Yang. Therefore, spontaneous sweating is attributed to Yang, and night sweating to Yin. This condition often arises from the failure of communication between the Heart and Kidney, where Water and Fire fail to interact harmoniously. Kidney deficiency leads to the loss of the closing and storing function, resulting in these symptoms. It is appropriate to use Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) or Shiquan Dabu Tang (Ten-Agent Great Supplement Decoction). For blood deficiency with internal heat, Danggui Liuhuang Tang (Angelica and Astragalus Decoction) is used. For heat in the Heart Meridian, Daochi San (Guide Out Red Powder) is indicated. For deficiency heat in the Liver Meridian, Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) is used. For night sweating due to blood depletion, Danggui Buxue Tang (Angelica Tonifying Blood Decoction) is prescribed. For wind-heat in the Liver and Gallbladder, Chaihu Qinggan San (Bupleurum Clearing Liver Powder) is used. For internal heat due to food stagnation, Erchen Tang (Two-Aged Decoction) combined with Zhishi (Immature Bitter Orange) and Zhizi (Gardenia Fruit) is used. For deficiency heat of Stomach Qi, Liujunzi Tang (Six-Gentleman Decoction) and Fumai San (Floating Wheat Powder) are used. For dual deficiency of Qi and Blood, Renshen Yangrong Tang (Ginseng Nourishing Grace Decoction) is indicated. For other syndromes, refer to the section on spontaneous sweating. [Zeng Shirong] Night sweating refers to sweating that occurs spontaneously during sleep. This is caused by Yang deficiency. If prolonged, it leads to emaciation and wasting. It results from insufficient Heart Qi and the reckless discharge of body fluids. Fu Shen Tang (Poria Cocos Decoction) added with Huangqi (Astragalus Root) is used, taken with water, ginger, jujube, and salt roasted in fire.
[Qian Yi] Huangqi San (Astragalus Powder) treats night sweating due to deficiency heat.

Huzhang San (Persicaria Polymorpha Powder) treats night sweating due to excess heat. Huzhang (Persicaria Polymorpha Rhizome) is decocted in water and administered according to severity, without fixed timing.
[Wang Haizhuang] The son of a Jin Dynasty Langzhong suffered from night sweating for seven years, from infancy to childhood, with no effect from various medicines. I prescribed Liangge San (Cooling Chest Powder) and Sanhuang Wan (Three-Yellows Pill), and the illness was cured in three days. The Kidney transforms into five fluids and becomes five dampnesses. Ministerial Fire forces Kidney Water upward, exploiting the deficiency of the Heart, entering the Hand-Shaoyin Heart Meridian. Heart Fire flares upward into the Lung, bullying the Lung which cannot withstand it. Consequently, the skin pores open, and the Couli (interstices) and Xuanfu (mysterious gates) fail to close, leading to sweating. Sweating during sleep is called night sweating because it ceases upon waking. Thus, the classic states: "Night sweats with aversion to wind." First, use Liangge San to drain the ministerial fire in the chest. Once the ministerial fire recedes, use Sanhuang Wan to drain Heart Fire to assist Yin, allowing Kidney Water to return to its home organ. The Xuanfu closes, and sweating stops spontaneously.
[Zeng Shirong] There was a child who, without disease, sweated profusely like water all over the body during sleep, remaining wet for a long time after waking. This is called accumulation-syndrome night sweating, caused by Cold Spleen. Sanleng San (Sparganium Powder) was used, decocted with water-warmed ginger, followed by Yihuang San (Yellow Elixir Powder) and Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder).
Sanleng San (for Pi Accumulation)
Occasional slight cold sweating, hair roots resembling beads, and visible changes on the forehead indicate a Startled Liver syndrome. Zhenjing Wan (Startle-Calming Pill) or Hubao Longwan (Amber Embracing Dragon Pill) should be used, along with Fu Shen Tang (Poria Cocos Decoction) added with Mahuanggen (Ephedra Root), decocted in water for administration to achieve efficacy.
