The person from Tongbi Mountain said: In general, when treating *gan* syndrome (childhood malnutrition and digestive disorder), there is no need to strictly distinguish among the five types of *gan*. One should simply tonify deficiency, clear heat, warm cold, treat vomiting when it occurs, treat diarrhea when it occurs, resolve accumulation when it exists, and expel parasites when they are present. These treatments do not go beyond adding or subtracting ingredients from the Jisheng Wan formula, which has proven effective in repeated trials.

Jisheng Wan (Collection of Holy Pills)
*Aloe vera* (Luhui), *Lycii Cortex* (Wulingzhi - note: text says Wulingzhi which is Trogopterus Dung, but standard Pinyin is Wu Ling Zhi; context implies Trogopteri Faeces), *Owl Droppings* (Yeming Sha, stir-fried), *Amomum Fruit* (Sharen), *Tangerine Peel* (Juhong), *Costus Root* (Muxiang), *Curcuma Rhizome* (Ezhu, baked), *Quisqualis Seed* (Shijunzi Ren, each 2 qian); *Coptis Rhizome* (Chuanlian), *Sichuan Lovage Rhizome* (Chuanxiong), *Toad Skin* (Ganchan, roasted, each 3 qian); *Angelica Sinensis* (Danggui), *Citrus Peel* (Qingpi, each 1.5 qian).
If caused by deficiency, add *Ginseng Radix* (Renshen) 2 qian and *Atractylodes Macrocephala Rhizome* (Baizhu) 3 qian, remove *Curcuma Rhizome* (Ezhu) and *Citrus Peel* (Qingpi). If caused by heat, add *Gentiana Scabra Root* (Longdancao) 3 qian, remove *Amomum Fruit* (Sharen) and *Curcuma Rhizome* (Ezhu). If caused by vomiting, diarrhea, or dysentery, add *Atractylodes Macrocephala Rhizome* (Baizhu) 2 qian, *Myristica Seed* (Rouguo), and *Terminalia Chebula Fruit* (Weihe Rou, baked) each 1.5 qian, remove *Citrus Peel* (Qingpi) and *Curcuma Rhizome* (Ezhu). If caused by thirst, add *Ginseng Radix* (Renshen) and *Atractylodes Macrocephala Rhizome* (Baizhu) each 2 qian, remove *Curcuma Rhizome* (Ezhu) and *Amomum Fruit* (Sharen).
Grind the above ingredients into a fine powder. Mix with bile from two male pigs and paste made of flour to form pills. Administer according to the child's age, taken down with rice water. This formula is derived from Shiquan Dan by removing *Areca Seed* (Binglang) and *Cardamom* (Baidoukou), and adding *Coptis Rhizome* (Huanglian), *Owl Droppings* (Yeming Sha), and *Amomum Fruit* (Sharen). It is neither hot nor cold, tonifying without causing stagnation, and resolving accumulation without depleting vitality, making it extremely stable and appropriate.

Discussion: Accumulation is the mother of Gan. Therefore, if accumulation is not treated, it becomes Gan syndrome. Furthermore, if accumulation treatment fails to resolve the accumulation, leaving the viscera deficient, Gan becomes even more severe. Generally, pediatric Gan syndromes involve frequent diarrhea without wind signs. Why? Because Gan diarrhea is called Heat Diarrhea. The movement of viscera is limited, so it does not become Wind Syndrome. Even if there is diarrhea without wind, it transforms into other syndromes: thirst, deficiency heat, irritability, dysentery, swelling, fullness, panting urgency—all are deficiency syndromes of Gan. Ancients said: Use tonification for Gan deficiency. Thus, Gan disease should not further drain the viscera. At onset, it is called Gan Qi. Large distended abdomen is called Gan Xu. Frequent diarrhea/dysentery is called Gan Ji. Deficiency irritability in five hearts is called Gan Re. Brittle hair, tufted hair, large belly with visible veins, craving strange foods is called Gan Ji (disease transmission to viscera is extreme). Alternating fever, emaciation, lack of spirit/color in face is called Gan Lao. Small hands/feet, long neck, exposed bones, buttocks without flesh, distended belly with protruding umbilicus is called Dingxi. Vomiting/regurgitation with increased food intake, separated skull sutures, thirst with drinking, worms from mouth is called Bulu. These are all Gan syndromes. Also, excessive consumption of cold, raw, sweet, fatty, sticky foods causes accumulation in the middle jiao that fails to transform, eventually becoming Gan. Treatment of Gan involves assessing severity, regulating viscera, harmonizing middle jiao, smoothing triple burner, warming stomach qi to accept food, strengthening spleen yuan to digest. Then viscera naturally harmonize, qi and blood merge, sinew and bone strength increase, spirit clears, gan dissipates, parasites dissolve, and gradual recovery occurs. If attacking the five viscera with drugs, draining intestines/stomach, removing accumulated toxins and parasites, although called medical treatment, it is not the correct method. Because pediatric viscera deficiency generates parasites, deficiency causes accumulation, deficiency leads to Gan emaciation, deficiency causes fullness. How can purgatives be used? If further draining, intestines/stomach become deficient, leading to Gan. Gan deficiency syndromes become Wugu (innocent) disease. Innocent children are difficult to save.

