[Yanshan] For newborn infants, sudden nasal congestion occurring on the third, fifth, sixth, or tenth day after birth, accompanied by difficulty in suckling and mouth-breathing, is mostly caused by the wet nurse falling asleep without supporting the infant's head, allowing the infant's nostrils to face her own mouth and exhale onto the infant's fontanelle. Alternatively, it may result from bathing or washing with water of inconsistent temperature, failing to avoid wind-cold pathogens, thereby causing nasal congestion. It is appropriate to apply Tongguan Gao (Pass-the-Barrier Ointment) externally and administer Xiaofeng San (Wind-Expelling Powder) internally. If symptoms include palpitations and fever, Duhobohe San (Du Mint Powder) should be taken. Tongguan Gao consists of equal parts of Bai Jiangcan (Bombyx Batryticatus), Zhu Ya Zao Jiao (Gleditsia Sinensis Fruit), Jingjie (Schizonepetae Herba), Xiang Fu Zi (Cyperi Rhizoma), Chuanxiong (Ligustici Chuanxiong Rhizoma), and Xixin (Asari Radix et Rhizoma). These are ground into powder, mixed with scallion white (Cong Bai) and pounded together, then applied to the fontanelle for excellent results.
Xiaofeng San (Wind-Expelling Powder): Treats fetal heat and fetal cold; also treats various wind pathogens attacking the head and eyes causing dizziness and pain, stiffness and tension of the neck and back with limb pain, vertigo, nasal congestion with frequent sneezing, skin numbness and hardness, pruritic sores and urticaria, and pediatric deficient wind.
Ingredients: Fuling (Poria), Chuanxiong (Ligustici Chuanxiong Rhizoma), Qianghuo (Notopterygii Rhizoma et Radix), Jingjie Sui (Schizonepetae Spica), Fangfeng (Saposhnikoviae Radix [deprived of radix]), Huoxiang (Agastaches Herba), Jiangcan (Bombyx Batryticatus [stir-fried]), Chantui (Cicadae Periostracum [deprived of mouthparts and claws]), Gancao (Glycyrrhizae Radix et Rhizoma [roasted]), Houpo (Magnoliae Officinalis Cortex [processed]), Chenpi (Citri Reticulatae Pericarpium [deprived of albedo]). All ingredients in equal proportions.
Usage: Grind the above ingredients into fine powder. Take 0.5 fen each time, mixed with clear tea liquor; mint decoction may also be used. For acute or chronic convulsions, mix with milk resin (Ruxiang) and Schizonepeta decoction. Adding Xionghuang (Realgar) creates Xiongfeng San (Realgar Wind-Expelling Powder).
Bohe Tang (Mint Decoction): Treats nasal obstruction as well as cold damage complicated by fright, and extreme heat transformation fevers.
Ingredients: Bohe Ye (Menthae Haplocalycis Herba) (5 qian); Qianghuo (Notopterygii Rhizoma et Radix), Quanxie (Scorpio), Mahuang (Ephedrae Herba [deprived of joints]), Gancao (Glycyrrhizae Radix et Rhizoma), Jiangcan (Bombyx Batryticatus [stir-fried]) (each 1 qian); Tianzhu Huang (Calami Tuber), Baifuzi (Typhonii Rhizoma [roasted]) (each 2.5 qian).
Usage: Grind into powder. Mix with mint decoction. If extreme heat generates wind, add a small amount of Zhuli (Bambusae Caulis in Taenia). Another formula variant includes Chaihu (Bupleuri Radix), Taixiong (Chuanxiong), Jiegeng (Platycodonis Radix), and Fuling (Poria), but excludes Quanxie (Scorpio), Jiangcan (Bombyx Batryticatus), Tianzhu Huang (Calami Tuber), and Baifuzi (Typhonii Rhizoma).
[Zhang Huan] Xinyi Gao (Magnolia Flower Ointment)
Ingredients: Xinyi Ye (Flos Magnoliae Biondii [washed and dried]), Xixin (Asari Radix et Rhizoma), Mutong (Akebiae Caulis), Xiang Baizhi (Angelicae Dahuricae Radix), Muxiang (Aucklandiae Radix) (each half liang).
Preparation: Pound the above herbs and sieve into fine powder. Separately, take Xingren (Prunus Armeniacae Semen [soaked in hot water, peeled and crushed]). Mix all ingredients with 2 liang of sheep marrow and 2 liang of lard. Slowly simmer in a stone vessel until a reddish-yellow ointment forms. Cool, then add Longnao (Borneolum Syntheticum) and Shexiang (Moschus) (each 1 qian), and mix well. Apply a small amount into the nose. For nursing infants with nasal congestion due to the wet nurse blowing air onto the fontanelle, apply only to the fontanelle.
"Zhang Jifeng Fang" (Prescriptions of Zhang Jifeng): Treats unclosed fontanelle and nasal obstruction.
Preparation: Select one large Tiannanxing (Arisaematis Rhizoma), slightly roast and grind into powder. Mix with light vinegar, spread on red silk fabric, and apply to the fontanelle. Frequently warm and press with hands.
Zhuang Shi's Formula for Pediatric Nasal Congestion
Preparation: Grind Huaiye (Sophorae Tonkinensis Folium) into powder. Mix with the wet nurse's saliva and apply thickly to the fontanelle.
[Ji Shi] Congxian Gao (Scallion Saliva Ointment): Treats infants aged three to five days with nasal congestion, rapid breathing, and incessant crying during feeding.
Ingredients: Congye (Allii Fistulosi Bulbus), Zhu Ya Zao Jiao (Gleditsia Sinensis Fruit [ground into powder, peeled] - seven strips each).
Preparation: Pulp the ingredients and mix with the Zaojiao powder to form an ointment. Apply to the anterior fontanelle until cured.