Huhuanglian Wan (Picrorhiza Pill) treats all infant Gan syndromes and all deficiency dysenteries. Other medicines are ineffective; this one is extremely effective.
*Picrorhiza Rhizome* (Huhuanglian), *Aloe* (Luhui), *Grass Coptis* (Caohuanglian), *Myristica Seed* (Roudoukou, roasted), *Cinnamon Twig* (Guixin), *Ginseng Radix* (Renshen), *Cinnabar* (Zhusha), *Quisqualis Seed* (Shijunzi, shelled), *Costus Root* (Muxiang), *Uncaria Rhynchophylla* (Gouteng), *Dragon Bone* (Longchi), *Poria* (Baifuling) (above each 1 qian), *Musk* (Moxiang, one zi, ground).
Use all raw. Grind to fine powder. Take juice from two pig gallbladders, mix with powder uniformly. Put in a bag, tie tightly, boil in water for half a day. Remove, cut bag open. Add *Datura Seed* (Langdazi, 2 qian) and *Lead Monoxide* (Huandan, 1 qian), grind separately to powder. Mix with previous medicine. Pound 500 times. Form pills the size of mung beans. For any Gan or dysentery, take 5-7 pills with porridge drink. Infants take 3 pills. If refusing porridge, suck on nipple. Can treat twelve types of dysentery and Wugu disease. Efficacy is extraordinary.

Discussion: Gan disease is critical, developing gradually. Dysentery reverses later, transmitting from prolongation (prolongation leads to reversal). Initially seen as mild, called normal. Later recognized as severe, no way to report. Thus, dysentery and Gan both arise from accumulated toxicity and indulgence of the mouth/stomach. Due to deficiency leading to further deficiency, harm causing further harm. Physicians seeing this are worried. Families want quick recovery, often impatient. Seeking survival, conditions deepen. Or Gan extreme with lower abdominal dysentery, or heat excess with increased thirst, or irritability with floating limbs, or sudden vomiting of food. Standard formulas cannot stabilize. Strong agents may damage harmony. All should take this.

Feiji Wan (Flesh-Thickening Pill) treats all pediatric Gan qi, emaciation, weakness, mental fatigue, lack of strength. Regular use kills parasites, resolves Gan, opens stomach, promotes appetite.
*Coptis Rhizome* (Huanglian, remove stamen), *Toosendan Fruit* (Chuanlianrou, stir-fried), *Sichuan Lovage Rhizome* (Chuanxiong) (each 0.5 liang), *Tangerine Peel* (Chenpi), *Cyperi Rhizome* (Xiangfuzi) (each 2.5 qian, boiled in wine, stir-fried dry), *Costus Root* (Muxiang, 2 qian).
Grind to powder. Boil fine flour paste. Form pills the size of hemp seeds. Take 30-50 pills with warm rice drink.

Discussion: Convulsion Gan, accumulation, and dysentery have distinct syndromes requiring different medicines. Now, a child suffers from Gan deficiency fatigue and also has dysentery. Two syndromes combined are most severe. Gan and dysentery running parallel indicate extreme visceral deficiency. Heat toxin is relatively severe. All relate to long-standing accumulation. Although called accumulation, there is no accumulation to treat; it is deficiency receiving the pathogen. Calling it deficiency, tonification is insufficient. Seeing the syndrome, lacking good formulas, how to treat? Even obtaining a formula, without assessing the syndrome, it is hard to cure. Indeed, pulse matches disease, medicine matches syndrome. Physician skill lifts the patient to peace. Huhuanglian Wan cannot be improved. Feiji Wan is also a good assisting formula.
